Tough Decisions for Premature Triplets.

When infants are born at the borderline of viability, doctors and parents have to make tough decisions about whether to institute intensive care or provide only palliative care. Often, these decisions are made in moments of profound emotional turmoil, and parents receive different information from different health professionals. Communication can become garbled. It may be difficult to tell when and whether the patient's clinical condition has changed enough so that certain choices that had once been permissible become impermissible. In this "Ethics Rounds," we present a case of triplets born a

Credentialing the clinical ethics consultant: an academic medical center affirms professionalism and practice.

In response to national trends calling for increasing accountability and an emerging dialogue within bioethics, we describe an effort to credential clinical ethicists at a major academic medical center. This effort is placed within the historical context of prior calls for credentialing and certification and efforts currently underway within organized bioethics to engage this issue. The specific details, and conceptual rationale, behind the NewYork-Presbyterian Hospital's graduated credentialing plan are shared as is their evolution and ratification within the context of institutional policy. Distanced perspectives: AIDS, anencephaly, and AHP.

US court decisions guaranteeing life-sustaining care to anencephalic infants have been viewed with disfavor, and sometimes disbelief, by some ethicists who do not believe in the necessity of life-sustaining support for those without cognitive abilities or an independently sustainable future. The distance between these two views--one legal and inclusive, the other medical and specific--seems unbridgeable. This paper reports on a program using multicriterion decision making to define and describe persons in a way which both acknowledges the differences perceived by many as well as those commonal

Specifying Specification.

This paper tackles the accusation that applied ethics is no serious academic enterprise because it lacks theoretical bracing. It does so in two steps. In the first step I introduce and discuss a highly acclaimed method to guarantee stability in ethical theories: Henry Richardson's specification. The discussion shows how seriously ethicists take the stability of the connection between the foundational parts of their theories and their further development as well as their "application" to particular problems or cases. A detailed scrutiny of specification leads to the second step, where I use ins

Key Stakeholders' Perceptions of Motivators for Research Participation Among Individuals Who Are Incarcerated.

Understanding motivations of research participants is crucial for developing ethical research protocols, especially for research with vulnerable populations. Through interviews with 92 institutional review board members, prison administrators, research ethicists, and researchers, we explored key stakeholders' perceptions of what motivates incarcerated individuals to participate in research. Primary motivators identified were a desire to contribute to society, gaining knowledge and health care, acquiring incentives, and obtaining social support. The potential for undue influence or coercion wer

Spinal muscular atrophy type I: do the benefits of ventilation compensate for its burdens?

We report the progress of an 8-year-old child with spinal muscular atrophy (SMA) type 1. The parents elected in infancy that the child should be on long-term ventilation, but all attempts to establish this care at home have failed, so the child remains ventilated in the hospital. The leader of the long-term ventilation team reports on the child's progress and describes a week in the child's life. Two paediatricians argue that the benefits of long-term ventilation have not and do not compensate the child for the burdens imposed on her by this treatment and explain why they would not support the

Community and justice: the challenges of bicultural partnership to policy on assisted reproductive technology.

Listening to other cultures offers challenges to our fundamental assumptions and worldviews. In New Zealand public policy on Assisted Reproductive Technology (ART) is being worked out in a society committed to the development of bicultural partnership honouring the Treaty of Waitangi, a treaty with the indigenous people. Strong claims to the cultural significance of genetic heritage by Maori have made apparent to non-Maori (Pakeha) their own assumptions. These claims also resist reductive understandings of genetics. In this paper I review, as a Pakeha ethicist, initiatives taken in New Zealand

Treating the SARS-CoV-2-positive patient with cancer: A proposal for a pragmatic and transparent ethical process.

The treatment of patients with cancer who test positive for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) poses unique challenges. In this commentary, the authors describe the ethical rationale and implementation details for the creation of a novel, multidisciplinary treatment prioritization committee, including physicians, frontline staff, an ethicist, and an infectious disease expert. Organizational obligations to health care workers also are discussed. The treatment prioritization committee sets a threshold of acceptable harm to patients from decreased cancer control that is

The persistent vegetative state and anencephaly: problematic paradigms for discussing futility and rationing.

Medical futility has been a controversial issue for the last several years. Although anencephalic infants and patients in the persistent vegetative state (PVS) have often been suggested as paradigmatic cases for discussing the issue of medical futility, there is no consensus among physicians or ethicists that treatment of such patients is futile and courts have not supported the futility argument. Focusing on the issue of medical futility will not resolve the management issues surrounding the care of patients in PVS or anencephalic infants. Instead, an analysis of the benefits and burdens of t

Including a Transgender Adolescent in a Support Group.

This month's Ethics Rounds presents a case of an adolescent and her parent who object to the presence of a transgender adolescent in an established sexual trauma support group. To what extent do clinicians have a duty to consider the most beneficial therapeutic setting for all attendees? To what extent should the religious beliefs of a cisgender adolescent or the identity of a transgender attendee impact the group attendance of others? The case raised an opportunity for ethicists, therapists, an adolescent medicine physician, a social worker, a chaplain, an attorney and conflict mediator, and

Pride Before a Fall: Shame, Diagnostic Crossover, and Eating Disorders.

This paper discusses the findings of qualitative research that examined the accounts of five "mostly recovered" ex-patients who had experienced transition between two or more eating disorder diagnoses. This study found that, in the minds of participants, the different diagnostic labels were associated with various good or bad character traits. This contributed to the belief in a diagnostic hierarchy, whereby individuals diagnosed with anorexia nervosa were viewed as morally better than those diagnosed with bulimia nervosa or binge eating disorder. Consequently, diagnostic crossover from a "bet

The Value of Knowing When: An Ethical Analysis of Onset Prediction in Genetic Neurodegenerative Diseases.

Biomarker-based technologies for predicting age of onset are currently being developed for carriers and individuals at 50% risk of autosomal dominant neurodegenerative diseases such as Huntington's disease, frontotemporal dementia and spinocerebellar ataxias. Qualitative interview studies indicate that carriers and individuals at 50% risk expect that onset predictions would be valuable for life planning but would also impact mental health. Ethically responsible implementation of biomarker-based onset prediction in research and clinical settings requires maximizing the utility and minimizing ne

Neuroethics, confidentiality, and a cultural imperative in early onset Alzheimer disease: a case study with a First Nation population.

The meaningful consideration of cultural practices, values and beliefs is a necessary component in the effective translation of advancements in neuroscience to clinical practice and public discourse. Society's immense investment in biomedical science and technology, in conjunction with an increasingly diverse socio-cultural landscape, necessitates the study of how potential discoveries in neurodegenerative diseases such as Alzheimer disease are perceived and utilized across cultures. Building on the work of neuroscientists, ethicists and philosophers, we argue that the growing field of neuroet

The responsibility of bioethicists: The case study of Yemen.

In this article, we describe in detail the health and general living conditions resulting from the ongoing armed conflict in Yemen, including the historical and geopolitical underpinnings. In addition to mere reporting, we use Yemen as a case study to examine the responsibility of bioethicists in general. We find it unacceptable that bioethics neglects the largest humanitarian crisis taking place in the world at the moment as well as the largest Cholera outbreak in history. We argue that bioethicists should do more to address armed conflicts and their resulting basic human rights violations. W

Independent donor ethical assessment: aiming to standardize donor advocacy.

Living organ donation has become more common across the world. To ensure an informed consent process, given the complex issues involved with organ donation, independent donor advocacy is required. The choice of how donor advocacy is administered is left up to each transplant center. This article presents the experience and process of donor advocacy at University of Texas Southwestern Medical Center administered by a multidisciplinary team consisting of physicians, surgeons, psychologists, medical ethicists and anthropologists, lawyers, a chaplain, a living kidney donor, and a kidney transplant

Defining the threshold of birth: ethical introduction of artificial placenta and artificial womb technology in the neonatal intensive care unit.

Artificial amnion and placenta technology (AAPT), also colloquially referred to as artificial womb technology, will likely move into first-in-human trials in the next few years. The results of the AAPT animal trials inspired numerous commentaries in the ethics literature, with some ethicists and philosophers advocating for the creation of a new "status of being" to describe a patient supported by AAPT. Proposed states of being include terms such as: "gestateling" (a fetus outside the womb) or "fetonate" (a neonate with full human rights but retained fetal physiology). Adopting the "gestateling

[Opioid dependent patient with two prescribers].

Opioid use has risen again in the past year, partly due to overtaken operations. Some of the patients undergoing surgery already chronically use opioids. A fentanyl dependent patient enters the pharmacy with a oxycodone prescription from the orthopedist. The pharmacist doubts whether the orthopedist has the intention to continue the fentanyl use. The patient does not want the pharmacist to contact the orthopedist or the general practitioner who prescribes the fentanyl. The pharmacist experiences a dilemma with multiple handling options and reflects on them based on the professional values that
