Neuroendovascular Surgery Applications in Craniocervical Trauma.

Cerebrovascular injuries resulting from blunt or penetrating trauma to the head and neck often lead to local hemorrhage and stroke. These injuries present with a wide range of manifestations, including carotid or vertebral artery dissection, pseudoaneurysm, occlusion, transection, arteriovenous fistula, carotid-cavernous fistula, epistaxis, venous sinus thrombosis, and subdural hematoma. A selective review of the literature from 1989 to 2023 was conducted to explore various neuroendovascular surgical techniques for craniocervical trauma. A PubMed search was performed using these terms: endovas

Craniocervical arterial dissection: spectrum of imaging findings and differential diagnosis.

Craniocervical artery dissection is a potentially disabling yet probably underrecognized condition that often occurs in young and middle-aged adults. Accurate and prompt diagnosis of this condition is crucial because timely and appropriate therapy can significantly reduce the risk of stroke and long-term sequelae. Because of the great diversity in the clinical features of craniocervical artery dissection, imaging plays a primary role in its diagnosis. The increased diagnosis of this disorder in the past two decades can be attributed to an increased awareness of the clinical manifestations of i

Stroke Following Blunt Head Trauma: A Case Report and Review of the Literature.

Ischemic stroke due to blunt traumatic cerebrovascular injury (TCVI) is rare and often overlooked, particularly in patients without any typical predisposing factors. Approximately 1-2% of patients with blunt cerebrovascular trauma experience extracranial injuries, and around 10% of these patients develop strokes, resulting in an estimated 4,800 cases annually in the United States. These strokes can present with atypical symptoms, causing a delay in diagnosis. We report a case of a 36-year-old female who experienced headaches, left-sided weakness, and numbness following a football-related injur…

Cervical spine fracture patterns associated with blunt cerebrovascular injuries: A literature review and meta-analysis.

PURPOSE: Cervical spine fractures (CSF) have been linked to blunt cerebrovascular injuries (BCVI) due to arterial proximity, but consensus on screening criteria remains elusive. This literature review hypothesises that BCVI rates differ with CSF types and aims to identify predictive CSF patterns for BCVI, enhancing screening accuracy and diagnostics. METHODS: All English literature (2000-2026) was reviewed according to PRISMA guidelines in PubMed and Embase. Inclusion criteria were adults (> 16 years), blunt cervical trauma, and specific CSF patterns associated with BCVI. Exclusion criteria w

Hyperventilation in head injury does it do more harm than good?

During the past decade the management of clients with severe head injuries has been influenced by the use of hyperventilation. Increased intracranial pressure (I.C.P.) is the major cause of death following head injury and outcome has been linked to the ability of the practitioner to control intracranial hypertension. Hyperventilation is one method that is frequently employed to control increases in I.C.P. There is no doubt that hyperventilation effectively reduces intracranial pressure, but controversy surrounds the influence that this procedure may inflict upon the cerebral hemodynamic reserv

Multi-Center Validation of the McGovern Pediatric Blunt Cerebrovascular Injury Screening Score.

Blunt cerebrovascular injury (BCVI) is defined as blunt trauma to the head and neck leading to damage to the vertebral and/or carotid arteries; debate exists regarding which children are considered at high risk for BCVI and in need of angiographic/vessel imaging. We previously proposed a screening tool, the McGovern score, to identify pediatric trauma patients at high risk for BCVI, and we aim to validate the McGovern score by pooling data from multiple pediatric trauma centers. This is a multi-center, hospital-based, cohort study from all prospectively registered pediatric (<16 years of age)

A Retrospective Analysis of the Clinical Use and Utility of Advanced Imaging in the Evaluation of Near-Hanging and Strangulation Injuries at a Canadian Level One Trauma Centre.

INTRODUCTION: Computed tomography (CT) of the neck is highly sensitive and may effectively rule-out cervical spine, cerebrovascular, and aerodigestive injuries after blunt and penetrating trauma. However, CT may be overutilized in the evaluation of hanging or strangulation injuries. The objective of this study was to determine the diagnostic yield of CT imaging among patients evaluated for hanging or strangulation mechanisms at a Canadian level-1 trauma center. METHODS: All adult patients evaluated for hanging or strangulation injuries over an eight-year period were reviewed. The primary outc

Inducible nitric oxide synthase expression in cerebrovascular smooth muscle and neutrophils after traumatic brain injury in immature rats.

The inflammatory response after traumatic brain injury (TBI) includes cytokine production, leukocyte infiltration, and microglial activation. Production of nitric oxide by inducible nitric oxide synthase (iNOS) occurs during acute inflammation outside of the CNS and in models of cerebral ischemia, and therefore may contribute to the inflammatory response after TBI. The purpose of this study was to localize and define the time course of iNOS expression after TBI in the immature rat. Immature Wistar rats (age 3.5-4.5 wk) were anesthetized and subjected to percussive trauma to the right parietal

Vascular injuries of the head and neck in martial Arts: A scoping review.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is one of the most feared complications of martial arts practice. Both striking and grappling disciplines place the cervical vasculature at risk either by way of direct blows or application of chokeholds, respectively. Research on BCVI in martial arts remains limited, primarily consisting of case reports. This study aimed to provide a scoping review of BCVI in martial arts. METHODS: Following the PRISMA guidelines, a systematic literature review was performed using PubMed and Ovid Embase databases from date of database inception to October 2024.

The effect of circle of willis anatomy and scanning practices on outcomes for blunt cerebrovascular injuries.

BACKGROUND: Limited research has explored the effect of Circle of Willis (CoW) anatomy among blunt cerebrovascular injuries (BCVI) on outcomes. It remains unclear if current BCVI screening and scanning practices are sufficient in identification of concomitant COW anomalies and how they affect outcomes. METHODS: This retrospective cohort study included adult traumatic BCVIs at 17 level I-IV trauma centers (08/01/2017-07/31/2021). The objectives were to compare screening criteria, scanning practices, and outcomes among those with and without COW anomalies. RESULTS: Of 561 BCVIs, 65% were male

Antiplatelets versus anticoagulants in the treatment of blunt cerebrovascular injury (BCVI) - A systematic review and meta-analysis.

INTRODUCTION: Blunt cerebrovascular injury (BCVI) occurs in 1-3% of blunt traumas and is associated with stroke, disability, and mortality if unrecognized and untreated. Early detection and treatment are imperative to reduce the risk of stroke, however, there is significant variation amongst centers and trauma care providers in the specific medical management strategy used. This study compares antiplatelets vs. anticoagulants to determine BCVI-related stroke risk and bleeding complications to better understand the efficacy and safety of various treatment strategies. METHODS: A systematic revi

Severe Pediatric Polytrauma Complicated by Stroke After Fall From Swamp Buggy.

Blunt cerebrovascular injuries (BCVIs) are rare but potentially severe complications of blunt trauma and can lead to acute ischemic stroke. Diagnosis requires imaging but necessitates more cautious consideration in pediatrics due to radiation concerns. Management options span observation, anticoagulation, and various endovascular interventions, but decisions are complicated by limited literature and the presence of other traumatic injuries. Further collaborative research is needed to provide clarity when caring for pediatric patients with BCVI to guide diagnosis and management. A four-year-old

Endoscopic reverse third ventriculostomy via the cisterna magna: anatomical study and proposal of a novel procedure.

Basilar artery injury has been known as a potential lethal complication of endoscopic third ventriculostomy. In order to avoid this complication, endoscopic reverse third ventriculostomy via a trans-cisterna-magna route was studied. A cadaveric study was performed for navigation of a flexible endoscope through the cisterna magna. Three fresh, unfixed cadavers were used for this endoscopic navigation. In the prone position, a small vertical paramedian skin incision is made at the mid-portion of the posterior neck. An 11-mm threaded plastic tube is inserted towards the posterior arch of the atla

[Central nervous disease and manic state].

We often observe several psychotic symptoms in physical illness-delusions, hallucinations, manic-depressive state and nervous state, for example. The purpose of this chapter is to account for the relationship between manic-depressive state and central nervous disease. Secondary mania is considered to occur in association with physical illness. The criteria for a diagnosis of it is a duration of at least one week, elated or irritable mood, and at least two of the following: hyperactivity, "push of speech", flight of ideas, gradiosity, decreased sleep, distractibility, and lack of judgement.
