Is the place of birth related to the mother's satisfaction with childbirth? A cross-sectional study in a rural district of the Lao People's Democratic Republic (Lao PDR).

BACKGROUND: The place of birth has been rapidly changing from home to health facility in Lao People's Democratic Republic (Lao PDR) following the strategy to improve the maternal and neonatal mortality. This change in the place of birth might affect the mother's satisfaction with childbirth. The objective of this study was to assess whether the place of birth is related to the mother's satisfaction with childbirth in a rural district of the Lao PDR. METHODS: A community-based survey was implemented in 21 randomly selected hamlets in Xepon district, Savannakhet province, between February and M

Case Report of Vaginal Breech Birth.

In this case report, we describe a successful unplanned vaginal breech birth (VBB) for a primigravid woman who presented to the hospital in labor. This woman transferred to our hospital from an attempted home birth and was highly motivated to achieve a vaginal birth. The staff were recently trained on the provision of physiologic breech birth support, and after receiving informed consent, they facilitated a successful VBB. Planned VBB services are largely unavailable in the United States. Offering women the option to undergo VBB involves screening for good candidates, obtaining informed consen

Women's views of the place of confinement.

BACKGROUND: The proportion of births taking place in the home of the mother has declined to less than 1% of the total, in spite of the lack of evidence of the benefits of hospitalization. Home confinement remains rare when supported by general practitioners; little is known of the preferences of women of childbearing age. AIMS: To examine the beliefs of women of childbearing age concerning the advantages and disadvantages of giving birth in hospital or their own homes, the choices of place of birth open to them, and the information required to enable an informed choice. METHOD: A self-admini

Early breastfeeding practices: Descriptive analysis of recent Demographic and Health Surveys.

The aim of this study was to describe early breastfeeding practices (initiation within 1 hr of birth, no prelacteal feeding, and a combination of both-"optimal" early breastfeeding) according to childbirth location in low- and middle-income countries. Using data from the most recent Demographic and Health Survey (2000-2013) for 57 countries, we extracted information on the most recent birth for women aged 15-49 with a live birth in the preceding 24 months. Childbirth setting was self-reported by location (home or facility) and subtype (home delivery with or without a skilled birth attendant; p

[Birth in Out-of-Hospital Settings--Differences in Maternal and Neonatal Outcome of Women with their Second Child and a Prior Caesarean Section Compared to First Paras].

BACKGROUND: Vaginal birth after Caesarean (VBAC) in out-of-hospital settings is controversial. With increasing Caesarean rates, more women with a prior Caesarean will decide to give birth in midwife-led birth-centres or at home. Therefore the study explores the question about maternal and neonatal outcomes in German out-of-hospital settings. METHOD: A retrospective study of German out-of-hospital data from 2005 to 2011 was undertaken. Included were 66,437 singleton pregnancies in cephalic presentation at term. This study describes the outcome parameters of first paras compared to mothers with

Decision Making about Hospital Arrival among Low-Risk Nulliparous Women after Spontaneous Labor Onset at Home.

INTRODUCTION: Postponing hospital admission until the active phase of labor is a recommended strategy to safely reduce the incidence of primary cesarean births. Success of this strategy depends on women's decisions about when to transfer from home to the hospital, a process that is largely absent from research about childbirth. This study aimed to determine the decision-making criteria used by women about when to go to the hospital after the self-identification of labor onset at home. METHODS: A qualitative study was conducted at an academic medical center with a sample of 21 nulliparous wome

Advance distribution of misoprostol for the prevention of postpartum hemorrhage in South Sudan.

OBJECTIVE: To determine if high uterotonic coverage can be achieved in South Sudan through a facility- and community-focused postpartum hemorrhage (PPH) prevention program. METHODS: The program was implemented from October 2012 to March 2013. At health facilities, active management of the third stage of labor (AMTSL) was emphasized. During prenatal care and home visits, misoprostol was distributed to pregnant women at approximately 32 weeks of pregnancy for the prevention of PPH at home births. Data on uterotonic coverage and other program outcomes were collected through facility registers, h

Perinatal outcomes of low-risk planned home and hospital births under midwife-led care in Japan.

AIM: It has not been extensively studied whether planned home and planned hospital births under primary midwife-led care increase risk of adverse events among low-risk women in Japan. METHODS: A retrospective cohort study was performed to compare perinatal outcome between 291 women who were given primary midwife-led care during labor and 217 women who were given standard obstetric shared care. Among 291 women with primary midwife-led care, 168 and 123 chose home deliver and hospital delivery, respectively. Perinatal outcomes included length of labor of 24 h or more, augmentation of labor pain

Deliveries among adolescent mothers in rural Bangladesh: who provides assistance?

OBJECTIVES: This paper sought to identify factors associated with modes of delivery assistance among adolescent mothers in rural Bangladesh. METHODOLOGY: Bangladesh Demographic and Health Survey of 2004 data for the last 5 years (N = 867) were used. Univariate statistical analysis and multivariate logistic regression methods were employed in analyzing the data. RESULTS: We observed that almost all adolescent deliveries (93.6 %) took place at home, and most (80.1%) were assisted by untrained traditional birth attendants, relatives or neighbours. Only 8.8% were attended by medically trained pe

Home-like versus conventional institutional settings for birth.

BACKGROUND: Many home-like birth centres have been established near conventional labour wards for the care of pregnant women who prefer and require little or no medical intervention during labour and birth. OBJECTIVES: The objective of this review was to assess the effects of care in a home-like birth environment compared to care in a conventional labour ward, on labour and birth outcomes. SEARCH STRATEGY: The Cochrane Pregnancy and Childbirth Group trials register was searched. SELECTION CRITERIA: Randomised and quasi-randomised trials comparing a home-like institutional birth environment

Women's experiences of unexpected situations during childbirth: A qualitative study.

BACKGROUND: An unexpected situation during childbirth is a significant risk for a negative or traumatic birth experience. Existing knowledge of such situations is fragmented, and research has tended to focus on medical problems, interventions, or women's expectations rather than on their overall experience. OBJECTIVE: To describe women's experiences of unexpected situations during childbirth. METHODS: This qualitative descriptive study used purposive sampling to recruit participants by posting an invitation on two social media sites popular among mothers of newborns. Eighteen women who had g
