Adverse clinical consequences of neodymium:YAG laser treatment of posterior capsule opacification.

PURPOSE: To evaluate the clinical consequences of complications from neodymium:YAG (Nd:YAG) laser capsulotomy for posterior capsule opacification (PCO) over the lives of cataract patients. SETTING: CHU de Caen, Caen, France. METHODS: A model was constructed to estimate the lifetime clinical consequences of postoperative PCO treated by Nd:YAG laser capsulotomy. The probability of death was modeled from French mortality tables as a polynomial function of sex and age. The probability of becoming blind was modeled as a function of age from data in the literature. The incidence of Nd:YAG complica

METHODS: The PCO density value in 100 consecutive diabetic patients and in 100 nondiabetic age-matched control patients who underwent cataract surgery was measured using a Scheimpflug videophotography system at 1 week and at 3, 6, 12, 18, 24, 30, and 36 months after surgery. The incidence of neodymium:yttrium-aluminum-garnet (Nd:YAG) laser posterior capsulotomy was also evaluated. Furthermore, the

The impact of capsulorhexis diameter, localization and shape on posterior capsule opacification.

BACKGROUND: The aim of this study was to evaluate the impact of capsulorhexis diameter, localization and shape on posterior capsule opacification (PCO) development after cataract extraction with phacoemulsification. MATERIAL/METHODS: We retrospectively analyzed of 297 patients who underwent phacoemulsification and AcrySof SA60AT implantation. In a first group of 97 patients, 53 received small capsulorhexis (3.9 to 4.9 mm in diameter) and 44 patients received large capsulorhexis (5.0 to 5.9 mm in diameter). Another group of 99 patients was split into subgroups--66 patients whose capsulorhexis

[The surgical treatment for posterior capsule opacification in children with pseudophakic eyes].

OBJECTIVE: To evaluate the surgical technique and outcome of pars plana capsulotomy and vitrectomy with infusion through the limbus to remove the posterior capsule opacification (PCO) in children with pseudophakic eyes. METHODS: This technique was performed in 51 cases (57 eyes) of complicated PCO with pseudophakic eyes, which was almost impossible to remove through the Nd:YAG capsulotomy. Pre- and post-operative visual acuity, intraocular pressure, density of cornea endothelium and complications during and after the operation were observed. The mean follow up time was 30 months. RESULTS: Th

Predictors of Successful Laser Capsulotomy for Significant Posterior Capsule Opacification after Phacoemulsification.

PURPOSE: This study aimed to determine the reasons behind the failure of laser capsulotomy (LC) performed for significant posterior capsular opacification (PCO). METHODS: Eighty-eight eyes of 88 patients referred for LC at a tertiary care center were retrospectively analyzed. The data recorded included the cause of cataract, visual acuity, duration of PCO, location of PCO, intraocular lens (IOL) position, IOL type, and lens capsule status. These data were later analyzed for determining the requirement of high pulse energy during LC and the success rate of primary LC. RESULTS: The mean age of

Cytoskeletal drugs prevent posterior capsular opacification in human lens capsule in vitro.

BACKGROUND: To determine whether the cytoskeletal drugs H-7 and Latrunculin B (LAT-B) inhibit posterior capsular opacification (PCO) in the cultured human lens capsular bag. METHODS: Following extracapsular cataract (lens) extraction in human donor eyes, the capsular bag was prepared and cultured by standard techniques. Forty-eight capsular bags were studied, of which 13 were treated with H-7 (50, 100 or 300 μM), 12 with 1% BSS (vehicle of H-7), 11 with LAT-B (2, 5 or 10 μM), and 12 with 0.25% DMSO (vehicle of LAT-B). Forty out of the 48 capsular bags were from paired eyes of 20 donors, with

Incidence of posterior capsule opacification after extracapsular cataract extraction in diabetic patients.

Posterior capsule opacification (PCO) is the most frequent complication following extracapsular cataract extraction (ECCE). To evaluate the rate of PCO we examined 939 eyes (786 patients) following ECCE after a period of 26.3 months on the average. In 202 eyes (21.5%) cataract was associated with a clinically manifest type I or type II diabetes mellitus. The overall incidence of PCO was 28.1% (264 eyes). In patients with no other diseases (371 eyes, 100%) it was 33.4% (124 eyes). Out of 202 eyes (100%) associated with diabetes PCO was seen in 21.8% (44 eyes). This difference was statistically

Argon laser debridement of the posterior capsule: a potential treatment for posterior capsule opacification after extracapsular cataract surgery.

We treated an opacified membrane on the anterior surfaces of the posterior capsule of two cynomolgus monkeys that had undergone extracapsular cataract extraction and intraocular lens insertion. After development of opacification of the posterior capsule, argon laser energy was employed in an attempt to debride the capsules while maintaining structural integrity of the capsule. With this technique, small areas of destruction of the membrane on the anterior surface of the posterior capsule were created with preservation of the capsule.

Research Progress of Drug Prophylaxis for Lens Capsule Opacification after Cataract Surgery.

Phacoemulsification combined with intraocular lens (IOL) implantation is the international standard operation procedure for cataract and has been generalized worldwide. However, lens capsule opacification, one of the common complications after cataract surgery, impacts the recovery of patients' visual function to a large extent. Lens capsule opacification has two types, anterior capsule opacification (ACO) and posterior capsule opacification (PCO), according to the location. There is not an accepted approach to treat ACO. Nd : YAG laser capsulotomy, the common treatment of PCO, can effectively

Influence of capsular tension ring on posterior capsule opacification in myopic eyes.

PURPOSE: To determine the effect of a capsular tension ring (CTR) implantation in preventing posterior capsular opacification (PCO) after cataract surgery in patients with high myopia. MATERIALS AND METHODS: In this prospective single-surgeon standardized-surgical-procedure fellow-eye comparison trial, 34 patients with high myopia had phacoemulsification surgery. Although one eye received an acrylic intraocular lens (IOL) and CTR, other eye received only an IOL as control. PCO, within the capsulorhexis overlap, was documented by standardized digital retroillumination images at least 2 years p

Effects of nuclear factor-kappaB small interfering RNA on posterior capsule opacification.

PURPOSE: The effects of small interfering (si)RNA of nuclear factor kappa B (NF-kappaB) on the development of posterior capsule opacification (PCO) were investigated both in vitro and in vivo in rabbits. METHODS: After application of p105 NF-kappaB siRNA to lens epithelial cells (LECs), Western blot analyses were performed to detect p105 and p50 NF-kappaB and a scratch assay was used to determine cell migration. In the capsular bag model, immunocytochemistry was performed to determine expression of p50 NF-kappaB and Western blot analyses for the presence of epithelial-to-mesenchymal transitio

Pharmacological attempts to reduce posterior capsule opacification after cataract surgery--a review.

Reduction of posterior capsule opacification (PCO) after cataract surgery has been achieved since the general acceptance of posterior chamber intraocular lens implantation 30 years ago. Attention to surgical technique on the one hand and changes in lens design and materials on the other have synergistically reduced the incidence of PCO to less than 5% at 5 years. But lens epithelial cells still proliferate and pharmacological prevention has been largely unsuccessful so far. Any agent must be toxic to these lens epithelial cells without being toxic to the corneal endothelium. This review looks
