Bronchioles are noncartilaginous small airways with internal diameter of 2 mm or less, located from approximately the eighth generation of purely air conducting airways (membranous bronchioles) down to the terminal bronchioles (the smallest airways without alveoli) and respiratory bronchioles (which communicate directly with alveolar ducts and are in the range of 0.5 mm or less in diameter). Bronchiolar injury, inflammation, and fibrosis may occur in myriad disorders including connective tissue diseases, inflammatory bowel diseases, lung transplant allograft rejection, graft versus host diseas

Viral bronchiolitis is the most common cause of hospitalization among infants. Despite its prevalence, no consistently effective therapy has been found to date, providing the driving force behind much of the ongoing research into this illness. In this review, we present a summary of the most recent published trials of interventions for bronchiolitis. Included are studies evaluating bronchodilators, corticosteroids, positive pressure ventilation, as well as 3 newer therapies for bronchiolitis: heliox, mucolytics, and leukotriene receptor antagonists.

OBJECTIVES: The study was undertaken to assess whether previously healthy infants with bronchiolitis develop elevated pulmonary artery pressures.
MATERIAL AND METHODS: We performed 2D and Doppler echocardiography in 94 infants, aged between 2 and 12 months, from November 2007 to December 2009. 47 infants had bronchiolitis and 47 were in the control group. We excluded children with congenital heart diseases and other conditions associated with pulmonary hypertension. The method of time to peak velocity corrected for heart rate was used to estimate pulmonary arterial pressure (PAP). A Student's

Respiratory syncytial virus, the most common cause of bronchiolitis, is the leading cause of infant hospitalization in developed countries and accounts for substantial mortality and morbidity in developing countries. Children at increased risk of developing severe bronchiolitis are those <6 weeks of age, those born prematurely and those with an underlying cardiopulmonary disorder or immunodeficiency. Approximately 80% of cases occur in the first year of life. By two years of age, virtually all children have been infected by at least one strain of the virus. Classically, respiratory syncytial v

Bronchiolitis is a common, acute, contagious lower respiratory tract illness of infants and young children. The majority of cases are secondary to respiratory syncytial virus. There are a number of risk factors for severe disease, including children less than six weeks of age and patients with atopy and/or asthma. The management requires vigilant monitoring and high-quality supportive care, including impressive use of fluids and nutritional support. Further research on anti-virals is essential to prevent respiratory syncytial virus induced bronchiolitis. Fortunately, the prognosis for the majo

Relationships between lower respiratory diseases with wheezing during infancy and asthma at school-age remain debatted. Indeed, although epidemiologic studies show that wheezy infants most often become asymptomatic before entry to school, they also demonstrate that severe bronchiolitis during infancy is a significant risk factor for persistence of wheezing episodes at school-age. One of the most important factors demonstrated to enhance the risk for persistence of asthma is an early allergic sensitization.

Bronchiolitis is the most common disease of the respiratory tract during the first year of life, and occurs in annual epidemics in winter. The etiology is viral, and respiratory syncytial virus (RSV) is the commonest agent. Respiratory symptoms remain generally mild, and treatment just supportive and at home. Certain infants are at high risk of severe illness (age less than 3 months, preterm birth, neonatal respiratory disease, bronchopulmonary dysplasia, underlying chronic diseases), and require hospitalisation. Most treatments are of unproved (corticosteroids), or limited benefit (inhaled br

Respiratory syncytial virus (RSV) bronchiolitis is associated with the clinical signs and symptoms of small airway obstruction. A major public health problem throughout the world, this condition is responsible for significant morbidity and mortality. Management is primarily preventive, through strict hand washing, avoidance of exposure during the respiratory illness season and intravenously administered prophylactic anti-RSV Immune globulin, especially in selected small infants with underlying cardiopulmonary disease. Supportive measures, including fluid hydration, good nutrition, aerosolized

Bronchiolitis obliterans organizing pneumonia (BOOP) is increasingly recognized as an important cause of diffuse infiltrative lung disease. It is a diagnostic consideration in patients with a febrile flu-like illness of a few weeks' duration and a roentgenogram showing bilateral patchy infiltrates that are not responsive to a typical course of antibiotics. It is defined as granulated tissue plugs within lumens of small airways that extend into alveolar ducts and alveoli. Clinically, a flu-like illness, cough, and crackles are common. Pulmonary function studies of patients show a decreased vita

Bronchiolitis is an acute viral infection primarily caused by respiratory syncytial virus, affecting children under 2 years of age with a peak prior to six months. The clinical picture results from an inflammatory process of the small bronchi and bronchioles. Infants present with tachypnea and wheezing often accompanied by respiratory distress and hyperinflation. Supportive care is essential, children respond slowly to hydration and careful observation. Respiratory distress requires hospitalization with treatment including oxygen and ventilator support as indicated.

BACKGROUND: Follicular bronchiolitis (FB) is an uncommon but important pulmonary manifestation in children.
METHODS: In this report, we present the clinical presentations and histopathological data of an 8-year-old boy with FB.
RESULTS: The patient had a history of recurrent cough and dyspnea for 5 years with progressive worsening of symptoms. An initial pulmonary function test showed an obstructive ventilatory defect. Chest X-ray demonstrated miliary nodules. High-resolution computed tomography showed reticulonodular opacification and central consolidation. Histopathological examination rev

BACKGROUND: Bronchiolitis is a serious, potentially life-threatening respiratory illness commonly affecting babies. It is often caused by respiratory syncytial virus (RSV). Antibiotics are not recommended for bronchiolitis unless there is concern about complications such as secondary bacterial pneumonia or respiratory failure. Nevertheless, they are used at rates of 34% to 99% in uncomplicated cases.
OBJECTIVES: To evaluate the effectiveness of antibiotics for bronchiolitis.
SEARCH STRATEGY: We searched the Cochrane Central Register of Controlled Trials (CENTRAL 2010, issue 4), which include

The pathogenesis, epidemiology, clinical features, sequelae, and treatment of bronchiolitis are reviewed. Acute bronchiolitis is the most common severe lower-respiratory-tract infection of infancy. During epidemics, more than 80% of cases may be caused by respiratory syncytial virus (RSV). Although signs and symptoms may become severe, most infections are self-limited and improvement occurs within several days. Approximately 1-2% of infants less than one year of age require hospitalization. Generally, patients who develop severe, life-threatening RSV bronchiolitis are those with underlying car

BACKGROUND: Mannose-binding lectin (MBL) encoded by the MBL2 gene, is an important component of the innate immunity. Low levels have been linked with respiratory infections and both high and low levels with allergy and asthma. The aims of the study were to evaluate the connection between polymorphisms of the MBL2 gene and viral findings, clinical characteristics and subsequent wheezing in young infants with bronchiolitis.
METHODS: In all, 129 full-term infants hospitalized for bronchiolitis at age less than 6 months have been followed-up until the mean age of 1.5 years. The genotyping of the

BACKGROUND AND OBJECTIVE: Postinfectious bronchiolitis obliterans (PIBO) is an uncommon and severe form of chronic obstructive lung disease in children. Adenovirus (ADV) is the main pathogen of PIBO in children. However, the risk factors of ADV-PIBO are not fully understood. This study aims to analyze the clinical characteristics of PIBO caused by ADV and compared with non-ADV-PIBO.
MATERIALS AND METHODS: A retrospective study of children under the age of 14 years diagnosed PIBO was performed in 56 ADV-PIBO cases, 29 non-ADV-PIBO, and 39 healthy controls to determine clinical characteristics

A review of the evidence on prevention of acute bronchiolitis is presented. Acute bronchiolitis prevention arises from three basic approaches: preventive treatment to reduce recurrent wheezing following an episode of acute bronchiolitis, preventive treatment to reduce the frequency and severity of RSV bronchiolitis in the population at risk (prematurity, bronchopulmonary dysplasia, congenital heart disease, etc.), and general preventive measures to reduce nosocomial infection with RSV. There is sufficient evidence on the lack of efficacy of inhaled corticosteroids, oral corticosteroids and mon
