Screening for Geriatric Problems in Older Adults Receiving Hemodialysis Using a Self-Administered Battery.

PURPOSE: Older adults receiving hemodialysis experience geriatric problems that affect quality of life and contribute to high healthcare utilization but are often underrecognized in dialysis care. Scalable approaches to identifying these problems are needed. This study evaluated the agreement between a self-administered screening battery and a geriatrician telehealth evaluation and assessed the feasibility of implementing the screening battery in dialysis clinics. PATIENTS AND METHODS: Participants aged ≥55 receiving hemodialysis completed the Geriatric screen for Older adults receiving Dialy

The Crossroads of Geriatric Cardiology and Cardio-Oncology.

Cancer and cardiovascular disease (CVD) are two major causes of mortality in older adults. With improved survival and outcomes from cancer and CVD, the role of the geriatrician is evolving. Geriatricians provide key skills to facilitate patient-centered and value-based care in the growing older population of cancer patients (and survivors). Cancer treatment in older adults is particularly injurious with respect to complications stemming from cancer therapy and as well as to CVD related to cancer therapy in the context of physiologic aging. To best meet their natural potential as caregiving lea

Preferences for deprescribing antihypertensive medications amongst clinicians, carers and people living with dementia: a discrete choice experiment.

BACKGROUND: Optimal management of hypertension in people with dementia may involve deprescribing antihypertensives. Understanding differing treatment priorities is important to enable patient-centred care. This study explored preferences for antihypertensive deprescribing amongst people living with dementia, carers and clinicians. METHODS: Discrete choice experiments (DCEs) are a stated preference survey method, underpinned by economic theory. A DCE was conducted, and respondents completed 12 labelled choice-questions, each presenting a status quo (continuing antihypertensives) and antihypert

Women Gerontologists' Personal Experiences with Aging: Results From the Women in Gerontology Legacy Project.

The Women in Gerontology Legacy (WIGL) Project involved interviewing 46 older women gerontologists between 2014 and 2015. The current study focuses on the WIGL interview questions regarding the interactions between participants' experiences as gerontologists and their own aging. Emergent themes focus on the direction of influence: professional lives influencing personal lives, personal lives informing professional lives and mutuality between personal and professional lives. Thematic findings expand our current understanding of how professional knowledge and experiences contribute to personal a

Improving Clinical Frailty Scale accuracy on a vascular ward for a perioperative medicine for older people undergoing surgery pilot.

Geriatricians in Leicester piloted a perioperative medicine for older people undergoing surgery (POPS) service for patients with a lower limb vascular pathology and Clinical Frailty Scale (CFS) score of 5 or more. We aimed to improve CFS accuracy assigned by the vascular team through a structured teaching programme and use of a mobile application. Frailty scoring improved after each intervention, facilitating accurate identification of patients for referral to geriatricians. Education within host surgical teams is therefore crucial to POPS service development.

Geriatric co-management of proximal femur fractures: total quality management and protocol-driven care result in better outcomes for a frail patient population.

Hip fractures in older adults are a common event, leading to substantial morbidity and mortality. Hip fractures have been previously described as a "geriatric, rather than orthopedic disease." Patients with this condition have a high prevalence of comorbidity and a high risk of complications from surgery, and for this reason, geriatricians may be well suited to improve outcomes of care. Co-management of hip fracture patients by orthopedic surgeons and geriatricians has led to better outcomes in other countries but has rarely been described in the United States. This article describes a co-mana

Perspectives on Community Geriatrics: An Applied Proposal From Colombia, South America.

This essay argues for the urgent need to redefine the role of geriatricians, as medical speciality, beyond hospital settings, emphasizing the importance of community geriatrics (CG) in Latin America. Despite global discussions on integrating CG in geriatric practice, its presence remains marginal in low-income countries, where older adults face systemic barriers to health care access. The essay calls for a discussion of CG within geriatric training model, which often prioritizes hospital-based care, and advocates for a community-based approaches to aging. Using a geriatric program in Colombia

Lessons Learned From A Geriatric Referral Model in People With HIV.

People with HIV experience geriatric syndromes commonly found in much older populations and may benefit from geriatrician consultation. Multiple clinical models have emerged to address this need. We describe a model at the University of Colorado and the barriers to its robust implementation. The UCHealth Infectious Disease clinic implemented a referral-based model to connect high-risk people with HIV ≥50 years old with the UCHealth Seniors clinic. During geriatric consultation, patients were screened for functional status, preventative care, socioenvironmental factors, and cognitive/mental hea

Effect of specialty and recent experience on perioperative decision-making for abdominal aortic aneurysm repair.

OBJECTIVES: To determine whether recent experience and specialty choice would affect physician adherence to evidence-based guidelines. DESIGN: In a series of computer-simulated encounters, participants weighed the risk of spontaneous abdominal aortic aneurysm (AAA) rupture against the risk of perioperative death to determine timing for elective repair. Guideline recommendations and statistical information on the risks of rupture and surgical death were provided. SETTING: Annual meetings of the American Geriatrics Society, American College of Surgeons, and American Society of Anesthesiologist

Behavioral disorders in the elderly.

Based on the study of 40 patients in a unit of a large geriatric center, as well as on long overall experience, the problems of aged men and women with various emotional disorders are discussed. Case abstracts are presented to illustrate chronic organic brain syndrome, delirium, hallucinatory behavior, paranoid ideation, depression, and reactions triggered by physical illness. The geriatrician should be fully aware of all the interlocking factors in the patient's adjustment to his condition. Only in this way can the geriatrician and staff choose the best relevant therapy.

Telemedicine vs Face-to-Face for Nursing Home Residents With Acute Presentations: A Noninferiority Study.

OBJECTIVES: Telemedicine and face-to-face outreach services to nursing homes (NHs) have been used to reduce hospital utilization rates for acute presentations. However, how these modalities compare against each other is unclear. This article examines if the management of acute presentations in NHs with care involving telemedicine is noninferior to care delivered face-to-face. DESIGN: A noninferiority study was conducted on a prospective cohort. Face-to-face intervention involved on-site assessment by a geriatrician and aged care clinical nurse specialist (CNS). New glaucoma medications in the geriatric population: efficacy and safety.

Glaucoma can be considered a disease of the aging eye. Most medications used to treat glaucoma are in topical eyedrop form and may cause numerous untoward systemic effects in older persons. In recent years, several new ocular hypotensive medications have become available. These medications are being used more commonly because there is a growing trend by ophthalmologists to aggressively lower intraocular pressure. Therefore, geriatricians require a comprehensive knowledge of medications used to treat glaucoma, in addition to an understanding of their mechanism of action profiles of untoward eff

Orthogeriatric co-management for the care of older subjects with hip fracture: recommendations from an Italian intersociety consensus.

BACKGROUND: Health outcomes of older subjects with hip fracture (HF) may be negatively influenced by multiple comorbidities and frailty. An integrated multidisciplinary approach (i.e. the orthogeriatric model) is, therefore, highly recommended, but its implementation in clinical practice suffers from the lack of shared management protocols and poor awareness of the problem. The present consensus document has been implemented to address these issues. AIM: To develop evidence-based recommendations for the orthogeriatric co-management of older subjects with HF. METHODS: A 20-member Expert Task

National survey on aspiration pneumonia in elderly hospitalized or institutionalized patients in France in 2023.

BACKGROUND: Aspiration pneumonia is not specifically addressed in French guidelines on pneumonia, despite it being a frequent infection affecting specific populations. The French Infectious Diseases Society's (French acronym SPILF) Recommendations Group and the Infectio-Geriatrics Group (GinGER-SPILF SFGG) have published the first recommendations on this topic in 2025. The objective of this study was to describe practices of French physicians to manage aspiration pneumonia in hospitalized or institutionalized people aged over 75 years. METHODS: A nationwide survey conducted from June to Novem

Regional Geriatric Trauma Advice for Older Patients with Chronic Subdural Haematoma-A Novel Service for Multi-Disciplinary Decision-Making.

Background: Chronic subdural haematoma (cSDH) is one of the most common neurocranial conditions in older adults and is increasing in incidence. Conservatively managed patients are typically older, frailer, and more comorbid than surgical candidates, yet specialist geriatrician input into their management is rarely formalised. Potential for geographic inequity in hub-and-spoke neurosurgical networks within the UK compounds this challenge. We describe a service evaluation of a novel regional geriatric trauma advice service providing specialist trauma geriatrician input into cSDH management along
