A 47-Year-Old Man with Hyperphosphatemia Due to Chronic Renal Failure Treated with Lanthanum Carbonate Tablets Presenting Acutely with Partial Large Bowel Obstruction.

BACKGROUND Hyperphosphatemia is a complication of chronic renal failure (CRF) due to reduction in the glomerular filtration rate. Lanthanum carbonate is a commonly used phosphate binder for patients with CRF and hyperphosphatemia, but has adverse effects if patients are not monitored. This report is of a 47-year-old man with hyperphosphatemia due to CRF treated with lanthanum carbonate tablets who presented acutely with partial large bowel obstruction. The incidence of lanthanum carbonate causing intestinal obstruction is rare, and few cases in the literature have described the course of the d

Cultural acceptability and potential effectiveness of dignity therapy for patients with terminal cancer in Taiwan: A Quasi-Experimental study.

PURPOSE: This study assessed the cultural acceptability and clinical effectiveness of dignity therapy (DT) among terminal cancer patients in Taiwan, addressing a significant gap in evidence regarding its applicability in East Asian contexts. METHODS: A quasiexperimental, nonrandomized controlled design with integrated qualitative feedback analysis was employed. Twenty-four terminal cancer patients were assigned to either an intervention group (DT plus comfort care, n = 12) or a control group (comfort care only, n = 12). Dignity-related distress was measured using the Patient Dignity Inventory

Ethical considerations of phone-based interviews from three studies of COVID-19 impact in Bihar, India.

Phone-based interviews present a range of ethical challenges, including how to ensure informed consent and privacy and maintain confidentiality. Our paper presents conceptual and practical ethical considerations taken into account across three telephone studies on the impact of COVID-19 conducted following India's nationwide lockdown imposed in March 2020. Two studies captured COVID-19 response impact on primary-level Reproductive Maternal Neonatal and Child Health (RMNCH) services and on provider wellness, respectively. The third study focused on how the gendered experience of COVID-19 and th

Neural Correlates of Hostile Jokes: Cognitive and Motivational Processes in Humor Appreciation.

Hostile jokes (HJs) provide aggressive catharsis and a feeling of superiority. Behavioral research has found that HJs are perceived as funnier than non-hostile jokes (NJs). The purpose of the present study was to identify the neural correlates of the interaction between type and humor by comparing HJs, NJs, and their corresponding hostile sentences (HSs) and non-hostile sentences (NSs). HJs primarily showed activation in the dorsomedial prefrontal cortex (dmPFC) and midbrain compared with the corresponding hostile baseline. Conversely, NJs primarily revealed activation in the ventromedial PFC

Aconitine accidental poisoning: two case reports and literature review.

INTRODUCTION: Chinese Herbal Medicine (CHM) is widely integrated into clinical practice across Asia and other regions for disease treatment and health maintenance. Aconitine, a potent diterpenoid alkaloid derived from plants of the Aconitum genus (e.g., Aconitum carmichaelii and Aconitum kusnezoffii), exhibits significant pharmacological activity but also considerable toxicity. While properly processed Aconitum herbs are prescribed for their analgesic and anti-rheumatic properties, inadequate detoxification during processing or improper clinical use can lead to accidental aconitine poisoning.

Experts of their own experience: adolescent and young adult cancer patients' advice-giving as a coping mechanism.

BACKGROUND: To better understand informal coping strategies among adolescents and young adults (AYAs) with cancer, the current investigation asked AYA study participants to describe the 'advice' they would offer to hypothetical peers about coping following diagnosis. This study explores the utility of the single item 'advice' prompt for supportive oncology research and practice. METHODS: AYA cancer patients (n = 27) aged 12-25 years were recruited through electronic health record query at a single-institution health system. Participants completed semi-structured interviews. Lactitol versus lactulose in the treatment of chronic hepatic encephalopathy. A double-blind, randomised, cross-over study.

Lactitol is a disaccharide analogue of lactulose which is available as a pure crystalline powder. The efficacy of lactitol in the treatment of chronic hepatic encephalopathy was assessed in 9 cirrhotic patients in a randomised, double-blind, cross-over comparison with lactulose. The sugars were dispensed in solutions, identical in taste and appearance and with similar physico-chemical properties, which contained either 66.7 g/100 ml of lactitol or 66.7 ml (44.5 g)/100 ml of lactulose syrup. Patients were treated for periods of 3 months with each sugar, during which time they were monitored fre

Does an attempted suicide actually have a cathartic effect?

It has been suggested in empirical studies that an attempted suicide has a cathartic effect. However, only one study used a control group of depressives who had not attempted suicide. A replication of these results using more strictly defined patient groups and a more comprehensive assessment of the psychopathology during index treatment was not possible. Major depressives with and without suicide attempts before index admission displayed similar courses of their depressive symptoms and somatic complaints during index treatment. The cathartic effect of a suicide attempt may be restricted to a

Use of patients' unsolicited correspondence to a family doctor to describe and understand valued components of a doctor-patient relationship: A Hermeneutics approach.

BACKGROUND: Communication and behavior within doctor - patient encounters have been examined using varied techniques; however the nature of unsolicited writings from patients to their family doctors has rarely been reported. This paper therefore aimed to explore the content of, and motivation for, such correspondence. METHODS: One hundred and seven writings to one family physician about care provided during a four decade period were considered. Univariate analyses were used to identify features of patients or family members who wrote personalized notes to the doctor, when, and in what fashion

Intestinal ultrasound reliably predicts fecal loading in patients with ulcerative colitis.

BACKGROUND: Constipation commonly causes abdominal symptoms in ulcerative colitis (UC). Intestinal ultrasound (IUS) shows promise for assessing fecal loading, but parameters remain poorly defined. The primary aim of this study was to validate IUS parameters against computed tomography (CT). A secondary aim was to assess sonographic changes following constipation management. METHODS: In this single-center, prospective, blinded study, adults with UC were prospectively enrolled if fecal loading was suspected on IUS. Same-day CT was performed, followed by catharsis and repeat IUS. Sonographic par

[Behavior and cognitive therapies focused on The Young Personality Disorders Schemas: a pilot study of 14 cases].

INTRODUCTION: Personality disorders are very frequent in the general population, and especially in psychiatric patients. Various types of psychotherapy are proposed to treat these disorders, directly or not, but some specific cognitive-behavioural therapies (CBT) have been developed for all personality disorders or some of them such as borderline personality disorder. Among the most known models, one can quote Beck, Young and Linehan's approaches. Young therapies are based on the concept of "early maladaptive schemas", and associate cognitive-behavioural techniques and a very specific work on

Potentially radiation-induced late toxicity after irradiation of pituitary adenoma, a retrospective dosimetric study.

BACKGROUND: Radiotherapy is considered as a second or third-line treatment for recurrent pituitary adenomas. As it is a benign tumor, the balance should clearly favor the benefit and induce the least amount of side effects. PURPOSE: This retrospective study aims to evaluate the incidence of side effects after treatment and their actual causality with radiotherapy through dosimetric analysis. METHODS: A cohort of 35 patients who underwent normofractionated radiation therapy following unsuccessful surgical or medical interventions was identified. 48.6% (17/35) had functioning adenomas (29.41%
