Navigating Legitimacy in the Development of Obstructive Sleep Apnea Diagnostics: A Qualitative Study.

IntroductionHealth technology innovations often challenge established practices, roles, and norms, which can impede their adoption and diffusion across healthcare systems. This study explores how those involved in sleep research and innovation facilitate adoption and diffusion of diagnostic solutions for obstructive sleep apnea (OSA) by addressing perceived legitimacy concerns-conceptualized here as cognitive, moral, and pragmatic legitimacy pressures-during solution development. We refer to these efforts as navigating legitimacy.MethodsThe study draws on 24 thematic interviews with individual

Knowledge translation in surgery: a scoping review of implementation strategies, effectiveness and contextual barriers and enablers.

BACKGROUND: Knowledge translation (KT) interventions are essential for implementing evidence-based practices in healthcare. However, despite their proven effectiveness in addressing global health challenges, KT strategies in surgery remain challenging to apply. This scoping review examines KT strategies in surgery, their effectiveness, and key barriers and enablers to their implementation. METHODS: This scoping review followed the Arksey and O'Malley and Levac et al. frameworks, integrating the RE-AIM (Reach, Effectiveness, Adoption, Implementation, Maintenance) and the PRISM (Practical Robus

Beware the next big thing.

Innovative management ideas that bubble up in other companies pose a perennial quandary for leaders: Should you attempt to borrow new ideas, and if so, which ones and how? Even the most promising practices can be disastrous if they're transplanted into the wrong company, writes Julian Birkinshaw of London Business School. Broadly speaking, there are two ways to borrow from innovative companies, he argues. The first, observe and apply, is the most commonly used approach for adopting new management ideas. It can and does work well, but only under Limited sets of circumstances: when the observed

Is it real? Can we win? Is it worth doing? Managing risk and reward in an innovation portfolio.

Minor innovations make up most of a company's development portfolio, on average, but they never generate the growth companies seek. The solution, says Day--the Geoffrey T. Boisi Professor of Marketing and a codirector of the Mack Center for Technological Innovation at Wharton--is for companies to undertake a systematic, disciplined review of their innovation portfolios and increase the number of major innovations at an acceptable level of risk. Two tools can help them do this. The first, called the risk matrix, graphically reveals the distribution of risk across a company's entire innovation p

Defeating feature fatigue.

Consider a coffeemaker that offers 12 drink options, a car with more than 700 features on the dashboard, and a mouse pad that's also a clock, calculator, and FM radio. All are examples of "feature bloat", or "featuritis", the result of an almost irresistible temptation to load products with lots of bells and whistles. The problem is that the more features a product boasts, the harder it is to use. Manufacturers that increase a product's capability--the number of useful functions it can perform--at the expense of its usability are exposing their customers to feature fatigue. The authors have co

Global strategies for the diffusion of robotic surgery.

BACKGROUND: The global adoption of robotic surgery has advanced rapidly in high-income countries, yet its diffusion remains limited in resource-constrained settings due to financial, infrastructural, and educational barriers. As surgical technology evolves, there is an urgent need to promote countries' equitable access to robotic platforms worldwide. AIMS: The aim of this study was to analyze global strategies employed to promote the diffusion of robotic surgery, with a particular focus on overcoming barriers in resource-limited settings, and to provide practical insights that can guide its e

How Can Urodynamic Innovations be Better Integrated Into the Clinical Pathway? ICI-RS 2025.

BACKGROUND: Urodynamics has seen, in common with any medical field reliant upon technology, many recent advances in the application of innovations. Novel and effective ideas have been developed and marketed, but relatively few have been incorporated into clinical practice. METHODS: A debate held at the International Consultation on Incontinence-Research Society (ICI-RS) meeting in Bristol, UK, in June 2025 looked at new technology and its possible inclusion into the patient pathway. RESULTS: Discussion acknowledged that new ideas have not always been comprehensively examined, assessed, or ap

Innovations in Care of the Elderly Hip Fracture Patient; a Nightmare No More.

Hip fractures that occur in the elderly population continue to be a growing problem in communities. In this particular population, people 65 years and older, patients often have multiple comorbidities. These comorbidities cause challenges for a patient's optimization preoperatively and throughout the continuum of care. Although unplanned and traumatic, hip fracture patients can benefit from care organized in a comprehensive and standardized way. Having a hip fracture program that is driven by a multidisciplinary team approach has proved to decrease length of stay and promote positive outcomes

Create three distinct career paths for innovators.

Large companies say they Create Three Distinct want to be Career Paths for Innovators innovative, but they fundamentally mismanage their talent. Expecting innovators to grow along with their projects-from discovery to incubation to acceleration--sets them up to fail. Most people excel at one of the phases, not all three. By allowing innovation employees to develop career paths suited to their strengths, companies will create a sustainable innovation function.

The innovation value chain.

The challenges of coming up with fresh ideas and realizing profits from them are different for every company. One firm may excel at finding good ideas but may have weak systems for bringing them to market. Another organization may have a terrific process for funding and rolling out new products and services but a shortage of concepts to develop. In this article, Hansen and Birkinshaw caution executives against using the latest and greatest innovation approaches and tools without understanding the unique deficiencies in their companies' innovation systems. They offer a framework for evaluating

When hospitals build their own AI: a comparison of the EU and US regulatory framework.

Hospitals are increasingly shifting from being passive purchasers of artificial intelligence (AI) systems to active developers of medical AI, particularly in the form of in-hospital machine learning tools tailored to local clinical needs. This paper examines how the European Union (EU) and the USA regulate in-hospital-developed AI-based medical devices, focusing on the legal and institutional conditions that shape innovation, oversight and diffusion. Through a comparative analysis across three dimensions-responsiveness to clinical need, distribution of innovative capacity and pathways for scal

Clarifying dissemination outcomes: a taxonomy to advance dissemination science.

BACKGROUND: Dissemination science lacks shared language for specifying what dissemination strategies are designed to achieve. Without clearly defined outcomes, researchers cannot design studies that explain why dissemination strategies succeed or fail, compare findings, or build cumulative evidence about how dissemination works. METHODS: We drew on dissemination scholarship, communication and behavior theory, innovation diffusion, public policy, and organizational readiness frameworks, and two decades of applied experience within a national HIV research network, to identify and iteratively re

Value innovation: an important aspect of global surgical care.

INTRODUCTION: Limited resources in low- and middle-income countries (LMICs) drive tremendous innovation in medicine, as well as in other fields. It is not often recognized that several important surgical tools and methods, widely used in high-income countries, have their origins in LMICs. Surgical care around the world stands much to gain from these innovations. In this paper, we provide a short review of some of these successful innovations and their origins that have had an important impact in healthcare delivery worldwide. REVIEW: Examples of LMIC innovations that have been adapted in high

Disruptive innovation for social change.

Countries, organizations, and individuals around the globe spend aggressively to solve social problems, but these efforts often fail to deliver. Misdirected investment is the primary reason for that failure. Most of the money earmarked for social initiatives goes to organizations that are structured to support specific groups of recipients, often with sophisticated solutions. Such organizations rarely reach the broader populations that could be served by simpler alternatives. There is, however, an effective way to get to those underserved populations. The authors call it "catalytic innovation.
