Background and purpose - Insufficient initial fixation or early micromotion of an implant is associated with a thin layer of fibrous tissue at the peri-implant interface. It is unknown if bone loss is induced by the fibrous tissue interface acting as an active biological membrane, or as a membrane that will produce supraphysiologic fluid flow conditions during gait, which activates the mechanosensitive osteocytes to mediate osteoclast differentiation. We investigated whether mechanically induced osteolysis is dependent on the fibrous tissue interface as a biologically active scaffold, or if it

BACKGROUND: Alcohol negatively affects bone health and fracture repair, yet its effects on implant osseointegration remain poorly understood. This in vivo study utilized a previously developed murine model to evaluate implant osseointegration before and after continuous ethanol consumption.
METHODS: Adult mice were given regular drinking water (control group) or 10% to 15% ethanol in their drinking water for 3 months before undergoing surgical implantation of a load-bearing, porous titanium implant in the proximal tibia. Ethanol consumption was continued until euthanasia at 4 weeks post-impla

PURPOSE: This study evaluated the bone mineral apposition rate (MAR) at the bone-implant interface region of alumina-blasted/acid-etched (AB/AE), plasma-sprayed hydroxyapatite (PSHA), and nanometric-scale bioceramic-coated surfaces at early implantation times in a dog tibia model.
MATERIALS AND METHODS: Implants (n = 12 per group) with three different surfaces-AB/AE, PSHA, and a bioceramic coating in the 300- to 500-nm thickness range-were placed bilaterally along the proximal tibiae of six male beagles. Implants remained for 3 and 5 weeks in vivo. Ten and 2 days prior to euthanization, calce

PURPOSE: Implant loosening in tibia after primary total ankle replacement (TAR) is one of the common postoperative problems in TAR. Innovations in implant structure design may ideally reduce micromotion at the bone-implant interface and enhance the bone-implant fixation and initial stability, thus eventually prevents long-term implant loosening. This study aimed to investigate (1) biomechanical characteristics at the bone-implant interface and (2) the influence of design features, such as radius, height, and length.
METHODS: A total of 101 finite-element models were created based on four comm

The initial fixation of an anatomical cementless glenoid component, provided by different numbers and types of screws, and the risk of bone fracture were evaluated by estimating the bone-implant interface micromotions and the principal strains around the prosthesis. Four different fixation configurations using locking or compression screws were tested. Estimation of the micromotions at the bone-implant interface was performed both experimentally, using an in vitro model, and computationally, using a numerical model. Principal bone strains were estimated using the numerical model. Subject varia

BACKGROUND: Tibial bone fractures in the malleolar regions are a major concern during the early postoperative period of total ankle replacement (TAR), affecting patient outcomes such as stability and recovery. Design, placement, and anatomic misalignment of implant components can contribute to malleolar fractures. The aim of this study is to understand the influence of implant design features, including keel, peg, stem, and bar type design, and bone-implant interfacial conditions on malleolar fracture following TAR.
METHODS: Three-dimensional finite element (FE) models were generated for the

The percutaneous osseointegrated (OI) prostheses have greatly improved the overall quality of life for amputees. However, the long-term maintenance of the OI prostheses is still challenging. A major problem is bone resorption around the bone-implant-skin interface, which might cause implant loosening or osteomyelitis. Another problem is the breakage of connecting components between the intramedullary implant and external prosthesis due to excessive stress. We designed a novel osseointegration implant by changing the bone-implant contact from the inner cortex to the outer surface of cortical bo

This study investigated the effects of the stiffness of a maxillary sinus graft and the surface roughness of an immediately loaded implant using a non-linear three-dimensional finite element (FE) analysis (3D). Six FE models were created, including two stiffness values of grafted bone (345 and 3450 MPa of elastic modulus) and three conditions of implant-bone interfaces (Frictional coefficient of 0.3 for machined surface, 0.45 for rough implant surface and a bonded implant-bone interface for an osseointegrated implant). Computer tomographic images of a human skull were used to construct a poste

INTRODUCTION: The aim of the work is to analyze stress distribution on 3D Finite Element (FE) models at bone, implant, and framework level of different designs for fixed implant-supported prostheses in completely edentulous patients, comparing results on whole and partially resected mandibles.
MATERIALS AND METHODS: 3D anisotropic FE models of a whole and of a partially resected mandible were created using a TC scan of a cadaver's totally edentulous mandible. Two types of totally implant-supported rehabilitation were simulated, with four implants: parallel fixtures on whole mandible and on re

Achieving a long-term stable implant interface is a significant clinical issue when there is insufficient cortical bone stabilisation at implant placement. Clinical outcomes studies suggest that the higher risk implants are those placed in compromised cortical bone (thin, porous, etc.) in anatomical sites with minimal existing trabecular bone (characterised as type IV bone). In establishing and maintaining an implant interface in such an environment, one needs to consider the impact of masticatory forces, the response of bone to these forces and the impact of age on the adaptive capacity of bo

A total of 2895 threaded, cylindrical titanium implants have been inserted into the mandible or the maxilla and 124 similar implants have been installed in the tibial, temporal or iliac bones in man for various bone restorative procedures. The titanium screws were implanted without the use of cement, using a meticulous technique aiming at osseointegration--a direct contact between living bone and implant. Thirty-eight stable and integrated screws were removed for various reasons from 18 patients. The interface zone between bone and implant was investigated using X-rays, SEM, TEM and histology.

We conducted a 2-year histologic and histometric evaluation of the tibial bone-titanium (Ti) implant interface in male rats. Thirty male 6-week-old rats were used in this study. They were divided into two groups: 15 for day 28 and 15 for day 730. Microscopic observation at day 28 revealed that the newly formed bone around the implant almost surrounded the implant, but fibroblastlike cells were interposed in some histologic sections. At day 730, in contrast, such cells were rarely seen, and the bone around the implant presented a lamellar structure. Transmission electron microscopic observation

BACKGROUND: Osseointegrated implants for transfemoral amputees facilitate direct load transfer between the prosthetic limb and femur; however, implant loosening is a common complication, and the associated implant-bone loads remain poorly understood. This case study aimed to use patient-specific computational modeling to evaluate bone-implant interface loading during standing and walking in a transfemoral amputee with an osseointegrated implant prior to prosthesis loosening and revision surgery.
METHODS: One male transfemoral amputee with an osseointegrated implant was recruited (age: 59-yrs,

The metal stem of the totally replaced hip carries load and resists fatigue, but it is electrochemically corroded. Metallic atoms act as haptens, induce type 1 T-helper cells/Th1-type immune responses and enhance periprosthetic osteolysis. Stiff metal implants, which do not have the same elasticity as the surrounding bone, cause stress shielding. Cyclic loading and lack of ligamentous support lead to mechanical and ischemia reperfusion injury and particle formation from bone, polymethylmethacrylate, and porous implant surfaces, which accelerate third-body polyethylene wear. Surgical injury and

The goal of placement of endosseous dental implants is to achieve osseointegration or biointegration of the bone with the implant. A wide variety of materials has been used for these implants, but only a few promote osseointegration and biointegration. Titanium and titanium alloy (Ti6A14V) have been the most widely used of these materials. The surface oxide of titanium appears to be central to the ability of this material to osseointegrate. The oxide limits dissolution of elements and promotes the deposition of biological molecules which allow bone to exist as close as 30 A to the surface of t

The aim of this study was to examine the bony interface underlying a femoral hemiresurfacing implant. The specimen was subjected to radiographic and histologic analyses. The presence of a cement mantle was not uniform throughout the interface. When cement was evident, trabecular bone was found interdigitated within the mantle. When cement was not present, a fibrous tissue membrane was detected between the metal surface of the implant and the host bone. Disruption of the bone stock surrounding the stem was observed. While the reason for the revision was for relief of severe pain for acetabular

In total knee arthroplasty (TKA), force is transmitted into the tibia by a combined plate-stem device along with cemented or cementless stem fixation. The present work analyzes this force transmission in finite element simulations with the main aim to avoid reported postsurgical bone density reduction as a consequence of a reduced tibial bone loading. In the numerical analysis different implant materials, stem/extension lengths and implant-to-stem interface conditions are considered, from a stiff fully cemented fixation to sliding contact conditions with a low friction coefficient. The impact

Little is known about the in vivo healing processes at the interface of implants placed in different grafting materials. For optimal sinus augmentation, a bone graft substitute that can regenerate high-quality bone and enable the osseointegration of load-bearing titanium implants is needed in clinical practice. Calcium sulphate (CaS) is one of the oldest biomaterials used in medicine, but few studies have addressed its use as a sinus augmentation material in conjunction with simultaneous implant placement. The aim of the present study was to histologically evaluate an immediately loaded provis
