One-food versus 4-food elimination diet for pediatric eosinophilic esophagitis: A multisite randomized trial.

BACKGROUND: A 6-food elimination diet in pediatric eosinophilic esophagitis (EoE) is difficult to implement and may negatively affect quality of life (QoL). Less restrictive elimination diets may balance QoL and efficacy. OBJECTIVE: We performed a multisite, randomized comparative efficacy trial of a 1-food (milk) elimination diet (1FED) versus 4-food (milk, egg, wheat, soy) elimination diet (4FED) in pediatric EoE. METHODS: Patients aged 6 to 17 years with histologically active and symptomatic EoE were randomized 1:1 to 1FED or 4FED for 12 weeks. Primary end point was symptom improvement by

A randomized controlled pilot study into the effects of a restricted elimination diet on family structure in families with ADHD and ODD.

BACKGROUND: Behavioural improvements of children with attention-deficit hyperactivity disorder (ADHD) and oppositional defiant disorder (ODD) following a restricted elimination diet (RED), may be due to concurrent changes in family environment. METHODS: Twenty-four children with ADHD, were randomized to either a 5-week RED intervention, or a control intervention consisting of healthy food advices in a pilot study. RESULTS: No differences in family environment were found, neither at baseline nor when comparing the start and end measurements of both groups. CONCLUSIONS: In this pilot study, t

Outcomes of Serum Food-Specific Immunoglobulin G 4 to Guide Elimination Diet in Patients With Eosinophilic Esophagitis.

INTRODUCTION: Eosinophilic esophagitis (EoE) is associated with atopy; however, recent studies have identified an association with food-specific immunoglobulin G 4 (FS-IgG 4 ) rather than immunoglobulin E antibodies. This study aimed to evaluate the role of serum FS-IgG 4 in guiding an elimination diet and its outcomes. METHODS: Patients with and without EoE were enrolled in a prospective, controlled, single tertiary center trial. Serum FS-IgG 4 titers, esophageal eosinophil counts, and dysphagia symptom questionnaire scores were assessed, and participants with elevated FS-IgG 4 (ImmunoCAP, c

[Food intolerance and irritable bowel syndrome of childhood: clinical efficacy of oral sodium cromoglycate and elimination diet].

Irritable bowel syndrome (IBS) is recognized to be a common cause of chronic diarrhea without failure to thrive in childhood. Several studies stressed the role of food intolerance as a major factor in the pathogenesis of IBS. The aim of this multicenter study was to investigate the offending role of food in IBS and to compare the therapeutic role of oral sodium cromoglycate versus elimination diet. 153 patients (mean age 4 years) with diarrhea (> 3 stools per day for four days in a week) and abdominal pain for about 10 months were enrolled in this trial. About half of the patients had a family

The results of skin testing with food additives and the effect of an elimination diet in chronic and recurrent urticaria and recurrent angioedema.

Ninety-one subjects suffering from chronic or recurrent urticaria or recurrent angioedema of 2 months to 17 yr duration were skin-prick and scratch tested with 18 various food additives. Twenty-four of the subjects (26%) had at least one histamine equivalent skin test (ST) reaction. In contrast, only 24 (10%) of 247 non-urticaria control subjects showed comparable results. An oral provocation test with food additives was carried out on 10 of the 24 ST-positive subjects with urticaria or angioedema, but only one positive reaction, caused by benzoic acid, was recorded. All 91 subjects were advis

The Frequency and Impact of Self-Imposed Elimination Diets on the Nutritional Status and Clinical Course of Disease in Children with Inflammatory Bowel Disease.

BACKGROUND AND AIMS: From the patients' perspective, diet has a relevant role in triggering symptoms of inflammatory bowel disease (IBD). There is a lack of prospective studies regarding the diet of children with IBD. The aim of this study was to assess the frequency and impact of self-imposed elimination diets on the nutritional status and clinical course of disease in the pediatric population. METHODS: This was a prospective case-control study that included newly diagnosed patients with IBD and healthy controls (age/sex-matched peers and siblings) over a one-year period. The participants we

Rational Use of Elimination Diets in Infants and Toddlers: Navigating the Overlap Between Disorders of Gut-Brain Interaction (DGBI) and Food Allergy-A Narrative Review.

Disorders of gut-brain interaction (DGBI) are the most common cause of gastrointestinal symptoms in infants and toddlers. The overall prevalence in the pediatric population aged 0-48 months is approximately one in four children. DGBI symptoms such as excessive crying, regurgitation, constipation or altered bowel movements can often mimic or overlap with food allergy (FA) manifestations, especially non-IgE-mediated allergy, thus posing a diagnostic and therapeutic challenge. As a consequence, prolonged empiric elimination diets are often initiated in DGBI management despite limited evidence-bas

Eliminating Animal Products from the Diet Puts Dutch Older Adults at Risk of Micronutrient Deficiencies: A Simulation Study.

BACKGROUND: Transitioning to more sustainable eating habits with less animal-based protein affects not only protein intake but also micronutrient consumption. Given that older adults are already vulnerable to insufficient micronutrient amounts, such dietary changes could further heighten their risk of deficiencies. OBJECTIVES: We simulated how switching toward a more sustainable eating pattern influenced micronutrient intake in older adults. METHODS: Data from the Dutch National Food Consumption Survey 2019-2021 (community-dwelling older adults, n = 607, food intake based on 2-d food diary)

Elimination of fibrin polymer formation or crosslinking, but not fibrinogen deficiency, is protective against diet-induced obesity and associated pathologies.

BACKGROUND: Obesity predisposes individuals to metabolic syndrome, which increases the risk of cardiovascular diseases, non-alcoholic fatty liver disease (NAFLD), and type 2 diabetes. A pathological manifestation of obesity is the activation of the coagulation system. In turn, extravascular fibrin(ogen) deposits accumulate in adipose tissues and liver. These deposits promote adiposity and downstream sequelae by driving pro-inflammatory macrophage function through binding the leukocyte integrin receptor αM β2 . OBJECTIVES: An unresolved question is whether conversion of soluble fibrinogen to a

Six-Food Elimination Diet Is Less Effective During Pollen Season in Adults With Eosinophilic Esophagitis Sensitized to Pollens.

INTRODUCTION: The role of inhaled and swallowed aeroallergens in treatment outcomes of adult patients with eosinophilic esophagitis (EoE) is unclear. We hypothesized that the pollen season contributes to the failure of the 6-food elimination diet (SFED) in EoE. METHODS: We compared outcomes of patients with EoE who underwent SFED during vs outside of the pollen season. Consecutive adult patients with EoE who underwent SFED and skin prick test (SPT) for birch and grass pollen were included. Individual pollen sensitization and pollen count data were analyzed to define whether each patient had b

Changes in mucosal IgG4+- and IL-10+-cell frequencies in adults with eosinophilic esophagitis on a two-food elimination diet.

Eosinophilic esophagitis (EoE) is increasingly diagnosed in patients with dysphagia. Type-2 immunity can induce EoE histopathology via non-IgE-dependent mechanisms, possibly involving IgG4 and IL-10. To elucidate the contribution of this response to EoE pathogenesis, we examined its association with clinical and histologic endpoints in adult EoE patients given a two-food elimination diet. IgG4- and IL-10-expressing cells were counted in esophageal biopsies and serum food-specific IgG4 measured at baseline and follow-up. Variables were correlated with histologic measures of disease activity. Pa

Long-Term Outcomes of the Six-Food Elimination Diet and Food Reintroduction in a Large Cohort of Adults With Eosinophilic Esophagitis.

INTRODUCTION: Eosinophilic esophagitis (EoE) is an immune-mediated inflammatory condition with tissue eosinophilia resulting in esophageal dysfunction. The six-food elimination diet (SFED) is an EoE treatment approach that removes milk, wheat, soy, eggs, tree nuts/peanuts, and fish/shellfish. After histologic remission, food reintroduction occurs to identify a food trigger. Outcomes from large series of adults undergoing SFED and food reintroduction as clinical care are not known. METHODS: A retrospective review (2006-2021) of adult patients with EoE from an academic center was completed. Pat

Effects of an elimination diet and a healthy diet in children with Attention-Deficit/Hyperactivity Disorder: 1-Year prospective follow-up of a two-arm randomized, controlled study (TRACE study).

BACKGROUND: An Elimination Diet (ED) or Healthy Diet (HD) may be effective in reducing symptoms of Attention-Deficit/Hyperactivity Disorder (ADHD), but long-term maintenance effects and feasibility have never been examined. METHODS: One-year prospective follow-up of a sample of 165 children (5-12 years) with ADHD randomized (unblinded; 1:1) to 5 weeks treatment with either ED (N = 84) or HD (N = 81) and a non-randomized comparator arm including 58 children being treated with Care as Usual (CAU). Dietary participants were allowed to add or switch to CAU treatment after 5 weeks. The primary out

Effects of a six-food elimination diet on symptoms, quality of life, and metabolomic profile in patients with functional dyspepsia: An exploratory study.

BACKGROUND: Functional dyspepsia (FD) is a disorder of gut-brain interaction, characterized by meal-related symptoms and growing evidence of duodenal dysfunction, including compromised mucosal integrity and immune activation. While proton pump inhibitors (PPIs) are considered the first-line treatment, many patients continue to experience symptoms and are often treated with neuromodulators, which can be limited by tolerability issues. Dietary interventions targeting luminal triggers may serve as a metabolically more stable, mechanistically relevant alternative therapy. AIM: To evaluate the cli

A Novel, IBS-Specific IgG ELISA-Based Elimination Diet in Irritable Bowel Syndrome: A Randomized, Sham-Controlled Trial.

BACKGROUND & AIMS: Personalized dietary therapies for irritable bowel syndrome (IBS) are needed and an immunoglobulin (Ig)G-antibody-based elimination diet presents a potential solution. However, existing studies have serious methodological limitations. This study aimed to assess the efficacy of an elimination diet by using a novel IBS-specific IgG assay. METHODS: We conducted a randomized, double-blind, sham-controlled trial enrolling subjects with IBS from 8 centers. Subjects positive for ≥1 food on an 18-food IgG assay and an average daily abdominal pain intensity score between 3.0 and 7.5

Uteroplacental Insufficiency Impairs Cholesterol Elimination in Adult Female Growth-Restricted Rat Offspring Fed a High-Fat Diet.

Uteroplacental insufficiency (UPI) causes intrauterine growth restriction (IUGR) and increases the risk of hypercholesterolemia and cardiovascular disease, which are leading causes of morbidity and mortality worldwide. Little is known about the mechanism through which UPI increases cholesterol. Hepatic Cholesterol 7 alpha-hydroxylase (Cyp7a1) is the rate-limiting and most highly regulated step of cholesterol catabolism to bile acids. Cholesterol 7 alpha-hydroxylase is regulated by transcription factor liver X receptor α (Lxrα) and by microRNA-122. We previously showed that microRNA-122 inhibit

Food immediate-contact hypersensitivity (FICH) and elimination diet in young children with atopic dermatitis. Preliminary results in 107 children.

In atopic dermatitis [AD], not only food consumption, but direct skin-contact too can provoke hypersensitivity reactions. We imitated food immediate-contact hypersensitivity [FICH] to cow's milk, egg, peanut or soy by a skin provocation test. This skin application food test [SAFT] was applied in 91 patients aged up to 5 years and suffering from AD, and in 16 healthy controls (all SAFT-negative). In the SAFT-positive patients (n = 61), FICH to egg was observed in 72%, to cow's milk in 47%, to peanut in 34% and soy in only 1 patient. SAFT and RAST scores correlated weakly. Nevertheless, many dis

IgG positive food elimination diet altered gut microbiota, decreased zonulin in migraine adults: a pilot dietary intervention trial.

BACKGROUND AND AIMS: Gut microbiota and intestinal permeability may play a role in food specific IgG antibodies mediated delayed hypersensitivity in migraine patients. We aimed to evaluate the effect of food specific IgG-guided elimination strategy by assessing the changes in gut microbiota, zonulin as well as their correlation with symptoms, IL-6, TNF-α and CGRP. METHODS: The study included 52 migraine patients in true diet group (excluding IgG positive foods) and 46 controls in sham diet group (excluding alternative IgG negative foods).
