Cutting and coagulation during intraoral soft tissue surgery using Er: YAG laser.

AIM: To find the optimal techniques and parameters that enables Er:YAG laser to be used successfully for small intraoral soft tissue interventions, in respect to its cutting and coagulation abilities. CASE REPORT: In vitro pre-tests: 4 different Er:YAG laser units and one CO2 unit as the control were used for incision and coagulation on porcine lower jaws and optimal parameters were established for each type of intervention and each laser unit: energy, frequency, type, pulse duration and distance. CASE SERIES: 3 different types of intervention using Er:YAG units are presented: crown lengthen

Esthetic Mucogingival Treatment After Biopsy of Inflammatory Fibroepithelial Hyperplasia.

INTRODUCTION: This case reports a biopsy of a large soft tissue defect with gingival grafting. The therapeutic approach presented here combines a partly de-epithelialized free gingival graft (PDFGG) with an envelope technique, which may be useful for cases needing a large reactive lesion biopsy. CASE PRESENTATION: A patient with an esthetic and functional complaint due to a fibroepithelial hyperplastic lesion was treated by excisional biopsy leaving a 10 mm cervical defect on tooth 23. A palatal PDFGG was harvested and sutured into the defect, using an "envelope technique" to improve the bloo

Periodontics and its influence on aesthetics.

The gingival tissues form the soft tissue frame of the dentition and play an important role in dental aesthetics in the maxillary anterior region of the mouth. Alterations to the form and symmetry of the gingivae through developmental, pathologic or iatrogenic factors can significantly alter the appearance and harmony of the natural dentition or prosthetic tooth replacement. This paper describes the techniques available for the management of gingival recession and for surgical lengthening of the clinical crown with particular emphasis on diagnosis and case selection.

Free gingival grafts. Current indications and techniques.

The free gingival graft is a simple, predictable technique for increasing the zone of attached gingiva. Dentists interested in performing minor surgical procedures should include the free gingival graft in their armamentarium. Indications and contraindications have been established, but the decision regarding the adequacy of the attached gingiva remains one of clinical judgment. The clinician must evaluate several variables, including the width of the attached gingiva, the patient's age, recession history, oral hygiene practices, teeth involved, and dental records. In the opinion of the author

Soft-tissue plastic surgery.

Soft-tissue plastic surgery was previously referred to as mucogingival surgery by Friedman (Tex Dent J 1957, 75:358-362). This term referred to any surgery "designated to preserve attached gingiva, to remove frena or muscle attachment, and to increase the depth of the vestibule." The aim of this type of surgery was to maintain an adequate mucogingival complex, focusing on the importance of the amount of attached gingiva. Soft tissue plastic surgery is important not only for reasons of health, but also for cosmetic purposes.

A contiguous mucosal graft.

The contiguous mucosal graft is designed primarily for use in vestibular deepening but may serve as an adjunctive technique in the management of other mucogingival defects. A large amount of donor material can be obtained by this procedure which is relatively simple to perform and generally unattended by surgical complications of hermorrhage and excessive pain. Its benefits are esthetic as well as functional since the replaced tissue reproduces the original both in appearance and histological details.

Outcome of Permanent Canine Teeth Following Extraction of Linguoverted Deciduous Mandibular Canine Teeth in 28 Dogs.

Linguoverted mandibular canine teeth (LMC) are a common cause of class 1 malocclusion, which may result in a traumatic occlusion. When deciduous LMC are diagnosed, extraction is commonly performed to alleviate traumatic occlusion. This retrospective study was designed to observe the outcome of the permanent dentition in dogs with a class 1 malocclusion receiving treatment for deciduous LMC. Medical records from a veterinary dental referral hospital were used to identify patients that underwent extraction of deciduous LMC's prior to the eruption of the permanent mandibular canine teeth. The per

Direct vs indirect restorations for diastema closure: determining the suitable approach.

Determining the appropriate technique for diastema closure is challenging, and the decision must be evidence based. The objective of these case reports is to describe different approaches to diastema closure using direct and indirect techniques, focusing on the characteristics of the patient and clinical requirements to guide treatment. In the first case, a 16-year-old patient had multiple diastemas in the maxillary anterior dentition. The clinical evaluation revealed microdontia of the lateral incisors and malpositioned teeth. The treatment included tooth whitening and placement of composite

Multidisciplinary approach to restore esthetics and function in a young patient with three consecutive impacted teeth: a case report with 18-month follow-up.

This case report describes the orthodontic treatment of an 11-year-old patient with three maxillary impacted teeth on the right side. Cone-beam computed tomography showed that these teeth were close together, with the lateral incisor in a lower position, followed by the central incisor, and the canine in a more apical position. Treatment included applying traction to these teeth. A transpalatal arch was used as an anchorage device, and surgical exposure of the lateral incisor was performed for traction with an elastic chain toward the hook welded to the 0.017 × 0.025-inch steel segmented arch.

Aesthetic Rehabilitation in Teeth with Wear from Bruxism and Acid Erosion.

BACKGROUND: Bruxism is defined as a repetitive activity of grinding the teeth through lateral or protrusive movements of the mandible, and it is considered the most complex and destructive functional disorder. In addition, erosion caused by diet or reflux can damage the remaining teeth. OBJECTIVE: In this report, a patient with bruxism and dental erosion was treated with a multidisciplinary approach to restore the function and aesthetic. METHODS: This clinical report describes the management of an adult woman, 33 years old, who was dissatisfied with the aesthetics of her smile and complained

Miniscrew-assisted single-tooth distraction osteogenesis to align an ankylosed infraoccluded maxillary central incisor: A case report.

The ankylosis of permanent incisors is usually caused by trauma. In a growing patient, the ankylosed tooth fails to move along with the vertical growth of the remaining alveolar process, which results in an infraoccluded tooth, gingival margin disharmony and unaesthetic smile. This case report presents an 23-year-old female patient whose maxillary right central incisor (tooth number 11) had been traumatised eight years earlier. A vertical discrepancy of about 4 mm was exhibited between teeth 11 and 12. To reposition the crown and gingival margins of the ankylosed tooth to an ideal level, single

Mucogingival surgery techniques: Coronally advanced flap versus tunnel approach.

BACKGROUND: Mucogingival surgery techniques for treating gingival recessions (GRs) have evolved significantly over decades, reflecting advances in surgical understanding and patient demands. This narrative review tracesexamines the historical development of the coronally advanced flap-multiple coronally advanced flap (CAF/MCAF) and tunnel techniques, highlighting how these procedures have been refined to improve clinical outcomes and esthetic results. Emphasis is placed on the biological principles underlying these approaches and their adaptation based on growing scientific evidence. METHODS:
