Identification of the PS1 Thr147Ile Variant in a Family with Very Early Onset Dementia and Expressive Aphasia.

BACKGROUND: Early onset dementias have variable clinical presentations and are often difficult to diagnose. We established a family pedigree that demonstrated consistent recurrence of very early onset dementia in successive generations. OBJECTIVE AND METHOD: In order to refine the diagnosis in this family, we sequenced the exomes of two affected family members and relied on discrete filtering to identify disease genes and the corresponding causal variants. RESULTS: Among the 720 nonsynonymous single nucleotide polymorphisms (SNPs) shared by two affected members, we found a C to T transition

Omalizumab treatment of systemic mast cell activation disease: experiences from four cases.

We report on the outcome of 4 patients with therapy-resistant systemic mast cell activation disease (MCAD) treated with the anti-IgE monoclonal antibody omalizumab in compassionate use. Two patients achieved an impressive persistent clinical response to treatment with omalizumab. In the third patient symptoms gradually improved. In the fourth patient omalizumab treatment had to be discontinued due to intolerable mast cell mediator-induced symptoms. In conclusion, omalizumab can lessen the intensity of the symptoms of systemic MCAD. Hence, omalizumab should be considered as a therapeutic option

Lipoprotein apheresis for anti-nephrin antibody-positive steroid-resistant nephrotic syndrome: a pediatric case report.

Steroid-resistant nephrotic syndrome (SRNS) in children carries a poor prognosis, and recent discoveries implicate anti-nephrin autoantibodies (ANABs) as mediators of podocyte injury. Lipoprotein apheresis, originally developed for familial hypercholesterolemia, has been explored as rescue therapy in refractory SRNS, but its immunologic effects remain uncertain. This case demonstrates a decline in ANAB titers accompanied by clinical improvement following lipoprotein apheresis. We report a 7-year-old male of Afro-Caribbean descent with biopsy findings consistent with minimal change disease and

Clinical and Translational Perspectives on Bacteriophage Therapy for Nontuberculous Mycobacterial Diseases.

Mycobacterium abscessus complex (MABC) infections are among the most intractable challenges in clinical mycobacteriology because of their extensive intrinsic and acquired antibiotic resistance. Recent studies on compassionate use and a systematic 20-patient cohort study demonstrated that bacteriophage therapy is safe and generally well-tolerated, and it has been proven capable of inducing clinically meaningful improvements. Nevertheless, patient outcomes remain heterogeneous, largely because of antibody-mediated neutralization during intravenous administration and morphotype-dependent suscepti

Lessons Learned From the First-In-Human Compassionate Use of Connect-EA™ in Ten Patients With Esophageal Atresia.

INTRODUCTION: Delayed primary repair of esophageal atresia in patients with high-risk physiologic and anatomic comorbidities remains a daunting challenge with an increased risk for peri-operative morbidity and mortality via conventional repair. The Connect-EA device facilitates the endoscopic creation of a secure esophageal anastomosis. This follow-up study reports our long-term outcomes with the novel esophageal magnetic compression anastomosis (EMCA) Connect-EA device for EA repair, as well as lessons learned from the ten first-in-human cases. We propose an algorithm to maximize the advantag

[Gemtuzumab ozogamicin for treatment of acute myeloid leukemia].

Gemtuzumab ozogamicin (GO) is a humanized monoclonal antibody anti-CD33 conjugated with calicheamicin and was indicated for acute myeloid leukemia (AML) patients. It was initially approved in 2000 by FDA thank to promising results from a phase II trial including elderly relapsing AML patients. Numerous studies evaluated this treatment as salvage and frontline therapy in various regimens including associations with intensive chemotherapy. Based on the results from a randomized phase III trial, this agent was withdrawal because of no evidence for its benefit in terms of survival associated with

Hypereosinophilic syndrome response to mepolizumab in the setting of a compassionate use program.

Mepolizumab, an anti-interleukin-5 antibody, has been proven a safe and effective glucocorticoid (GC)-sparing drug for many patients with nonclonal hypereosinophilic syndrome (HES) and is now approved in many countries. It remains unclear, however, which patients are most likely to benefit from therapy and whether the currently approved dosing regimen is appropriate for all. This observational retrospective study included all patients with HES who were enrolled in the MHE104317 compassionate use program (CUP) in our center. Patient and disease characteristics, mepolizumab dosing, and both clin

[Effectiveness and safety of lenalidomide in myelofibrosis patients: a case series from the Spanish compassionate use program].

OBJECTIVE: Myelofibrosis (MF) is a chronic myeloproliferative neoplasm (CMPN) characterized by clonal proliferation of the pluripotent hematopoietic germinal cell, fibrosis, and bone marrow angiogenesis, and with extra medullary hematopoiesis. Lenalidomide is an immunomodulatory and antiangiogenic agent that has shown clinical benefit in MF patients in several phase II clinical trials. In this work, we present the results of the retrospective assessment of a series of 32 patients diagnosed with MF that received treatment with lenalidomide within the Spanish program of hospital compassionate us

Transarterial Onyx Embolization of Bilateral Transverse-Sigmoid Dural Arteriovenous Malformation with Transvenous Balloon Assist-Initial U.S. Experience with Copernic RC Venous Remodeling Balloon.

BACKGROUND: Currently, the mainstay treatment of dural arteriovenous fistula (DAVF) involves endovascular approaches, especially for high-grade lesions. Transarterial embolization with preservation of venous sinuses has become the preferred approach due to the development of newer liquid embolic agents. For further precision during embolization, the use of temporary balloon occlusion to protect the patency of dural sinuses from the embolic agent's migration has been described. METHODS: A 64-year-old man presented with bilateral pulsatile tinnitus and visual decline. A diagnostic cerebral angi

Trends in intracranial stenting among medicare beneficiaries in the United States, 2006-2010.

BACKGROUND: It is uncertain how intracranial stenting (ICS) has been adopted nationally during a period characterized by a restrictive payment policy by the Centers for Medicare & Medicaid Services, humanitarian device exemption approval by the Food and Drug Administration, and insufficient evidence of effectiveness. We sought to determine the trends in rates of ICS use and associated outcomes in the United States. METHODS AND RESULTS: From 65 211 328 Medicare Fee-for-Service beneficiaries hospitalized between 2006 and 2010 in acute care hospitals in the United States, we included patients wi

Compassionate use of linezolid for adult Taiwanese patients with bone and joint infections.

BACKGROUND: Compassionate use of linezolid for bone and joint infections has been reported and hematological adverse reactions are a cause for concern. A similar experience in Asian people has not been reported. METHODS: Adult cases that received linezolid for bone and joint infections at 3 Taiwanese medical centers from 2002 to 2006 were analyzed. We investigated factors associated with treatment outcome and hematological adverse reactions. RESULTS: Of the 51 cases that received linezolid for more than 3 days, 41 (80.4%) had a favorable outcome and 10 had a failed outcome. Based on univaria

Oncologists' reflections on patient rights and access to compassionate use drugs: A qualitative interview study from an academic cancer center.

The U.S. Food and Drug Administration (FDA) allows patients with serious illnesses to access investigational drugs for "compassionate use" outside of clinical trials through expanded access (EA) Programs. The federal Right-to-Try Act created an additional pathway for non-trial access to experimental drugs without institutional review board or FDA approval. This removal of oversight amplifies the responsibility of physicians, but little is known about the role of practicing physicians in non-trial access to investigational drugs. We undertook semi-structured interviews to capture the experience

Efficacy of glucarpidase (carboxypeptidase g2) in patients with acute kidney injury after high-dose methotrexate therapy.

STUDY OBJECTIVE: Because the incidence rate of renal impairment is 2-10% for patients treated with high-dose methotrexate and renal impairment develops in 0-12.4% of patients treated for osteosarcoma, we sought to evaluate the efficacy of glucarpidase, a recently approved drug that rapidly hydrolyzes methotrexate to inactive metabolites, which allows for nonrenal clearance in patients with delayed renal methotrexate elimination. DESIGN: Pooled analysis of efficacy data from four multicenter single-arm compassionate-use clinical trials using protocols from 1993 to 2007. PATIENTS: Of 476 patie
