ncRNAs in breast milk of mothers with SLE: potential implications for neonatal immune development.

Systemic lupus erythematosus (SLE) is an autoimmune disease primarily characterized by the production of autoantibodies that induce tissue damage. Although the etiology of the disease remains unclear and may be attributed to numerous factors, various studies emphasize the role of epigenetic mechanisms in SLE. In particular, alterations in non-coding RNA (ncRNA) expression have been identified as potential causes as well as promising biomarkers of the disorder. ncRNAs are a diverse group of molecules involved in the regulation of gene expression. Due to their accessibility in bodily fluids, suc

Exosomes derived from colostrum and mature human breast milk protect against experimental necrotizing enterocolitis.

OBJECTIVE: Human milk-derived exosomes can protect intestinal organoids from lipopolysaccharide induced injury. The aim of this study is to investigate effects of exosomes derived from different periods of lactation on intestinal injury caused by experimental necrotizing enterocolitis (NEC). METHODS: Colostrum and mature milk from healthy lactating human mothers were collected and isolated exosomes using serial ultracentrifugation and filtration. NEC was induced in mice pups by hypoxia, gavage of feeding of formula, and lipopolysaccharide (LPS) administration between postnatal days 5 and 9. B

The effects of formula milk on development, nutrient absorption, and immune characteristics of piglets.

BACKGROUND: Breast milk is widely regarded as the optimal nourishment for infants. However, due to physiological or psychosocial factors, many infants do not receive adequate breastfeeding. So, the formula milk is proposed as a substitute of breast milk, and an increasing number of infants are being formula-fed. Numerous studies have highlighted the difference between breast and formula feeding, but the underlying mechanisms remain elusive. Given the ethical constraints in conducting experimental studies on human breastfeeding, we employed pig, which have similar physiological structure with h

Comparison of Milk Removal Efficiency and Maternal Perception Between a Hospital-Grade and Personal-Use Pump.

Background/Objectives: Breast pumps are essential tools when breastfeeding dyads are separated or there are breastfeeding challenges that prevent or reduce effective milk removal. Hospital-grade and personal-use pumps offer different features and performance characteristics. Direct comparative studies evaluating pump efficacy and maternal satisfaction are limited in this area, hampering healthcare professionals in their support of lactating women. We compared the efficacy and maternal perceptions of a hospital-grade breast pump (Symphony; Medela AG) and an electric personal-use pump (Pump in S

Evaluation of serum retinol, the modified-relative-dose-response ratio, and breast-milk vitamin A as indicators of response to postpartum maternal vitamin A supplementation.

BACKGROUND: Conflicting results have been reported regarding the relative performance of serum retinol, the modified-relative-dose-response (MRDR) ratio, and breast-milk vitamin A concentrations in detecting changes in maternal vitamin A status. OBJECTIVE: We used receiver operating characteristic analyses and standardized differences to compare the ability of these indicators to detect a response to postpartum vitamin A supplementation in lactating Bangladeshi women. DESIGN: At 2 wk postpartum, women were randomly assigned to receive either a single dose of vitamin A [200000 IU (60000 retin

Breast milk cellular HIV-specific interferon γ responses are associated with protection from peripartum HIV transmission.

OBJECTIVE: Breast milk is a major route of infant HIV infection, yet the majority of breast-fed, HIV-exposed infants escape infection by unknown mechanisms. This study aimed to investigate the role of HIV-specific breast milk cells in preventing infant HIV infection. DESIGN: A prospective study was designed to measure associations between maternal breast milk HIV-specific interferon-γ (IFN-γ) responses and infant HIV-1 detection at 1 month of age. METHODS: In a Kenyan cohort of HIV-infected mothers, blood and breast milk HIV-gag IFN-γ ELISpot responses were measured. Logistic regression was

Amniotic fluid and breast milk: a rationale for breast milk stem cell therapy in neonatal diseases.

Amniotic fluid and breast milk play important roles in structural development throughout fetal growth and infancy. Given their significance in physical maturation, many studies have investigated the therapeutic and protective roles of amniotic fluid and breast milk in neonatal diseases. Of particular interest to researchers are stem cells found in the two fluids. These stem cells have been investigated due to their ability to self-replicate, differentiate, reduce tissue damage, and their expression of pluripotent markers. While amniotic fluid stem cells have received some attention regarding t

Transmission of community-associated methicillin-resistant Staphylococcus aureus from breast milk in the neonatal intensive care unit.

Methicillin-resistant Staphylococcus aureus (MRSA) has become increasingly common in neonatal intensive care units and can lead to severe outcomes. Baby C, of a set of quadruplets, died of MRSA sepsis. The surviving siblings were colonized with MRSA. Expressed breast milk was fed to all infants; tested breast milk samples were all MRSA-positive. Pulsed field gel electrophoresis results of isolates from the infants and breast milk were indistinguishable.

Effect of warm breastshields on breast milk pumping.

The effect of using a warm breastshield on the efficiency, effectiveness, and comfort of expressing milk with an electric breast pump was assessed. The authors tested a standard breastshield at 25°C and a breastshield warmed to 39°C on 25 mothers using their maximum comfortable vacuum or -125 mm Hg. Using a warm breastshield effectively warmed the nipple and areola and, combined with maximum comfortable vacuum, decreased the time to remove 80% of the total milk yield and increased the percentage of available milk removed after 5 minutes of expression, with no change in the percentage of availa

Measurement of milk yield in women.

The measurement of milk yield in any suckling mammal is difficult, and studies on women pose additional restrictions because of the limitations on the nature of the techniques which can be used. The most commonly used method for determining milk yield in women is test weighing. In this method either the infant or the mother is weighed before and after a breast feed, and the difference between the two weights is considered to be the amount of milk produced. The reliability of the results obtained by the test weighing procedure depends on both the interval over which milk yield is recorded and t

Insights Into Policies, Stakeholders, and Governance of Breast Pump Provision in Germany.

In Germany, low breastfeeding rates are a public health concern, undermining long-term health benefits for both mothers and children. To address this, some public health and workplace policies supporting breastfeeding and milk expression have already been established. This paper examines policies in Germany related to breastfeeding and human milk expression, with a focus on breast pumps as a key medical technology, and explores pathways for accessing breast pumps. Stakeholders relevant for governing aspects around breastfeeding at work and breast pumps in Germany were identified. In addition,

Neon Pink Breast Milk Stain in the NICU.

Pink discoloration in breast milk is a rare phenomenon arising from various benign or pathological causes. Serratia marcescens, a gram-negative bacterium known for its characteristic red pigment prodigiosin, is a notable cause. This case report examines the clinical approach, diagnostic workup, microbiological findings, and management strategies for a neonate presenting with neon pink stains in breast milk. A female neonate born at 30+5 weeks gestation via repeat cesarean section was admitted to the neonatal intensive care unit (NICU). On day of life (DOL) 28, bright pink staining was observed

[Herpesviruses and breast milk].

Breast milk has always been the best source of nourishment for newborns. However, breast milk can carry a risk of infection, as it can be contaminated with bacterial or viral pathogens. This paper reviews the risk of acquisition of varicella-zoster virus (VZV) and cytomegalovirus (CMV), herpesviruses frequently detected in breastfeeding mothers, via breast milk, focusing on the clinical consequences of this transmission and the possible strategies for preventing it. Maternal VZV infections are conditions during which breastfeeding may be temporarily contraindicated, but expressed breast milk s

Birth to breast--a feeding care map for the NICU: helping the extremely low birth weight infant navigate the course.

Breast milk has been shown to contribute significantly to positive neurodevelopmental and medical outcomes in the extremely low birth weight (ELBW) infant population. It is crucial that ELBW infants receive their mother's colostrum as a first feeding, followed by expressed breast milk for as long as possible. Evidence-based literature supports the difficult challenges both mothers and ELBW infants face if they are to succeed at breast pumping and breastfeeding. Influencing factors include the medical fragility of the infant, limited frequency and duration of kangaroo care between mother and in

Differences between neonatal units with high and low rates of breast milk feeding for very preterm babies at discharge: a qualitative study of staff experiences.

BACKGROUND: Breast milk has significant benefits for preterm babies, but 'very preterm' babies are unable to feed directly from the breast at birth. Their mothers have to initiate and sustain lactation through expressing milk for tube feeding until their babies are developmentally ready to feed orally. There are wide disparities between neonatal units in England in rates of breast milk feeding at discharge. This study explored health professionals' experiences of barriers and facilitators to their role in supporting breast milk feeding and breastfeeding for very preterm babies. METHODS: 12 he

Circadian Variation of Breast Milk Components and Implications for Care.

Several components of breast milk show circadian variability. It is likely that at least some of these macronutrients, hormones, and micronutrients produce circadian stimuli that enhance the well-being of breast-fed infants. Future research should determine whether high-risk infants benefit if breast milk is given during the same circadian phase as it was expressed.

Transcriptome and metabolome analyses reveal the effects of formula and breast milk on the growth and development of human small intestinal organoids.

Breast milk is widely acknowledged as the ideal nutritional resource for infants and can well meet the nutritional requirements for baby's growth and development. Infant formula is a substitute for breast milk, designed to closely mimic its composition and function for breast milk. Most of the previous studies used tumor colorectal cancer cell lines to study the nutritional potency of formula and its components, so realistic data closer to the baby could not be obtained. Small intestinal organoids, derived from differentiated human embryonic stem cells, can be used to simulate nutrient absorpt
