[Pathogenesis of the neonatal cholestasis syndrome (author's transl)].

Cholestasis means reduced flow of bile between hepatocytes and duodenum. Functional as well as mechanical factors may be important in the pathogenesis of cholestatic syndromes. Extra- and intrahepatic cholestasis can be distinguished. Intrahepatic bile ducts use to be dilated in extrahepatic cholestasis after early infancy; they are however not dilated in obstructive jaundice of the newborn. Neonatal cholestatic syndromes caused by liver cell diseases may in addition cause an obstruction of extrahepatic bile ducts. At the present time most workers in the field do believe that extrahepatic chol

Histopathology of cholestasis.

Cholestasis may be extrahepatic or intrahepatic in origin. The block in bile secretion may be complete or incomplete to variable extent. Complete cholestasis occurs in case of primary parenchymal disease (intrahepatic cholestasis) or total obstruction of extrahepatic bile ducts (extrahepatic cholestasis). Incomplete block in bile secretion is due to incomplete obstruction of intra- or extrahepatic bile ducts (intra- or extrahepatic cholestasis or both). Histologically, it is useful to distinguish between bilirubionstasis and cholate-stasis. Complete secretory block causes as early changers: bi

Predictive value of assessment of different modalities in the diagnosis of infantile cholestasis.

This study investigated the relative accuracy and roles of abdominal ultrasonography, hepatobiliary scintigraphy and liver biopsy in the diagnosis of infantile cholestasis. A total of 50 infants (27 females) aged 1 - 12 months were classified into those with intrahepatic causes of cholestasis (n = 22) and those with extrahepatic causes (n = 28). Cholestasis is caused by a wide range of conditions and diagnosis requires meticulous history taking, thorough clinical examination and many laboratory tests. The most common cause of intrahepatic cholestasis was found to be idiopathic neonatal hepatit

[Rational diagnosis of the cholestasis syndrome].

Aim of a rational diagnostic approach is keeping the risks and challenges of the patient at a minimum, shortening the time interval till the diagnosis is established and putting the charges to a low level. Anamnesia, clinical examination and laboratory results allow the experienced clinician in about 90% a correct diagnosis if an extrahepatic cholestasis is the case in question. On the other hand the result is only 75-80% if there is an intrahepatic cholestasis. Non dilated bile ducts do not exclude an extrahepatic cholestasis and additional diagnostic measures like ERCP or/and PTC are recomme

Mitofusin 2 protects hepatocyte mitochondrial function from damage induced by GCDCA.

Mitochondrial impairment is hypothesized to contribute to the pathogenesis of chronic cholestatic liver diseases. Mitofusin 2 (Mfn2) regulates mitochondrial morphology and signaling and is involved in the development of numerous mitochondrial-related diseases; however, a functional role for Mfn2 in chronic liver cholestasis which is characterized by increased levels of toxic bile acids remain unknown. Therefore, the aims of this study were to evaluate the expression levels of Mfn2 in liver samples from patients with extrahepatic cholestasis and to investigate the role Mfn2 during bile acid ind

Bile acids in serum and bile of infants with cholestatic syndromes.

The concentration of individual bile acids in serum was measured in 18 neonates and infants with various cholestatic conditions (extrahepatic biliary atresia, neonatal hepatitis syndrome, chronic intrahepatic cholestasis and posthemolytic cholestasis). The cholate/chenodeoxycholate ratio in serum was smaller than one in all patients with neonatal hepatitis syndrome or extrahepatic biliary atresia, cholestatic conditions which were accompanied by signs of liver cell injury. It was greater than one in the patients with chronic intrahepatic cholestasis. Administration of cholestyramine to patient

Cholestasis: human disease and experimental animal models.

Cholestasis may result from a failure in bile secretion in hepatocytes or ductular cells, or from a blockade to the free bile flow. Human cholestasis may be induced by many drugs, being antibiotics the more common. Other types of cholestasis seen in humans are a group of familial cholestatic disorders, obstructive cholestasis, primary biliary cirrhosis, extrahepatic biliary atresia, primary sclerosing cholangitis, cholestasis of pregnancy, oral contraceptive-induced cholestasis, and sepsis-induced cholestasis. Experimental animal models allow the understanding of pathophysiological mechanisms

Robustness testing and optimization of an adverse outcome pathway on cholestatic liver injury.

Adverse outcome pathways (AOPs) have been recently introduced as tools to map the mechanisms underlying toxic events relevant for chemical risk assessment. AOPs particularly depict the linkage between a molecular initiating event and an adverse outcome through a number of intermediate key events. An AOP has been previously introduced for cholestatic liver injury. The objective of this study was to test the robustness of this AOP for different types of cholestatic insult and the in vitro to in vivo extrapolation. For this purpose, in vitro samples from human hepatoma HepaRG cell cultures were e

[Cholestasis].

Cholestasis is a condition, where the bile flow into the intestine is defective. The causes may be extrahepatic (in large biliary ducts) or intrahepatic (at the level of hepatocytes or minor biliary ducts). The lack of bile in the intestine results in serious consequences, mainly malabsorption, malnutrition and skeletal changes. The compounds, which are normally excreted in the bile, accumulate and liver biliary cirrhosis ensues. Cholestatis is characterized by a typical laboratory picture, outlined in publications, which serves for differentiation of individual forms of cholestasis. There are

Increased microfilaments in hepatocytes and biliary ductular cells in cholestatic liver diseases.

To assess the extent of microfilaments in cholestatic liver diseases we examined the cytoplasmic microfilaments in intrahepatic and extrahepatic cholestasis in man by electron microscopy. Study subjects were two patients with drug-induced intrahepatic cholestasis, three patients with intrahepatic cholestasis due to viral hepatitis, four patients with extrahepatic cholestasis due to stones of the common bile duct and two patients with primary biliary cirrhosis. Two biopsied specimens from patients without clinical or histological evidence of liver disease served as noncholestatic controls. The

Care of the cholestatic patient.

Cholestasis is defined as impairment of bile formation or bile flow. Care of the patient with cholestatic features is dependent on identifying the cause of the cholestasis, initiating appropriate treatment of reversible conditions, and the recognition and management of cholestasis-specific complications. Cholestasis may include extrahepatic ducts and intrahepatic bile ducts, or may be limited to one or the other. Jaundice and pruritus are the hallmarks of cholestasis clinically but biochemical evidence may, and often does, precede the clinical manifestations.

Increased intercellular adhesion molecule-1 serum concentration in cholestasis.

BACKGROUND/AIMS: Increase of serum levels of the soluble intercellular adhesion molecules in patients with the cholestatic liver diseases primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) are known and have been thought to indicate activation of the immune system and the grade of the inflammatory process. In hepatitis and cholestatic diseases, expression of adhesion molecules was found on the surface of bile duct epithelia and hepatocytes. MATERIALS AND METHODS: Serum levels of sICAM-1 in patients with intrahepatic cholestasis in PBC (n = 42) and extrahepatic cholestasi

[The treatment characteristics of cholestatic forms of viral hepatitides].

Taking in consideration that there are quite a few matters of debate in treating cholestatic forms of viral hepatitis, the paper analyzes the experience gained with the treatment of patients presenting with the above-mentioned manifestations. Presented in the paper is a detailed scheme of treatment in different variants of cholestasis in viral hepatitis. The need for the use of neutral drugs for the affected liver is pointed out. Conservative treatment in extrahepatic cholestasis against the background of viral hepatitis may in a number of cases result in recovery from the malady without surgi

Morphology of liver cell tight junctions in ethinyl estradiol induced cholestasis.

Using freeze-fracture techniques the junctional complex of the hepatocytes in female rats is studied after ethinyl estradiol induced cholestasis. The gap junctions are not changed in this situation in contrast to the altered pattern found in extrahepatic bile duct obstruction. The more or less parallel tight junctional strands around the canalicular lumen are converted into a irregular loose network after estradiol treatment in a similar way as in extrahepatic cholestasis. These modifications in the geometrical arrangement of the strands suggest an increase permeability through the tight junct
