Catatonia after epidural morphine.

A 43-year-old male with bronchogenic carcinoma was treated with continuous morphine via a thoracic epidural catheter. On the fifth day, after a total dose of 24 mg morphine chloride in 10 or 20 ml saline, he developed hallucinations, hyperthermia, spasticity, narcolepsy and opisthotonos. Respiratory rate and blood pressure were unaffected. Intravenous naloxone reversed all neurological abnormalities. This unusual syndrome was probably caused by a rostal spread of morphine. The location of the catheter and volume seem to be important variables. Hallucinations are important signs of impending in

Catatonia vis-à-vis delirium: the significance of recognizing catatonia in altered mental status.

BACKGROUND: Catatonia is seldom considered in evaluation of altered mental status (AMS) in medical settings. Furthermore, catatonia often meets delirium criteria due to incoherence, altered awareness and behavioral change. Catatonia may co-occur with or be preferentially diagnosed as delirium. METHODS: We conducted a systematic literature review of MEDLINE, EMBASE and PsycINFO on the relationship between catatonia and delirium. We also juxtapose clinical features of these syndromes and outline a structured approach to catatonia evaluation and management in acute medical settings. RESULTS: Th

Validation of the Catatonia Quick Screen in the Original Bush-Francis Catatonia Rating Scale Cohort.

INTRODUCTION: Delayed catatonia diagnosis may cause preventable harm. The 4-item Catatonia Quick Screen (CQS) allows rapid screening, but its specificity requires evaluation. We assessed CQS sensitivity and specificity using the original Bush Francis Catatonia Rating Scale (BFCRS) cohort. METHODS: Retrospective reanalysis of data from 215 consecutive inpatient psychiatric admissions. CQS classifications were compared against BFCRS, DSM-5-TR, and ICD-11 criteria. 95% confidence intervals (CIs) were calculated via Clopper-Pearson exact methods, treating the eight single-sign patients conservati

Catatonia is not schizophrenia: Kraepelin's error and the need to recognize catatonia as an independent syndrome in medical nomenclature.

Catatonia is a motor dysregulation syndrome described by Karl Kahlbaum in 1874. He understood catatonia as a disease of its own. Others quickly recognized it among diverse disorders, but Emil Kraepelin made it a linchpin of his concept of dementia praecox. Eugen Bleuler endorsed this singular association. During the 20th century, catatonia has been considered a type of schizophrenia. In the 1970s, American authors identified catatonia in patients with mania and depression, as a toxic response, and in general medical and neurologic illnesses. It was only occasionally found in patients with schi

Chinese versions of the northoff catatonia rating scale and the northoff scale for subjective experience in catatonia.

OBJECTIVE: Catatonia was increasingly recognized as a transdiagnostic psychomotor syndrome, but validated Chinese tools for multidimensional clinician-rated assessment and patient-reported subjective experience were limited. This study developed Chinese versions of the Northoff Catatonia Rating Scale (NCRS) and the Northoff Scale for Subjective Experience in Catatonia (NSSC) and evaluated their psychometric properties. METHODS: Participants completed clinician-rated and self-report assessments. Reliability was examined through internal consistency, inter-rater reliability and test-retest reli

Periodic catatonia in the elderly: A case series highlighting the diagnostic challenge between delirium and catatonia.

Differentiating catatonia from delirium in elderly patients presents significant diagnostic challenges due to overlapping clinical features. This case series describes three elderly patients with periodic catatonia characterized by alternating states of stupor and agitation. Specific features, such as negativism and refusal to eat, were pivotal in distinguishing catatonia from delirium. Febrile episodes in these cases appeared to exacerbate symptoms, suggesting a potential role for systemic inflammation and cortico-striato-thalamo-cortical (CSTC) circuit dysfunction in the pathophysiology. Catatonia Secondary to Depolarization Block.

Catatonia is a severe psychomotor disorder that is associated with a 60-fold increased risk of premature death. Its occurrence has been associated with multiple psychiatric diagnoses, the most common being type I bipolar disorder. Catatonia can be understood as a disorder of ion dysregulation with reduced clearance of intracellular sodium ions. As the intraneuronal sodium concentration increases, the transmembrane potential is increased, and the resting potential may ultimately depolarize above the cellular threshold potential creating a condition known as depolarization block. Catatonia-Induced Saddle Pulmonary Embolism.

In this case report, we highlight a patient with catatonia secondary to schizoaffective disorder, depressive type. This patient developed a bilateral deep venous thrombosis progressing to a saddle pulmonary embolism without any predisposing factors to hypercoagulability other than immobility and obesity. The goal of this case report is to increase monitoring and prophylactic treatment for deep venous thrombosis in patients with catatonia, retarded type.

Catatonia due to alprazolam withdrawal.

A 50-year-old man with multiple psychiatric comorbidities including major depressive disorder and general anxiety disorder presented to the emergency room (ER) with altered mental status, immobility and mutism. The patient was unresponsive to commands and unable to provide any history. In the ER he was given a provisional diagnosis of cerebrovascular accident (CVA). Vital signs on admission were stable. On physical examination, he exhibited grimacing, muscle rigidity and areflexia. Workup for CVA and infectious aetiology was unremarkable and the patient's urine toxicology screen was negative.

Resignation Syndrome: Catatonia? Culture-Bound?

Resignation syndrome (RS) designates a long-standing disorder predominately affecting psychologically traumatized children and adolescents in the midst of a strenuous and lengthy migration process. Typically a depressive onset is followed by gradual withdrawal progressing via stupor into a state that prompts tube feeding and is characterized by failure to respond even to painful stimuli. The patient is seemingly unconscious. Recovery ensues within months to years and is claimed to be dependent on the restoration of hope to the family. Descriptions of disorders resembling RS can be found in the

Dissociative disorder presenting as catatonia.

Three cases of dissociative disorder presenting with catatonia are described. Catatonia is generally believed to be associated with schizophrenia. However, many other conditions are also known to cause catatonia. A brief review of literature is provided. All the cases improved rapidly with a few ECTs. This report aims to highlight the presentation of dissociative disorders with catatonia. It also seeks to bring to notice the need to avoid lumping all non-organic catatonics under the rubric of schizophrenia so as to ensure proper treatment.

Typhoid catatonia responsive to ECT.

Twelve patients with typhoid fever presented with a catatonic syndrome that persisted after other signs of the fever had disappeared. The syndrome was distinct from the delirium seen in typhoid fever and did not have the characteristics of an affective or schizophrenic illness. Electric convulsion therapy produced rapid and lasting improvement.

Catatonia. A prospective clinical study.

We studied 55 patients admitted during 14 months to two inpatient psychiatric units of a municipal hospital who exhibited one or more of the catatonic signs of mutism, stereotypy, posturing, catalepsy, automatic obedience, negativism, echolalia/echopraxia, or stupor. Only four of the 55 patients satisfied our research criteria for schizophrenia, whereas over two thirds had diagnosable affective disorders, usually mania. The eight catatonic motor signs were nonspecific and homogeneously distributed among the various research diagnostic groups, with the number and type of individual signs unrela

Catatonia in Systemic Lupus Erythematosus.

BACKGROUND: Systemic lupus erythematosus (SLE) is reported to be the most common rheumatological disorder associated with catatonia. To date, reports on catatonia manifestations in SLE patients are uncommon in published literature, which has often favored a fragmented vision. We performed a narrative review with the aim of identifying all published reports of catatonia in SLE patients to ascertain-in a comprehensive view-its clinical characteristics and to provide useful insights for daily clinical practice. METHODS: Comprehensive literature searches were carried out on 10 March 2025 (subsequ
