Ambulatory ability after hip fracture. A prospective study in geriatric patients.

Three hundred thirty-six community-dwelling, previously ambulatory, geriatric patients with hip fracture were observed prospectively to determine ambulatory ability at a minimum followup of 1 year. One hundred thirty-seven (41%) patients maintained their prefracture ambulatory ability at a minimum followup of 1 year; 134 (40%) patients remained ambulatory but became more dependent on assistive devices; 39 (12%) previous community ambulators became household ambulators, and 26 (8%) patients became nonfunctional ambulators. Analysis was performed to determine which pre- and postinjury factors we

Musculoskeletal consequences of near-drowning in children.

Nearly drowned children sustaining cardiac or respiratory arrest or altered neurologic status developed anoxic encephalopathy (30%), died in the ensuing days (36%), or were discharged neurologically normal (34%). In encephalopathic children, spasticity was more malignant than in children with other causes of spasticity. The most common musculoskeletal problem was contracture, especially equinus, hip adductor, hamstring, and quadriceps. Hip subluxation or dislocation occurred in 34% as early as 1 month after injury. Scoliosis developed in 18%, with some requiring bracing and surgery. Thirty-one

Effects of AM1346, a high-affinity CB1 receptor selective anandamide analog, on open-field behavior in rats.

AM1346 is a cannabinoid receptor type 1 (CB1R) anandamide analog [alkoxyacid amide of N-eicosa-(5Z, 8Z, 11Z, 14Z)-tetraenylamine] with high affinity and selectivity for the CB1 vs. CB2 receptor [Ki (CB1)=1.5 nmol/l; Ki (CB2)=152 nmol/l]. The present study characterized the effects of AM1346 (5.6-18 mg/kg) and its interaction with the CB1R antagonist/inverse agonist SR141716 (1-5.6 mg/kg) on open-field behaviors of rats. AM1346 reduced ambulation (horizontal activity), rearing (vertical activity) and increased the degree of circling and the latency to leave the central area of the open-field ar

Preclinical and clinical evaluation of epratuzumab (anti-CD22 IgG) in B-cell malignancies.

Community ambulation is essential for maintaining a healthy lifestyle, but it poses significant challenges for individuals with limb loss due to complex task demands. In wearable robotics, particularly powered prostheses, there is a critical need to accurately estimate environmental context, such as walking speed and slope, to offer intuitive and seamless assistance during varied ambulation tasks. We developed a user-independent and multicontext, intent recognition system that was deployed in real-time on an Open Source Leg (OSL). We recruited 11 individuals with transfemoral amputation, with

Upper Extremity Variant Vascular Anatomy: Navigation Techniques.

Upper limb approaches for diagnostic cardiac catheterization and percutaneous coronary intervention (PCI) have gained widespread adoption due to their advantages in reducing bleeding complications, improving patient comfort, and enabling earlier ambulation compared to transfemoral access. However, the success and safety of these procedures is heavily dependent on a thorough understanding of the vascular anatomy of the upper limb, particularly its numerous congenital and acquired variations. This article explores the spectrum of upper extremity vascular anomalies and their clinical implications

Positive HLA-B27 and sacroiliitis is not always spondyloarthritis.

A 36-year-old man was treated for several years with multiple agents for ankylosing spondylitis based on positive human leukocyte antigen-B27 and sacroiliitis. He was also diagnosed with osteoporosis and hypophosphatemia. Over these years, from being an avid runner, he became dependent on a walker for ambulation. The lack of treatment response and the low phosphorus were clues that eventually led to a diagnosis of tumor-induced osteomalacia. This case discusses the importance of not solely relying on genetic markers and sacroiliitis for diagnosing ankylosing spondylitis as other conditions can

Total calcanectomy for the treatment of chronic calcaneal osteomyelitis.

The purpose of this study is to present the surgical and functional results of a total calcanectomy procedure as a foot salvage alternative in patients with extensive chronic osteomyelitis of the calcaneus. A retrospective review identified eight patients treated with a total calcanectomy for a chronic nonhealing plantar ulcer of the heel and osteomyelitis of the calcaneus. The primary diagnosis was insulin-dependent diabetes mellitus (six patients), pneumococcal septicemia (one patient), and an open calcaneal fracture (one patient). The average age of the patients was 52 years. Prior procedur

Functional outcome after infrainguinal bypass for limb salvage.

PURPOSE: Functional outcome after infrainguinal bypass (IB) has recently been assessed with global health status questionnaires but not by criteria specific to the objectives of IB (i.e., maintenance of independent living and ambulation). Preoperative and postoperative living situation and ambulatory status were evaluated in patients who underwent IB for limb salvage (LS) indications. METHODS: For patients in whom IB was performed for LS from January 1980 to July 1995, living situation (independent or dependent) and ambulatory status were assessed before the onset of the need for LS surgery a

The impact of video laryngoscopy on the first-pass success rate of prehospital endotracheal intubation in The Netherlands: a retrospective observational study.

PURPOSE: The first-pass success rate for endotracheal intubation (ETI) depends on provider experience and exposure. We hypothesize that video laryngoscopy (VL) improves first-pass and overall ETI success rates in low and intermediate experienced airway providers and prevents from unrecognized oesophageal intubations in prehospital settings. METHODS: In this study 3632 patients were included. In all cases, an ambulance nurse, HEMS nurse, or HEMS physician performed prehospital ETI using direct Laryngoscopy (DL) or VL. RESULTS: First-pass ETI success rates for ambulance nurses with DL were 45.

Disability weight for each level of the Expanded Disability Status Scale in multiple sclerosis.

BACKGROUND: The Expanded Disability Status Scale (EDSS) is the most widely employed ordinal disability scale in multiple sclerosis (MS). However, how far apart the individual EDSS levels are along the disability spectrum has not been formally quantified. OBJECTIVES: The objective of this paper is to generate refined disability weights (DWs) for each of the ordinal EDSS levels. METHODS: We performed the person trade-off (PTO) procedure to derive DWs of five representative EDSS categories (2, 4, 6, 7 and 9), and DWs of the remaining EDSS categories were assigned by linear interpolation. The mo

Impact of extreme temperatures on ambulance dispatches in London, UK.

BACKGROUND: Associations between extreme temperatures and health outcomes, such as mortality and morbidity, are often observed. However, relatively little research has investigated the role of extreme temperatures upon ambulance dispatches. METHODS: A time series analysis using London Ambulance Service (LAS) incident data (2010-2014), consisting of 5,252,375 dispatches was conducted. A generalized linear model (GLM) with a quasi-likelihood Poisson regression was applied to analyse the associations between ambulance dispatches and temperature. The 99th (22.8°C) and 1st (0.0°C) percentiles of t

Statistical Genetics of DMD Gene Mutations in a Kazakhstan Cohort: MLPA/NGS Variant Validation and Genotype-Phenotype Modelling.

Background: Duchenne muscular dystrophy (DMD) results from pathogenic variants in the DMD gene, one of the most significant and most mutation-prone genes in the human genome. Although global mutation registries are well developed, genetic data from Central Asian populations remain extremely limited, leaving essential gaps in regional epidemiology and in the understanding of genotype-phenotype patterns. Methods: We conducted a retrospective analysis of patients with genetically confirmed dystrophinopathy in Kazakhstan. Variants were identified using multiplex ligation-dependent probe amplificat

Genotype-phenotype correlations of GFAP variants in type I Alexander disease subtypes.

INTRODUCTION: Alexander disease (AxD) is a rare and progressive leukodystrophy caused by variants in the Glial Fibrillary Acidic Protein (GFAP) gene. AxD presents as Type I and Type II, based on clinical and neuroradiological features. However, Type I patients variable disease progression has led to sub-classify them into subtypes Ia-Ib-Ic-Id. Our study investigated genotype-phenotype correlations in Type I AxD across these subtypes. METHODS: A cohort of 74 genetically confirmed Type I AxD patients was analysed, with variants classified according to the clinical subtypes Ia, Ib, Ic, and Id. G

Salbutamol in Congenital Myasthenic Syndrome: A Systematic Review.

BACKGROUND: Congenital myasthenic syndromes (CMS) are inherited disorders of neuromuscular transmission with few effective treatments. Salbutamol, a β2-adrenergic agonist, has shown anecdotal benefit, yet no comprehensive assessment of its clinical effectiveness across genetic subtypes exists. This systematic review aimed to evaluate outcomes associated with Salbutamol therapy in genetically confirmed CMS. METHODS: A systematic literature search was conducted in PubMed and Scopus through May 19, 2025, identifying studies involving Salbutamol treatment in patients with genetically confirmed con
