The effect of varus and valgus osteotomies on femoral version.

BACKGROUND: Although seldom described, varus and valgus osteotomies of the proximal femur can affect femoral version. The magnitude of the effect can be predicted with an understanding of the distinction between femoral version and femoral neck inclination. The purpose of this study was to elucidate this relationship. METHODS: Version, inclination, apparent neck-shaft angle, and true neck-shaft angle were defined and measured in 72 preserved femora. RESULTS: Average values were 19.8+/-11.8 degrees for version versus 14.5+/-10.1 degrees for inclination (P=0.004), giving a mean difference of 2

Hip Fractures after Intramedullary Nailing Fixation for Atypical Femoral Fractures: Three Cases.

Secondary hip fractures (SHFs) rarely occur after intramedullary nailing (IMN) fixation without femoral neck fixation for atypical femoral fractures (AFFs). We report three cases of older Japanese women who sustained SHFs presumably caused by osteoporosis and peri-implant stress concentration around the femoral neck after undergoing IMN without femoral neck fixation for AFF. All cases were fixed with malalignment. In AFF patients, postoperative changes due to postoperative femoral bone malalignment may affect the peri-implant mechanical environment around the femoral neck, which can result in

[Static metatarsalgia].

Static metatarsalgia involves pain of non-inflammatory origin in the region of the metatarsal heads. It is caused by a functional disorder or anatomic derangement of the architecture over the ball of the foot, whether congenital or acquired, evident or not. Clinical examination, including of the shoe and of the plantar orthosis, distinguishes five types of anomalies: 1. horizontal malalignment of the metatarsal heads with insufficiency at the first metatarsal-phalangeal joint, dominated by hallux valgus, and involvement of the second metatarsal bone, sometimes favouring Freiberg's disease; 2.

Congenital onychodysplasia of the index fingers--Iso-Kikuchi syndrome. A case involving the second toenail.

We report a case of congenital onychodysplasia of the index fingers (Iso-Kikuchi syndrome) in a 2-year-old boy who had nail deformities on both index fingers and the left second toe. He had a micronychia of the left index fingernail, malalignment and abnormal lunula of the right index fingernail and micronychia and malalignment of the left second toenail. Congenital onychodysplasia of the index fingers (COIF) is a rare condition characterized by various forms of nail dysplasia commonly involving the index fingers, but not infrequently also the neighbouring fingers such as the middle fingers an

Carpal Fractures Other than Scaphoid in the Athlete.

Carpal fractures of bones other than the scaphoid occur at a much lower rate than scaphoid fractures. The close relationship between the carpus, intrinsic and extrinsic wrist ligaments, and wrist kinematics makes a thorough history, clinical examination, and interpretation of imaging for carpal malalignment essential. Carpal malalignment should be addressed with reduction and fixation. Nondisplaced fractures are often treated nonoperatively and displaced intraarticular fractures are almost always treatment operatively. The physician should keep in mind the athlete's specific goals and needs. T

Editorial Commentary: Increased Tibial Slope and Decreased Medial Proximal Tibial Angle Negatively Affect Anterior Cruciate Ligament Graft Maturation: Objective Evidence on When to Add a Lateral Extra-Articular Augmentation Procedure to a Soft-Tissue Anterior Cruciate Ligament Reconstruction.

Recent anterior cruciate ligament (ACL) research focuses on risk factors for ACL graft failure and techniques and augmentations to limit failure. One of the most recognized risk factors is sagittal malalignment in the form of high posterior tibial slope (PTS), especially PTS ≥12°, which leads to increased force through the ACL and ACL graft. To reduce the risk associated with increased PTS, lateral augmentation techniques, typically either a lateral extra-articular tenodesis or an anterolateral ligament reconstruction, improve clinical outcomes, and the authors preferred graft choice, particul

Relationship of meniscal damage, meniscal extrusion, malalignment, and joint laxity to subsequent cartilage loss in osteoarthritic knees.

OBJECTIVE: Progressive knee osteoarthritis (OA) is believed to result from local factors acting in a systemic environment. Previous studies have not examined these factors concomitantly or compared quantitative and qualitative cartilage loss outcomes. The aim of this study was to test whether meniscal damage, meniscal extrusion, malalignment, and laxity each predicted tibiofemoral cartilage loss after controlling for the other factors. METHODS: Laxity and alignment were measured at baseline in individuals with knee OA. Magnetic resonance imaging included spin-echo coronal and sagittal imaging

Axial alignment is a critical regulator of knee osteoarthritis.

Although osteoarthritis (OA), a leading cause of disability, has been associated with joint malalignment, scientific translational evidence for this link is lacking. In a clinical case study, we provide evidence of osteochondral recovery upon unloading symptomatic isolated medial tibiofemoral knee OA associated with varus malalignment. By mapping response correlations at high resolution, we identify spatially complex degenerative changes in cartilage after overloading in a clinically relevant ovine model. We further report that unloading diminishes OA cartilage degeneration and alterations of

Combined laminoplasty with posterior lateral mass plate for unstable spondylotic cervical canal stenosis--technical note.

A technique of combined expanding laminoplasty using longitudinal interspinous iliac bone graft with posterior lateral mass plate is described for the treatment of cervical canal stenosis associated with spinal instability. A 52-year-old male and a 76-year-old female presented with cervical myelopathy. Imaging studies demonstrated spondylotic cervical canal stenosis associated with spinal instability. Posterior stabilization with lateral mass plate by the Axis Fixation System was performed after expanding laminoplasty using interspinous iliac bone graft. The symptoms improved and instability a

What Are the Optimal Sagittal Alignments in Primary Total Knee Arthroplasty: A Systematic Review and Meta-Analysis.

OBJECTIVE: Total knee arthroplasty (TKA) is a well-established intervention for end-stage osteoarthritis (OA), offering substantial pain relief and functional improvement. However, a considerable proportion of patients remain dissatisfied postoperatively due to multifactorial causes. While numerous studies have investigated implant alignment, the sagittal plane alignment has received comparatively less attention, and its clinical relevance remains controversial. This systematic review aims to identify the optimal sagittal alignment (SA) parameters in TKA and to evaluate their impact on clinica

Is our current paradigm for evaluation and management of the bunion deformity flawed? A discussion of procedure philosophy relative to anatomy.

Of the >100 procedures that have been proposed to treat hallux valgus or the "bunion" deformity, most have focused on correction through metatarsal osteotomies at various levels combined with soft tissue balancing procedures at the first metatarsophalangeal joint. This paradigm of metatarsal osteotomy and soft tissue balancing has been so commonplace, any argument for a fundamental change to the approach becomes uncomfortable and seems unwarranted to most foot and ankle surgeons. However, the simple fact that so many procedures exist, with so many modifications of these procedures, can be inte

We recommend gradual correction of distal tibia varus as most applicable for patients with severe deformities that would not be adequately corrected with acute methods. Complex deformity, compromised soft tissues, and limb shortening are, in general, better managed with this technique.

Varus malalignment of the talar neck. Its effect on the position of the foot and on subtalar motion.

We performed an in vitro study on twelve specimens of the foot and ankle from cadavera to determine whether varus malalignment of the talar neck alters the position of the foot and subtalar motion. An osteotomy of the talar neck was performed, and the specimens were studied with and without removal of a medially based wedge of bone. Removal of the wedge produced an average varus malalignment of the talar neck of 17.1 +/- 2.4 degrees (range, 12.5 to 21.0 degrees). In the coronal plane, the average arc of motion of the subtalar joint decreased from 17.2 +/- 3.3 degrees before the osteotomy to 11

An analytical investigation into possible mechanical causes of bone remodelling.

Three-dimensional finite element analyses were carried out on idealisations of two situations where bone remodelling has been reported clinically viz. the malaligned femur and a maxillary central incisor under orthodontic loading. The mechanical responses, i.e. stress, strain and change of curvature, in various directions, are presented. They are compared with the biological response, either bone resorption or bone deposition. The results are discussed in relation to three current theories of bone remodelling.

Radiographic cortical thickness parameters as predictors of rotational alignment in proximal femur fractures: A cadaveric study.

Radiographic assessment tools such as the cortical step sign (CSS) or the diameter difference sign (DDS) aim to identify clinically relevant rotational malalignment after long bone fracture fixation. We aimed to analyze the effect of rotational malalignment on CSS and DDS parameters in a subtrochanteric fracture model and to construct a prognostic model to identify clinically relevant rotational malalignment. A subtrochanteric transverse osteotomy was set in human femora. Rotation was set stepwise from 0° to 30° in internal and external rotation. Images were obtained using a C-arm and transfer

Varus ankle and osteochondral lesions of the talus.

Considerable recent orthopedic literature is dedicated to either the surgical management of OLTs and correction of varus ankle alignment, but little is published on the combination of these 2 problems. We anticipate that future clinical and biomechanical research will address their simultaneous treatment; until then, we will continue to extrapolate from their independent management.

Coronal malalignment of lower legs depending on the locations of the exostoses in patients with multiple hereditary exostoses.

BACKGROUNDS: Though malalignment of lower legs is a common pathologic phenomenon in multiple hereditary exostoses (MHE), relationship between locations of exostoses and malalignment of lower legs remains unclear. This study examined radiographs of MHE patients in an attempt to evaluate the tendency of coronal malalignment of lower legs with different location of exostoses on lower legs consisting of two parallel long bones. METHODS: Between 2000 and 2017, we retrospectively reviewed the anteroposterior films of the teleo-roentgenographics of 63 patients with MHE. The patients were classified

The Poller Screw Technique: A Method of Fine-Tuning the Reduction in Locked Nailing.

Intramedullary nailing is the treatment of choice for diaphyseal fractures in long bones. However, nailing of long bone fractures at the metaphyseodiaphyseal junction is technically difficult and can cause malalignment because of the mismatch in the diameter of the bone. One of the most common and recently described methods of correcting deformity during nailing is the poller screw technique. We describe a modified technique to correct malreduced fractures with the nail in situ, which we have used successfully in 3 patients.

Outcomes of periprosthetic distal femur fractures following total knee arthroplasty: Intramedullary nailing versus plating.

BACKGROUND: Periprosthetic distal femur fractures (PPDFFs) present a challenge in terms of optimizing fixation in patients with poor bone quality and limited bone stock. The main treatment options include laterally based plating and intramedullary nailing. We hypothesized that treatment of PPDFFs with intramedullary nails would result in improved union rate, fewer complications, and an equivalent rate of malalignment compared to plating. MATERIALS AND METHODS: Cases of PPDFFs were identified through a query of our institutional trauma database between 2011-2018. Adult patients (>18 years) wer
