Current status of artificial hearts and ventricular assist devices.

Currently available approaches for patients with refractory heart failure include intravenous inotropic therapy, partial left ventriculectomy, dynamic cardiomyoplasty, mechanical circulatory support, and heart transplantation. Heart transplantation is limited by the availability of donor organs, which is never expected to meet the increasing demand. Numerous devices have been developed for circulatory support. Currently available devices differ in configuration, anatomic location, flow characteristics, and durability. Many devices are currently available for mechanical support, and they can ge

Ventricular assist device as a bridge to heart transplantation in adults.

BACKGROUND: Because of the shortage of donor hearts, various ventricular asist devices (VAD) have been used in decompensated patients to prolong patient survival until a suitable heart becomes available. In this paper, we present our single-center report of adult patients in whom bridging was used with VAD. METHODS: We performed a retrospective review of 14 adult patients who underwent heart transplantation after insertion of a long-term VAD. The study spans from February 2006 until September 2010. The mean patient age was 44.28 ± 11.06 years. We used the Berlin Heart EXCOR VADs (n = 11; Berl

Ventricular assist device as a bridge to heart transplantation in children.

The ventricular assist device (VAD) is a life-saving option for patients in heart failure refractory for conventional therapy. The aim of study was to assess the influence of VAD on heart transplantation (HT) outcome in children <16 years. Between October 1988 and August 2008, 73 children underwent HT: Group 1 (n=9) who received VAD as bridge to HT (left ventricular - 4, biventricular - 5), and Group 2 (n=64), without previous VAD. Diagnoses included cardiomyopathy (n=50 (68.5%)) and congenital heart defects (n=23 (31.5%)). Retrospective analysis of perioperative and long-term follow-up data w

Clinical application of the ellipsoid left heart assist device.

The ellipsoid left heart assist device (E-LVAD) was implanted in eight patients suffering from intraoperative heart failure. It was not possible to remove these patients from extracorporeal circulation following an intracardiac procedure; therefore, implantation of the E-LVAD was performed during extracorporeal circulation. The inflow connector was pushed forward from a purse-string suture on the right superior pulmonary vein, across the mitral valve and into the left ventricle. The outflow connector was joined to the ascending aorta. In two patients, the artificial heart chamber was removed a

Aortic insufficiency and right heart failure in patients treated with durable left ventricular assist devices after preoperative support with microaxial flow devices.

PURPOSE: Temporary mechanical circulatory support (tMCS) with microaxial flow pumps (mAFP) reverses the shock state and improves hemodynamics prior to durable left ventricular assist device (dLVAD) implant. Aortic insufficiency (AI) and right heart failure (RHF) are recognized sourced of late morbidity and mortality during dLVAD support. We examined the impact of tMCS with mAFP on AI and RHF progression given their transaortic deployment and unique interaction with the aortic valve and right ventricle. METHODS: The Society of Thoracic Surgeons Intermacs Database was queried for all fully magn

Impact of ICD Presence in Patients With Left Ventricular Assist Devices for End-Stage Heart Failure.

BACKGROUND: The prognostic relevance of implantable cardioverter defibrillator (ICD) in patients with end-stage heart failure (HF) with left ventricular assist device (LVAD) remains controversial. OBJECTIVES: To evaluate the prognostic impact of ongoing use of ICD and associated complications in HF-patients with LVAD-implantation. METHODS: We retrospectively analyzed all consecutive patients (n = 351) who underwent LVAD-implantation and follow up between 2009 and 2021. Patients were categorized according to the presence (n = 254) or absence (n = 97) of ICD. To reduce baseline imbalances betw

Vascular function in patients with advanced heart failure and continuous-flow or pulsatile ventricular assist devices.

BACKGROUND: A significant proportion of patients with heart failure (HF) progress to an advanced stage, which is associated with a substantial increase in morbidity and mortality. These patients may be eligible for advanced treatment strategies such as mechanical circulatory support with ventricular assist devices (VAD). Vascular dysfunction is a hallmark of heart failure pathophysiology and prognosis. However, whether and to what degree the hemodynamic benefits of VADs influence vascular function remain unknown. METHODS AND RESULTS: In this study, we evaluated endothelial vascular function w

Waitlist and transplant outcomes in heart transplant candidates bridged with temporary endovascular right ventricular assist devices.

BACKGROUND: Advances in mechanical circulatory support and changes in allocation policy have shifted waitlisting practices for heart transplantation (HT) in the United States. This analysis reports waitlist and transplant outcomes among HT candidates bridged with temporary endovascular right ventricular assist devices (tRVADs). METHODS: Patients awaiting HT from 2008 to 2022 in the United Network of Organ Sharing registry were grouped by the presence of tRVAD while waitlisted and propensity matched. Waitlist outcomes were HT and a competing outcome of death/deterioration requiring waitlist in

Impact of durable ventricular assist devices on post-transplant outcomes in adults with congenital heart disease.

BACKGROUND: There are no published data on post-transplant outcomes in durable ventricular assist device (VAD)-supported adult congenital heart disease (ACHD) patients. METHODS: We compared post-transplant outcomes in VAD-supported vs non-VAD-supported ACHD patients using the Scientific Registry of Transplant Recipients. RESULTS: At 1 year, there was no difference in post-transplant mortality between VAD-supported (12 patients) and non-VAD-supported (671 patients) ACHD patients. CONCLUSIONS: In appropriate ACHD patients, VAD use as a bridge to transplant is a reasonable strategy.

Use of Heart Failure Medical Therapies Among Patients With Left Ventricular Assist Devices: Insights From INTERMACS.

BACKGROUND: Use of left ventricular assist devices (LVADs) for treatment of advanced heart failure has expanded significantly over the past decade. However, concomitant use of heart failure medical therapies after implant is poorly characterized. METHODS AND RESULTS: We examined the use of heart failure medications before and after LVAD implant in adult patients enrolled in the Interagency Registry for Mechanically Assisted Circulatory Support (INTERMACS) between 2008 and 2013 (N = 9359). Using logistic regression, we examined relationships between patient characteristics and medication use a

Can long-term ventricular assist devices be safely implanted in low-volume, non-heart transplant centres?

BACKGROUND: Mechanical circulatory support (MCS) using long-term ventricular assist devices (VADs) is an established therapy in select patients with advanced heart failure. Studies have suggested that outcomes after VAD implantation may be dependent on institutional procedural volume, and outcome data from non-transplant centres are lacking. This study reviews the outcomes of patients who received a long-term VAD at our centre to determine if these devices can be safely implanted at tertiary care, low-volume, non-transplant centres. METHODS: We conducted a single-centre retrospective cohort s

Quality of life outcomes after heart transplantation in individuals bridged to transplant with ventricular assist devices.

BACKGROUND: Increasing numbers of individuals receive ventricular assist devices (VADs) as bridges to heart transplantation. Physical morbidity risks and benefits, and quality of life (QOL) during VAD support have been documented. Effects of pre-transplant VAD support on functional and QOL outcomes after transplantation have received no empirical attention. METHODS: Sixty-three VAD patients who received heart transplants underwent QOL evaluations of physical functioning, emotional and cognitive well-being, and social functioning at 2, 7, and 12 months after transplant (response rate = 95%). Heart transplantation in patients with ventricular assist devices: Impacts of the implantation technique and support duration.

BACKGROUND: Orthotopic heart transplantation (HTx) is the gold standard treatment for patients with terminal heart failure. As donor organs are limited, patients are often on ventricular assist device (VAD) support before receiving HTx. We aimed to compare the outcome after HTx in patients with and without preoperative VADs as well as in patients who underwent different VAD implantation techniques. METHODS: A total of 126 patients underwent HTx at our department between 2010 and 2019 and were retrospectively analyzed. While 47 patients underwent primary transplantation (No VAD), 79 were on VA

Creation of a self-made total artificial heart using combined components of available ventricular assist devices.

We describe the case of a 39-year-old woman who received a self-made total artificial heart built of components from the Thoratec and ExCor Berlin Heart systems. The patient had a severe aortic/myocardial infection following replacement of the ascending aorta with a Shelhigh conduit due to type A aortic dissection. The surgical technique is described in detail in this article. This technique is feasible if a total artificial heart is not available and implantation of a biventricular assist device is not possible.

Preoperative predictors of survival in patients with Thoratec ventricular assist devices as a bridge to heart transplantation. Thoratec Ventricular Assist Device Principal Investigators.

Approximately two-thirds of patients implanted with ventricular assist devices recover sufficiently to requalify for heart transplantation, and the other one-third die of complications that are often secondary to delayed ventricular assist device implantation and subsequent end-organ failure. To determine whether any preoperative predictors of survival exist, univariate statistics and multivariate stepwise logistic regression analysis were performed on pre-ventricular assist device demographics, hemodynamics, and blood chemistry in 186 patients receiving Thoratec ventricular assist devices (Th

Berlin Heart ventricular assist device in a child with hypoplastic left heart syndrome.

We report the implantation of a Berlin Heart ventricular assist device (VAD) in a 4-year-old boy with hypoplastic left heart syndrome previously palliated with Norwood and Glenn operations, who presented with progressive ventricular failure and hypoxemia. Insertion of a 30-mL pneumatic pediatric pump with cannulation of the systemic right ventricle and aorta had a salutary effect on cardiac output, improving oxygen saturations. While awaiting heart transplantation, multiple thromboembolic complications developed and he died, despite therapeutic heparinization and aspirin therapy. Important les
