High fluoroquinolone MIC is associated with fluoroquinolone treatment failure in urinary tract infections caused by fluoroquinolone susceptible Escherichia coli.

BACKGROUND: Suboptimal clinical response to fluoroquinolone (FQ) therapy has been clearly documented in patients with Salmonella typhi infection with reduced FQ susceptibility. However, the clinical impact of reduced FQ susceptibility on other infections including E. coli urinary tract infections (UTIs) has never been evaluated. METHODS: We conducted a retrospective cohort study of female patients with fluoroquinolone susceptible E. coli (FQSEC) UTIs who received FQ therapy at outpatient services within University of Pennsylvania Health System, Philadelphia. Exposed patients were those with h

How Safe are Fluoroquinolones for Diabetic Patients? A Systematic Review of Dysglycemic and Neuropathic Effects of Fluoroquinolones.

INTRODUCTION: The US Food and Drug Administration issued safety warnings about neuropathy in 2013 and dysglycemia in 2018 caused by fluoroquinolone use, mainly based on case reports and case series. We conducted this systematic review to evaluate the safety of fluoroquinolones in diabetic patients by investigating their dysglycemic and neuropathic effects. METHODS: PubMed, Scopus, and Google Scholar were searched for randomized controlled trials and observational studies published from inception till September 2019 evaluating the safety of fluoroquinolones. Efficacy studies of fluoroquinolone

Fluoroquinolone resistance without fluoroquinolone use; the genie is truly out of the bottle.

The global dissemination of multidrug-resistant (MDR) bacteria is not merely a clinical byproduct of antibiotic overuse, but an evolutionary transition driven by highly successful, human-adapted clonal lineages. While MDR phenotypes are frequently associated with resistance to various drug classes, FQR-mediated by specific mutations in the Quinolone Resistance-Determining Regions (QRDR) of the chromosomal GyrA and ParC genes-stands out as a primary driver of clonal success. Contrary to the traditional "fitness cost" paradigm of antibiotic resistance, recent evidence suggests long-term commensa

Fluoroquinolone resistance in Mycobacterium tuberculosis: the effect of duration and timing of fluoroquinolone exposure.

RATIONALE: Fluoroquinolones are the most commonly prescribed antibiotic class in the United States. They have the potential to become first-line antituberculosis therapy, but the effect of fluoroquinolone use on fluoroquinolone resistance in Mycobacterium tuberculosis is not well characterized. OBJECTIVES: To determine the prevalence of and risk factors for fluoroquinolone-resistant tuberculosis in a large United States population. METHODS: We identified all people with culture-confirmed tuberculosis enrolled in TennCare (Medicaid) and reported to the Tennessee Department of Health from Janu

Influence of fluoroquinolone susceptibility on the therapeutic response of fluoroquinolone-treated bacterial keratitis.

OBJECTIVE: To estimate how a corneal isolate's minimal inhibitory concentration for a fluoroquinolone agent affects the rate of clinical response of bacterial keratitis to fluoroquinolone therapy. DESIGN: Prospective cohort study. METHODS: Six hundred sixty-three individuals with suspected bacterial keratitis underwent diagnostic corneal scraping and were treated with topical 0.3% ciprofloxacin solution or ointment. Of 407 patients with culture-confirmed bacterial keratitis, improvement and cure rates with ciprofloxacin monotherapy were estimated for 391 who had in vitro ciprofloxacin suscep

Flumequine, a fluoroquinolone in disguise.

Fluoroquinolone resistance in E. coli isolates from livestock in Europe remains high despite EMA restrictions on fluoroquinolone use in animals. However, flumequine, a quinolone not classified as a fluoroquinolone by various regulatory bodies, is still used in livestock in the Netherlands, Belgium, Greece and France. We investigated whether flumequine selects for the same resistance mechanisms in E. coli. Resistant and non-resistant E. coli isolates were obtained from caecal fermentation assays and broilers exposed to concentrations of flumequine and enrofloxacin. Flumequine usage leads to an

Gatifloxacin: a new fluoroquinolone.

Gatifloxacin is a new 8-methoxy-fluoroquinoline antimicrobial agent. It has enhanced activity against Gram-positive and atypical agents, while retaining broad-spectrum antiGram-negative activity. For example, the MIC(90) values for respiratory tract pathogens are < or = 0.5 microg/ml for organisms such as Streptococcus pneumoniae (regardless of penicillin susceptibility), Haemophilus influenzae (beta-lactamase positive or negative), Moraxella catarrhalis (beta-lactamase positive or negative), Legionella species, Mycoplasma pneumoniae, methicillin-sensitive Staphylococcus aureus, beta-haemolyti

Overview of fluoroquinolone safety.

The safety of the fluoroquinolone antimicrobial agents is reviewed, discussing documented and potential clinical and laboratory adverse effects and drug-drug interactions. In prospective, randomized, double-blind clinical trials comparing fluoroquinolones to nonquinolone drugs or placebo, the fluoroquinolones were not significantly different (22 studies) or were superior (5 studies) to comparison agents but were only rarely more toxic (2 studies). Adverse effects included mild gastrointestinal toxicities and less common but more problematic central nervous system toxicities. Clinically importa

[Relevance of fluoroquinolones prescriptions in medical service].

INTRODUCTION: Fluoroquinolones (FQ) are antibiotics which favour the emergence of resistance and remain widely prescribed in the French hospital environment. A focus of prescription recommendation was published by the French Infectious Diseases Society (SPILF) in 2015 in order to preserve their use. The pneumology-oriented medical service of Salon de Provence's hospital had high FQ consumption for the year 2015; thus a relevant assessment of prescriptions was carried out. METHODS: This retrospective study was conducted between January 1 and December 31, 2015 and concerned patients who receive

Recent updates of fluoroquinolones as antibacterial agents.

Fluoroquinolones remain one of the most important kind of antibacterial agents used nowadays. The emergence of more virulent and resistant strains of bacteria by the development of either mutated DNA-binding proteins or efflux pump mechanism for drugs is considered the main problem associated with the therapeutic use of these drugs. This situation participated in pushing researchers to design new fluoroquinolone derivatives, mainly with different substituents at C-7 to withstand these resistant strains of bacteria and to obtain a wider spectrum of activity including activity against anaerobic

Fluoroquinolones in children: update of the literature.

The use of fluoroquinolones (FQ), antibiotics, which have excellent pharmacokinetic and pharmacodynamic characteristics and a broad spectrum of action, has increased dramatically in recent years, both in adults and children. Numerous safety and efficacy data are now available on FQ use in children, particularly regarding the treatment of complicated infections of the urinary tract, typhoid fever, pneumonia, acute external otitis, acute media otitis, meningoencephalitis, sepsis in neutropenic children and multidrug-resistant tuberculosis (MDR-TB), though sometimes studies have the limitations t

[Pseudomonas aeruginosa corneal abscess refractory to fluoroquinolones].

PURPOSE/METHOD: To report a clinical case of a 21-year old male with a severe corneal abscess due to Pseudomonas aeruginosa refractory to intensive treatment with topical Ciprofloxacin. RESULTS/CONCLUSIONS: A corneal scrapping performed after a wash-out period showed Pseudomonas aeruginosa specimens, which were not sensitive to any of the fluoroquinolones tested. A fortified topical treatment with Ceftazidime healed the corneal abscess. Nowadays, the possibility of resistance to fluoroquinolones should be kept in mind, and we recommend treatment guided by the results of a microbiological stud

The role of fluoroquinolones in tuberculosis today.

Tuberculosis is a growing international health concern; it is the leading infectious cause of death in the world today. The fluoroquinolones are the most recent class of drugs offering hope in the fight against this disease. Ciprofloxacin, ofloxacin, levofloxacin and sparfloxacin are currently the most commonly used agents used against Mycobacterium tuberculosis (TB), with in vitro minimum inhibitory concentrations (MICs) of 0.1 to 4 mcg/ml. Resistance in TB to fluoroquinolones may occur spontaneously or may be acquired, especially when these agents are used inappropriately. Cross-resistance a

Oral fluoroquinolones for maintenance treatment of melioidosis.

Ciprofloxacin (20 mg/kg/d) or ofloxacin (12 mg/kg/d) given for a median of 15 weeks (range 12-40) were used for maintenance treatment of 57 adult patients with melioidosis. The median duration of follow-up in the 45 patients who complied with treatment and were followed for at least 6 months was 28 months (range 6-65). Fluoroquinolone treatment was well tolerated. There were 13 treatment failures (5 failures to respond, 8 relapses), a failure rate of 29% (95% confidence interval 17-43%). The median time to treatment failure was 7 months (range 2-26). These results are inferior to those with co

Is fluoroquinolone use controversial.

INTRODUCTION: Fluoroquinolones have been in clinical use for more than 40 years and have benefitted hundreds of millions of patients in both hospital and community settings. AREAS COVERED: Black box or prominent safety warnings from regulating agencies about rare but serious adverse events has mandated a rethinking about when and where to use these antimicrobial agents. Additionally, the critical role of some fluoroquinolones for treatment of Mycobacteria tuberculosis (TB) infection raises concerns about their use for mild community acquired infections. Regardless, fluoroquinolones remain ent
