Potentially inappropriate medications according to PRISCUS list and FORTA (Fit fOR The Aged) classification in geriatric psychiatry: a cross-sectional study.

Adverse drug reactions (ADRs) constitute a frequent cause of hospitalization in older people. The risk of ADRs is increased by the prescription of potentially inappropriate medications for older people (PIMs). The PRISCUS list and the FORTA classification represent established tools to detect PIMs. The aim of the present study was to examine the prevalence and characteristics of PIM prescriptions on the gerontopsychiatric ward of a university hospital in Germany. To this aim, medication charts of 92 patients (mean age 75.9 ± 7.7 years; 66.3% female) were analyzed on a weekly basis until patien

Life-Course Brain Health as a Determinant of Late-Life Mental Health: American Association for Geriatric Psychiatry Expert Panel Recommendations.

This position statement of the Expert Panel on Brain Health of the American Association for Geriatric Psychiatry (AAGP) emphasizes the critical role of life course brain health in shaping mental well-being during the later stages of life. Evidence posits that maintaining optimal brain health earlier in life is crucial for preventing and managing brain aging-related disorders such as dementia/cognitive decline, depression, stroke, and anxiety. We advocate for a holistic approach that integrates medical, psychological, and social frameworks with culturally tailored interventions across the lifes

Practical Guide of the Korean Association for Geriatric Psychiatry to Anti-Amyloid Monoclonal Antibody Therapy for Alzheimer's Disease: Focused on Lecanemab.

The advent of anti-amyloid monoclonal antibody (mAb) therapies represents a paradigm shift in the treatment of Alzheimer's disease (AD), from symptomatic relief to disease modification. Lecanemab, a humanized mAb targeting soluble Aβ protofibrils and plaque, received regulatory approval in Korea in July 2024, following global randomized controlled trial data demonstrating its efficacy to reduce amyloid burden and slow cognitive decline. However, the introduction of such therapies into routine clinical realm presents several practical and systemic challenges, particularly in the context of Kore

Vitamin B12, Homocysteine, and Cognitive Function in the Older Adults Attending a Tertiary Geriatric Psychiatry Center: A Retrospective Case Notes-Based Study.

BACKGROUND: Cognitive impairment in older adults has been linked to low serum Vitamin B12 (B12) and elevated homocysteine (HCY) levels, but evidence remains inconclusive. This study examined their relationship with cognitive impairment among older adults in India. METHODS: This retrospective case note-based study included 96 subjects: cognitively normal (n = 21), Alzheimer's disease (AD; n = 42), and other cognitive disorders (n = 33). The mean age was 70.1 (± 8.9) years, with 58% males. Cognitive function was assessed using the Hindi Mental Status Examination (HMSE). Serum B12 (pg/ml) and HC

Post-ICU Syndrome and the "3 Ds" in Geriatric Psychiatry: A Complicated Case Report of Major Depression With Psychosis in a 96-Year-Old Woman.

In the growing field of geriatric psychiatry, the "3 Ds"-depression, dementia, and delirium-are a complex clinical challenge, especially in patients with medical comorbidities. This is a case report of a 96-year-old Saudi woman with chronic kidney disease, heart failure, and recurrent hyponatremia presented with worsening sleep, depression, persecutory delusions, and hallucinations following an intensive care unit (ICU) stay for urinary tract infection. Examination revealed cognitive decline and depressive symptoms, with sodium at 123 mmol/L. After a psychiatric evaluation, she was diagnosed w

An integrated, collaborative healthcare model for the early diagnosis and management of dementia: Preliminary audit results from the first transdisciplinary service integrating family medicine and geriatric psychiatry services to the heart of patients' homes.

BACKGROUND: The number of dementia cases is expected to rise exponentially over the years in many parts of the world. Collaborative healthcare partnerships are envisaged as a solution to this problem. Primary care physicians form the vanguard of early detection of dementia and influence clinical care that these patients receive. However, evidence suggests that they will benefit from closer support from specialist services in dementia care. An interdisciplinary, collaborative memory clinic was established in 2012 as a collaborative effort between a large family medicine based service and a spec

Evidence for Greater Forensic Education of all Psychiatry Residents.

Booth and his colleagues have made an important contribution to the emerging evidence base that shows education in forensic psychiatry topics can improve attitudes toward the field. Given the desinstitutionalization of those with severe mental illness from state psychiatric facilities and the incarceration of many individuals with severe mental illness in correctional facilities, the need to train many more psychiatrists with competence in correctional settings is clear. Simply training more forensic psychiatrists will not both meet the psychiatric needs of incarcerated patients and fulfill th

Becoming Bilingual in Science: What My Mentors Saw Before I Did.

Geriatric psychiatry, in particular, demands cross-disciplinary fluency because aging is at once biological, psychological, social, and structural. Effective mentorship, therefore, cultivates a kind of "bilingualism" in science--the ability to move across disciplinary languages while maintaining intellectual depth and authenticity. It anchors early-career scientists in their home discipline while intentionally exposing them to adjacent and even distant fields. When this occurs early in training (e.g., during the postdoctoral or K-award stage), it fosters careers defined by intellectual flexibi

Dementia care in old age psychiatry over 50 years.

The global increase in number of people living with dementia and its mental health implications is closely linked to the development of Old Age Psychiatry (OAP) in the past 50 years. OAP focuses on addressing the complex psychiatric and physical needs of older adults, particularly those with dementia. Reflecting on the significance of early neuropsychiatric symptoms (NPS) in dementia, recent breakthroughs in early diagnostics and the amyloid targeting therapy (ATT) for Alzheimer's disease, it is time to examine the ongoing challenges and the role of OAP clinicians in managing dementia. The fut

The effect of serum lithium levels on renal function in geriatric outpatients: a retrospective longitudinal study.

BACKGROUND: Lithium remains an important treatment for bipolar disorder; however, whether elevated lithium levels lead to long-term renal problems is unknown. Previous consensus opinion was that levels should be kept below 0.6 mmol/L in geriatric patients to minimize renal toxicity. OBJECTIVE: We hypothesized that elevated serum lithium levels correlate with decreased renal function [estimated glomerular filtration rate (eGFR)] in geriatric psychiatry outpatients. STUDY DESIGN: This was a 4-year retrospective cohort study (2007-2011). SETTING: We performed this study in three Canadian unive

Anxiety among older psychiatric patients: a hidden comorbidity?

OBJECTIVES: Our aims were to explore prevalence of anxiety among patients admitted to departments of geriatric psychiatry for treatment of various diagnoses and to examine how often anxiety was registered as a previous or ongoing diagnosis. METHOD: In all, 473 patients admitted to one of five departments of geriatric psychiatry were included in a quality register and examined according to a standardized protocol. The Geriatric Anxiety Inventory (GAI) was used to measure anxiety during the first week after admission. Diagnoses were made at discharge. RESULTS: Using a cutoff on the GAI of 8/9,

Patterns of referring of patients with frontotemporal lobar degeneration to psychiatric in- and out-patient services. Results from a prospective multicentre study.

Dementia with frontotemporal lobar degeneration (FTLD) is clinically characterized by the occurrence of various psychiatric symptoms. In a recent study, the hospital-based prevalence of FTLD and the circumstances of the patients' admission to German psychiatric state hospitals were estimated. On the basis of further continuous assessment, this original FTLD group (n = 33) has been enlarged to 58 patients. The authors here present demographic and clinical data, and reasons for admission to geriatric psychiatry hospitals in comparison with 17 patients, who primarily attended the Memory Disorders

Geriatric referrals to a psychiatric consultation-liaison service.

Geriatric patients constitute a significant proportion of referrals to a psychiatric consultation-liaison service, accounting for 29% of referrals in the present study. Organic mental disorders, the most common psychiatric diagnosis, were identified in 51% of geriatric referrals. The next most common psychiatric conditions were affective disorders and adjustment disorders, each of which were diagnosed in 17% of geriatric referrals. Transfers to in-patient psychiatry were unusual and the most common recommendations were for psychotropic medications, further medical investigations, and for psych
