RTN4IP1-associated non-syndromic optic neuropathy and rod-cone dystrophy.

BACKGROUND: Biallelic variants in RTN4IP1 are a well-established cause of syndromic and nonsyndromic early-onset autosomal recessive optic neuropathy. They have more recently been reported to cause a concomitant but later-onset rod-cone dystrophy with or without syndromic features. METHODS: A comprehensive evaluation was performed that included assessment of visual and retinal function, clinical examination, and retinal imaging. Childhood ophthalmic records as well as the results of genetic testing were evaluated. RESULTS: A 24-year-old female described longstanding reduced visual acuity wit

Young monozygotic twin sisters with fundus albipunctatus and cone dystrophy.

OBJECTIVE: To describe young monozygotic twin sisters with fundus albipunctatus (a type of autosomal recessive stationary night blindness caused by mutations of the 11-cis retinol dehydrogenase gene [RDH5]) associated with cone dystrophy, previously reported in elderly men. METHODS: Ophthalmologic examinations were performed, and the RDH5 gene was analyzed by direct genomic sequencing. RESULTS: Twin 23-year-old sisters with high myopic refractive errors of approximately -13 diopters were diagnosed as having fundus albipunctatus. Their photopic electroretinographic responses were markedly red

Isolated rod-cone dystrophy in homozygous IFT140 missense allele p.Tyr923Asp.

OBJECTIVE: IFT140 encodes a core intraflagellar transport protein essential for ciliary function. While biallelic IFT140 variants typically cause syndromic ciliopathies such as Mainzer-Saldino syndrome, isolated retinal phenotypes are increasingly recognized. We describe two affected siblings from a consanguineous Pakistani family with lifelong nyctalopia and progressive peripheral field loss associated with a homozygous IFT140 missense variant, c.2767T > G (p.Tyr923Asp) (NM_014714.3). METHOD: Detailed ophthalmic evaluation included multimodal retinal imaging, electrodiagnostics, and microper

Novel compound heterozygous CNGA3 mutation associated with retinal cone dystrophy.

Retinal cone dystrophy (COD) is an inherited retinal disease characterized by reduced central vision, color vision defects and photophobia, resulting from the degeneration of photoreceptors in cone cells, and commonly occurs due to mutations in cyclic nucleotide-gated channel subunit α 3 (CNGA3). CNGA3 mutations are associated exclusively with autosomal recessive retinal disorders, requiring homozygous or compound heterozygous mutations for pathogenicity. In the present study, whole-exome sequencing was performed on a 9-year-old girl diagnosed with COD and her parents, which identified a compo

Progressive Cone-Rod Dystrophy and RPE Dysfunction in Mitfmi/+ Mice.

Mutations in the mouse microphthalmia-associated transcription factor (Mitf) gene affect retinal pigment epithelium (RPE) differentiation and development and can lead to hypopigmentation, microphthalmia, deafness, and blindness. For instance, an association has been established between loss-of-function mutations in the mouse Mitf gene and a variety of human retinal diseases, including Waardenburg type 2 and Tietz syndromes. Although there is evidence showing that mice with the homozygous Mitfmi mutation manifest microphthalmia and osteopetrosis, there are limited or no data on the effects of t

A new PDE6A missense variant p.Arg544Gln in rod-cone dystrophy.

PURPOSE: Thus far, only one Japanese patient with autosomal recessive rod-cone dystrophy (AR-RCD) associated with the phosphodiesterase 6A gene (PDE6A) has been reported. The purpose of this study was to analyze the clinical features of a Japanese female patient with AR-RCD with a novel missense variant in PDE6A. METHODS: We performed whole-exome sequencing (WES) to identify the disease-causing variant and a comprehensive ophthalmic examination including full-field electroretinography (ERG). RESULTS: WES analysis revealed that the patient carried a novel homozygous missense variant (c.1631G

CNGB1-related rod-cone dystrophy: A mutation review and update.

Cyclic nucleotide-gated channel β1 (CNGB1) encodes the 240-kDa β subunit of the rod photoreceptor cyclic nucleotide-gated ion channel. Disease-causing sequence variants in CNGB1 lead to autosomal recessive rod-cone dystrophy/retinitis pigmentosa (RP). We herein present a comprehensive review and analysis of all previously reported CNGB1 sequence variants, and add 22 novel variants, thereby enlarging the spectrum to 84 variants in total, including 24 missense variants (two of which may also affect splicing), 21 nonsense, 19 splicing defects (7 at noncanonical positions), 10 small deletions, 1 s

Mutations in PCYT1A cause spondylometaphyseal dysplasia with cone-rod dystrophy.

Spondylometaphyseal dysplasia with cone-rod dystrophy is a rare autosomal-recessive disorder characterized by severe short stature, progressive lower-limb bowing, flattened vertebral bodies, metaphyseal involvement, and visual impairment caused by cone-rod dystrophy. Whole-exome sequencing of four individuals affected by this disorder from two Brazilian families identified two previously unreported homozygous mutations in PCYT1A. This gene encodes the alpha isoform of the phosphate cytidylyltransferase 1 choline enzyme, which is responsible for converting phosphocholine into cytidine diphospha

Whole exome sequencing reveals GUCY2D as a major gene associated with cone and cone-rod dystrophy in Israel.

PURPOSE: The Israeli population has a unique genetic make-up, with a high prevalence of consanguineous marriages and autosomal recessive diseases. In rod-dominated phenotypes, disease-causing genes and mutations that differ from those identified in other populations often are incurred. We used whole exome sequencing (WES) to identify genetic defects in Israeli families with cone-dominated retinal phenotypes. METHODS: Clinical analysis included family history, detailed ocular examination, visual function testing, and retinal imaging. Whole exome sequencing, followed by segregation analysis, wa

Clinical Validation of a CRX Variant Leading to a Cone-Rod Dystrophy.

Patients with cone-rod dystrophy (CORD) due to CRX mutations have progressive visual impairment characterized by central vision loss, photophobia, and color vision defects. On ophthalmic examination, patients with CRX-associated CORD may have macular abnormalities, changes in the retinal pigment epithelium, and progressive macular degeneration, affecting central visual function. Variants in the CRX gene located on chromosome 19q13 lead to photoreceptor dysfunction and retinal degeneration. Variant classification in CRX presents unique challenges, as approximately half of heterozygous missense

Deletion mapping of a retinal cone-rod dystrophy: assignment to 18q211.

Deletion of 18q211 was observed in a mentally retarded young man with electrophysiologically demonstrated cone-rod dystrophy, present since childhood. He had hypogonadism and a central postsynaptic hearing impairment. This is the first case of a chromosome deletion in a patient with a cone-rod dystrophy. Three patients with more distal deletions on chromosome 18 did not present retinal dystrophies. We suggest that one of the loci for cone-rod dystrophy may be located on chromosome 18 at q211-213. Reports of similar findings will be necessary for confirmation of this assumption.

Cone dystrophy and supernormal dark-adapted b-waves in the electroretinogram.

A male patient suffering from cone dystrophy was followed over 9 years. In addition to the typical clinical and electrophysiologic signs, supernormal b-waves were found in the dark-adapted electroretinogram. Our case is compared with 12 similar patients described in the literature. Our patient differed from the other patients in the following aspects: he was male and had a congenital stationary disease with a small pigment epithelial scar in the left eye only and no other fundus changes up to the age of 22 years. He did not complain of night blindness. The dark-adapted electroretinogram of our

Atp1b2Atp1b1 Knock-In Mice Exhibit a Cone-Rod Dystrophy-Like Phenotype.

The Na,K-ATPase is a heterodimeric ion pump consisting of various combinations of a catalytic α-subunit (α1, α2, α3, or α4, encoded by ATP1A1-ATP1A4) and a β-subunit (β1, β2, or β3, encoded by ATP1B1-ATP1B3). We have previously shown that Atp1b2 knock-out (ko) mice exhibit rapid photoreceptor cell degeneration, whereas Atp1b2Atp1b1 knock-in (ki) mice, which express the β1-subunit instead of the β2-subunit under regulatory elements of the Atp1b2 gene, exhibit slowly progressive retinal dystrophy. Here, we performed a detailed analysis of the retinal phenotype of the Atp1b2Atp1b1 ki mouse. We fo

Spectral domain optical coherence tomography findings in bilateral peripheral cone dystrophy.

INTRODUCTION: To report spectral domain optical coherence (OCT) tomography findings in a case of bilateral peripheral cone dystrophy. CASE REPORT: A 21-year-old man complained of blurred visual fields on the lateral sides of both eyes. The patient's best-corrected visual acuity was 20/20 in both eyes. Fundus examination revealed mild temporal pallor, while fluorescein angiography did not show any abnormalities. Humphrey's visual field revealed a C-shaped scotoma. Photopic electroretinogram (ERG) and 30-Hz flicker revealed decreased cone function; however, scotopic ERG noted a normal response.

Cone dystrophy, nyctalopia, and supernormal rod responses. A new retinal degeneration.

An unusual retinal degeneration considered to be inherited as an autosomal recessive trait occurred in two of four children in a Hispanic family. The abnormality causes a progressive and generalized loss of cone vision, including decreased acuity, decreased color vision, central scotomas to small test objects, photo-phobia, and a profound diminution of the cone-mediated electroretinographic (ERG) pattern. A loss of the foveal reflex and an increased granularity of the macula is seen funduscopically. In addition, there is a most unusual alteration of the rod system detectable in the rod-mediate
