This seven-case anecdotal report is presented to alert physicians to the possibility of the formation of functional ovarian cysts during use of phasic contraceptive pills. The patients were studied by the combination of history, physical examination, ultrasound examination, and in two cases surgery. The occurrence of functional ovarian cysts during the use of phasic contraceptive pills is well established while not well appreciated by practicing physicians. Levonorgestrel, as used in oral contraceptives, has antiovulatory activity at doses far lower than those producing androgenic effects. Triphasil, containing levonorgestrel was compared with Diane, containing cyproterone acetate in a trial of acne treatment. A triphasic oral contraceptive Trigynon (Schering) or Triphasil (Wyeth) was administered to 13 anovulatory women with hyperandrogenism, enlarged polycystic ovaries and reversal of LH/FSH ratio. After three months of treatment total Testosterone, Androstenedione and 17-OH-Progesterone levels significantly decreased while SHBG significantly increased. Reduction of hair growth and improvement of acne were noted. Side effects were minimal.

A case is reported of mesenteric venous occlusion in a young woman who by mistake received oral contraceptives during pregnancy. A resection of the damaged small bowel was performed followed by an end to end enteroenterostomy. It is suggested that the hypercoagulable state normally found in pregnancy was aggravated by the hormonal intake and lead to the mesenteric vein thrombosis.

A variety of benign liver tumors associated with the use of oral contraceptives has been described. However, there is controversy regarding the possible relation of focal nodular hyperplasia of the liver to oral contraceptive therapy.
