The nationwide study of epikeratophakia for aphakia in older children.

A nationwide study of epikeratophakia for aphakia in older children was conducted from March 1984 to March 1986. Sixty-three patients, 8 to 18 years of age, underwent this procedure in 65 eyes. Twenty-eight patients had congenital cataracts and 35 had traumatic cataracts. Fifty-one of the 65 eyes were aphakic at the time of surgery (secondary procedures). All surgeries were successful; no tissue lenses were lost or removed. Postoperatively, 73% of the patients were within 3 diopters (D) of emmetropia. The patients with congenital cataracts gained an average of one Snellen line of best-correcte

[Epikeratophakia--a new procedure in refractive surgery of the cornea].

The epikeratophakia operation developed by Kaufman represents an additional procedure in refractive surgery of the cornea. The application of individually prepared corneal tissue lenticles on top of the recipient cornea can be used in refractive (Keratolens) and tectonic (Keratopatch) problems. The most important indication for this operation is monocular aphakia in which a contact lens is not tolerated. As is pointed out in the literature, postoperative visual acuity is reduced in some patients by interface densities and incomplete epithelialization. The advantage of this newly developed meth

Epikeratophakia: the surgical correction of aphakia. Update: 1982.

Sixty-five epikeratophakia procedures have been performed in 63 patients; visual acuity data have been tabulated on 31 patients with 4 to 30 months follow-up. Patients with more than a year of follow-up showed stable keratometry readings. Early patients achieved 70% of the predicted dioptric correction; more recent patients have achieved 87% with improvements in the lathing procedure, tissue handling, and surgical technique. Visual acuities improve with time. At any given time after surgery, acuities measured with a hard contact lens are better than those measured with spectacles; the decrease

[Surgical therapy of keratoconus. Epikeratophakia versus penetrating keratoplasty].

Fifteen consecutive keratoconus patients undergoing epikeratophakia and 10 consecutive keratoconus patients undergoing penetrating keratoplasty were included in a prospective study aimed at comparing the visual and refractive results of the two surgical methods. All patients had a minimum postoperative follow-up time of 12 months. Although patients undergoing epikeratophakia showed a tendency for quicker visual rehabilitation, visual acuity was comparable in the two groups as early as 3 months after surgery. The refractive results did not differ significantly in the two groups 12 months postop

Five year follow-up of epikeratophakia in children.

Epikeratophakia alters the anterior curvature of the cornea by the addition of a machine-carved donor lenticule. Since March 1980, 88 patients under eight years of age have had epikeratophakia, with at least six months of follow-up. Eighty percent of the original surgeries were successful; some failed grafts were replaced successfully, so that in all, 89% of the patients had successful grafts. The average increase in curvature of the cornea was 14.7 diopters, and the average spectacle overcorrection was +0.56 diopters. In these growing eyes, we documented a myopic shift of 1.5 diopters per yea

Epikeratophakia -- the surgical correction of aphakia. Update 1981.

Epikeratophakia grafts used to rehabilitate vision in adult monocular aphakic patients intolerate of contact lens correction or not candidates for IOLs were studied prospectively. Thirty-two patients who entered this study between its inception in February 1980 through February 1981 were followed for at least eight months. Patients with normal visual potential before surgery had average visual acuities with spectacle overcorrection of 20/50 at four months, 20/40 at eight months, and 20/30 at one year. Few long-term complications were seen. Empirical modification of the lathing process has decr

The use of epikeratophakia grafts in pediatric monocular aphakia.

Epikeratophakia is a form of refractive surgery in which the recipient's epithelium is removed and a pre-shaped donor lenticule is sutured to the patient's cornea. Seventeen patients ranging in age from two months to 6.6 years received 19 epikeratophakia grafts for the correction of aphakic vision. The correction provided by the graft in combination with vigorous amblyopia therapy has yielded some improvement in vision in the 12 patients with successful grafts. This procedure may be particularly suitable for pediatric aphakic patients because it is extraocular and reversible and may be employe

Epikeratophakia to correct traumatic aphakia after penetrating keratoplasty.

A 19-year-old man with keratoconus sustained ocular trauma and became aphakic in his operated left eye 2 months after penetrating keratoplasty. Original corneal wound repair was performed without intraocular lens implantation. Attempts to correct his aphakia with a contact lens failed when the patient became intolerant to its use. As an alternative, the patient had elective epikeratophakia. A standard 8.5 mm lenticule was placed over existing corneal graft. This operation resulted in +12.25 diopters of correction and a best corrected visual acuity of 20/25 at 30 months postoperatively. There w

In vivo evaluation of epikeratophakia lenses by means of Scheimpflug photography.

The recovery of clarity in frozen, lyophilized human tissue lenticules used for epikeratophakia has been a matter of controversy. According to some authors, several months are necessary for the lenticules to resume normal transparency; others report clinical experience of much shorter times. Up to now, objective documentation of such findings has not been substantiated. We used a Topcon LS-45 camera to photograph four eyes that underwent epikeratophakia, preoperatively and at 2, 4, 6, and 8 weeks after surgery, and analyzed the film negatives by linear microdensitometry. The degree of film bla

Epikeratophakia with commercially prepared tissue for the correction of aphakia in adults.

The records of all adult patients at Louisiana State University Eye Center, New Orleans, who underwent epikeratophakia for aphakia with commercially prepared tissue since February 1984 were reviewed. The epikeratophakia lenticule was maintained in 37 (92.5%) of 40 patients; complications required the removal of three tissue lenses. The average refractive error three months after surgery was +0.67 +/- 1.97 diopters; 25 (90%) of 28 patients were within 3 D of emmetropia. At six months, the average refractive error was -0.18 +/- 2.27 D. Fourteen (58%) of 24 patients whose visual acuity was 20/40

Keratocyte activity in wound healing after epikeratophakia in rabbits.

Epikeratophakia is a refractive surgical procedure for the correction of aphakia, high myopia, or keratoconus. To solve clinical problems associated with epikeratophakia, a basic knowledge of its postoperative healing process is needed. The authors investigated keratocyte activities, particularly cell proliferation and collagen synthesis, during wound healing after epikeratophakia in rabbits. Epikeratophakia was done on rabbit corneas with a homologous cryolathed keratolens. Ten, 16, 28, 45, 63, 90, 254, and 360 days after the operation, the corneas were excised, labeled with either 3H-thymidi

Epikeratophakia following previous refractive keratoplasty surgery: two case reports.

Epikeratophakia is becoming a significant and worthwhile surgical modality for selected cases of pediatric and adult aphakia, keratoconus, and myopia. Ultimately, it may also provide a safe and effective means of obtaining satisfactory surgical correction when the results from a previous refractive technique are insufficient. We present two successful cases of epikeratophakia following refractive keratoplasty: one following radial keratotomy for myopia and one following penetrating keratoplasty for keratoconus.

[Complications of epikeratophakia in correction of aphakia, myopia, hyperopia and keratoconus].

Epikeratophakia for the correction of aphakia, myopia, hyperopia and keratoconus has overcome many of the objections to earlier lamellar refractive procedures (keratophakia and keratomileusis), such as the lamellar dissection of the recipients' healthy cornea over the visual axis and the need for expensive surgical equipment. It is reasonably reversible and repeatable. The complications observed in a series of 119 operations performed at the 2nd Department of Ophthalmology, University of Vienna, are discussed and compared with those reported in the literature. Serious complications, both intra

Modified surgical technique for repeated epikeratophakia surgery in aphakic eyes.

BACKGROUND: Incomplete epithelial removal from the peripheral keratectomy at the time of repeat epikeratophakia surgery can lead to interface downgrowth and consequent failure, as observed in the case reported herein. We present a modification of the standard surgical technique aimed at avoiding such complication. METHODS: A 69-year-old white female underwent a third aphakic epikeratophakia modified to reduce the risk of epithelial interface downgrowth, which had caused a previous failure. The aphakic tissue lens, 8.5 mm in diameter, was sutured into a corneal pocket obtained at a deeper leve

[Keratocyte activity in wound healing process following epikeratophakia in rabbits].

Epikeratophakia is a refractive surgery for the correction of aphakia, high myopia and keratoconus. Although many clinical studies of epikeratophakia have been performed, its wound healing process is not well understood. Traumatic aphakia treated with an iris prosthesis/intraocular lens or epikeratophakia.

OBJECTIVE: We retrospectively analyzed the visual results and postoperative complications associated with severely traumatized eyes in which aphakia was corrected with epikeratophakia or a sutured iris prosthesis/intraocular lens (IOL). METHODS: Fourteen eyes (14 patients) with traumatic aphakia and severe anterior segment complications were corrected either with epikeratophakia or a sutured iris prosthesis/IOL. All eyes lacked lens capsule or iris support for an IOL. The surgical technique of implanting an iris prosthesis/IOL employed transcleral suturing in the ciliary sulcus combined with

Epikeratophakia for the correction of myopia: lenticule design and related histopathological findings.

In spite of the numerous changes in surgical technique, epikeratophakia for the correction of myopia is still far from achieving optimal results. The removal rate of myopic lenticules is much higher than that of aphakic or keratoconus lenticules and has prompted the FDA to deny its approval to the industrial production of epi-lenses for the correction of myopia. We report a patient who underwent epikeratophakia for the correction of high myopia in his left eye and required removal of the lenticule because of unsatisfactory visual outcome. The histologic and electron microscopic examination of

Results of penetrating keratoplasty after epikeratophakia for keratoconus in the nationwide study.

In the Nationwide Study of Epikeratophakia, 22 eyes underwent penetrating keratoplasty (PKP) after epikeratophakia for keratoconus. Reasons for PKP included visual acuity unsatisfactory to the patients (6 eyes), recipient corneal scarring (4 eyes), recipient Descemet's folds (3 eyes), scarring in the tissue lens (2 eyes), vascularization of the tissue lens (2 eyes), astigmatism (2 eyes), failure of the tissue lens to reepithelialize (1 eye), corneal ulcer (1 eye), and scarring of the donor and recipient cornea (1 eye). After PKP, all patients had clear grafts and 20/40 or better visual acuity.

[New central, epithelial iron deposit following epikeratophakia in high-grade myopia].

Following epikeratophakia for the correction of myopia an epithelial iron dot was observed in the center of the lenticule as early as five months following surgery. Biomicroscopic examination of 55 cases revealed indistinct borders, a yellow to dark brown coloration and a localization in the deep epithelial layers. It was apparent in 100% of the patients one year after surgery, with no tendency to regression over a follow-up period of up to 23 months, and did not appreciably affect postoperative visual acuity. The occurrence of this iron dot is attributed to a localized pooling of tear fluid i
