Human poisoning with pentachlorophenol and its treatment.

A case of intentional intoxication with pentachlorophenol has been described. Salient features observed included pyrexia, diaphoresis, hyperkinesis, muscle twitching, tremors, epigastric tenderness, leg pain, tachypnea, and tachycardia. The patient's restlessness and agitation were controlled with phenytoin and phenobarbital. Forced diuresis with furosemide and mannitol resulted in a large increase in urinary excretion of pentachlorophenol. It is suggested that such therapy may be life saving in such intoxications.

[Characteristics of the Tourette syndrome-like disorders in various mental disorders].

Fifty-four children with Gilles de la Tourette's syndrome were examined from 1987 to 1989. In 11 of them, Gilles de la Tourette-like disorders occurred within the framework of schizophrenia and in 6, of encephalopathy. The characteristic features of hyperkinesis associated with those conditions and their differences from the routine syndrome are depicted. The typology of Gilles de la Tourette's syndrome reflecting its clinical pleomorphism is suggested.

Sleep apnea in amyloidosis.

Obstructive sleep apnea (OSA) occurs in patients with narrowing of the pharyngeal airway due to a variety of pathologic conditions. We documented obstructive and mixed sleep apnea in a patient with narrowing of the airway caused by macroglossia associated with amyloidosis. Tracheostomy relieved the symptoms of OSA (hypersomnolence, confusion upon awakening, and hyperkinesis during sleep) despite the emergence of striking central sleep apnea following the procedure.

Fragile (X) syndrome: a study of the psychological profile in 23 prepubertal patients.

In this study a further analysis of the psychological profile in the prepubertal fragile (X) (fra(X] male was performed. The results of the psycho-diagnostic examination of 23 fra(X) boys were compared to a control group of 17 males of the same age with 'non-specific' mental retardation. A number of important quantitative and qualitative differences were observed between the two groups. In the preschool age group the majority of fra(X) boys was mildly mentally retarded. In the school-age group, however, most boys were moderately to severely mentally retarded. This indication of a decline in in

Takotsubo cardiomyopathy associated with bronchoscopic operation: A case report.

BACKGROUND: Takotsubo cardiomyopathy (TTC), a syndrome of acute left ventricular (LV) dysfunction, is characterized by transitory hypokinesis of LV apices with compensatory hyperkinesis of the LV basal region. The symptoms of TTC mimic acute myocardial infarction, without significant coronary stenoses on coronary angiography. Echocardiogram plays a key role in the diagnosis and prognosis of TTC. New indicators from echocardiograms may be helpful in disease evaluation. CASE SUMMARY: A 67-year-old man with a 10-year history of non-small cell lung cancer was admitted to our hospital for emerging

[Progressive supranuclear paralysis with levodopa-induced dyskinesia].

Progressive supranuclear palsy (PNP) is a neurodegenerative disease characterized by a combination of progressive akinetic-rigid syndrome, postural instability with frequent falls, supranuclear ophthalmoplegia, pseudobulbar syndrome and frontal dementia. The disease usually develops after the sixth decade of life, and has a progressive course. An own description of the clinical case of progressive supranuclear palsy in a 79-year-old patient with oromandibular hyperkinesis while taking levodopa is presented.

Measurement of regional wall motion from biplane contrast ventriculograms: a comparison of the 30 degree right anterior oblique and 60 degree left anterior oblique projections in patients with acute myocardial infarction.

The value of performing biplane vs single plane analysis of regional wall motion from contrast ventriculograms was determined in 102 patients who received thrombolytic therapy and who underwent biplane ventriculography during acute myocardial infarction (n = 67), at follow-up more than 2 weeks later (n = 80), or both (n = 45). Hypokinesis in the infarct region and hyperkinesis in the noninfarct region were measured by the centerline method in the respective artery territories, which were defined from the data of 62 patients with single-vessel disease and were expressed in units of standard dev

Surgical and conservative methods for restoring impaired motor function - facial nerve, spinal accessory nerve, hypoglossal nerve (not including vagal nerve or swallowing).

The present review gives a survey of rehabilitative measures for disorders of the motor function of the mimetic muscles (facial nerve), and muscles innervated by the spinal accessory and hypoglossal nerves. The dysfunction can present either as paralysis or hyperkinesis (hyperkinesia). Conservative and surgical treatment options aimed at restoring normal motor function and correcting the movement disorders are described. Static reanimation techniques are not dealt with. The final section describes the use of botulinum toxin in the therapy of dysphagia.

Attention deficit.

Attention deficit, among the most commonly diagnosed functional deficits in pediatric neurology, is, like epilepsy, most often idiopathic. It can also be a symptom of many neuropathologic states. Although a lifelong problem, attention deficit is most troublesome during school years, because, like asthma, it is highly sensitive to environmental influence. The neurologist can consider attention deficit in its own right, apart from hyperkinesis and other associated behavior disorders, as a cognitive limitation and handicap to academic progress.

Hemichorea-hemiballism and anosognosia following a contralateral infarction of the caudate nucleus and anterior limb of the internal capsule.

The authors describe a patient who experienced the sudden occurrence of hemicorea-hemiballism on the left side of the body and the contemporary agnosia of it. An unenhanced CT-scan disclosed an ischemic lesion involving the head of the caudate nucleus and of the anterior limb of the internal capsule of the contralateral hemisphere. The authors discuss the possible pathophysiologic mechanisms explaining the rare association of these two symptoms and suggest that the involvement of the pyramidal tract in the internal capsule usually does not allow hyperkinesis to manifest itself.

Atypical variant stress (Takotsubo) cardiomyopathy associated with gastrointestinal illness: rapid normalisation of LV function.

A 34-year-old female with a diarrhoeal illness and palpitations was found to have an abnormal ECG and troponin T. Subsequent coronary angiography identified angiographically normal epicardial coronary arteries with moderate impairment of left ventricular systolic function due to mid-ventricular akinesis with apical hyperkinesis. Cardiac MRI, performed 1 week later, demonstrated complete resolution of ventricular dysfunction and a diagnosis of atypical variant stress cardiomyopathy, due to gastrointestinal illness, was proposed.

Botulinum toxin in the management of facial paralysis.

PURPOSE OF REVIEW: Complete flaccid facial paralysis, as well as the synkinetic and hyperkinetic sequelae of partial recovery, has significant impact on quality of life. Patients suffer from functional deficiencies, cosmetic deformity, discomfort and social consequences leading to emotional distress. Despite an extensive and sophisticated array of available interventions for facial reanimation, most patients have persistent issues that require consistent follow-up. In long-term management, botulinum toxin (BT) injection remains a critical tool in the treatment of the facial paralysis patient,

[Case-report of neuroacanthocytosis associated with a compound mutation in the VPS13A gene].

Neuroacanthocytosis is a group of neurodegenerative diseases manifested by a combition of neurological symptoms (most often choreic hyperkinesis) and the presence of an increased number of acanthocytes (erythrocytes with horns) in the peripheral blood. This group includes chorea-acanthocytosis, MacLeod's syndrome, pantothenate kinase-associated neurodegeneration, Huntington-like disease type 2, and some other very rare diseases. This article presents a genetically confirmed clinical case of chorea-acanthocytosis associated with a compound mutation in the VPS13A gene, discusses in detail the st

A new variant of Tako-tsubo cardiomyopathy: transient mid-ventricular ballooning.

Stress cardiomyopathy is a reversible left ventricular dysfunction precipitated by emotional stress. Affected patients are generally women, whose symptoms are similar to myocardial infarction with reversible apical dyskinesis associated with hypercontractile basal segments and no evidence for hemodynamically significant coronary arterial stenoses by angiography. We report the case of an 82-year-old woman who presented with acute onset of chest pain after emotional stress and with reversible left ventricular dysfunction consisting of akinesis of the midventricular segments and hyperkinesis of t

[Physiological manifestations of action of a gene regulating predisposition to a pendulum-like movements in rodents].

Physiological effects of the gene determining predisposition to a stereotypical hyperkinesis in the form of pedulum movements (PM) are manifested in some behavioral peculiarities (total motor activity, emotionality, intensity of the startle reflex, sensitivity of the 5-HT2 receptors, predisposition to catalepsy). High frequency of the PM occurring in Wistar rat stock suggests that the gene determining the PM expression is not a pathological one but controls some adaptive properties of the nervous system. Relationships between the PM expression and the degree of predisposition to catalepsy are

[Haloperidol, reserpine, l-dopa and amantidine in the treatment of Huntington chorea (author's transl)].

Two drugs that should reduce striatal dopamine activity (haloperidol and reserpine) and two other drugs that should increase it (L-Dopa and amantadine) have been successively adminstered to six patients suffering from Huntington's Chorea. The most effective treatment in reducing hyperkinesis was haloperidol, followed by reserpine. Treatment with L-Dopa did not have appreciable effect, on this type of involuntary movements. Against some theoretical expectations administration of amantadine was followed by a slight improvement in the control of involuntary activity.

Effect of Vesicular Monoamine Transporter Type-2 Inhibitors on Motor Features in HD.

BACKGROUND: Huntington's disease (HD) is an inherited neurodegenerative disorder primarily associated with motor and psychiatric manifestations. No disease-modifying treatment currently exists, with vesicular monoamine transporter type-2 inhibitors (VMAT2i's) being the only FDA-approved therapy for motor features in HD. Previous studies evaluated the antichorea effects of VMAT2i's but did not address its effects on non-choreiform features. OBJECTIVES: This study aimed to characterize the effect of VMAT2i's on hyperkinetic, hypokinetic and total hypokinetic subscores of the UHDRS motor assessm

Hyperactivity with tactile defensiveness as a phobia.

Conceptualizing some cases of hyperkinesis (those with tactile defensiveness) as tactile phobias provides the rationale for two unconventional management approaches modeled on the two most popular behavioral treatments of phobia: systematic desensitization and implosion. Both methods are adapted to a nonverbal level, befitting the age of the afflicted children and the nature of the problem. The first method uses the sensory integration techniques developed by Ayres to desensitize the child to touch systematically and gradually. The other method, developed by Henderson and Dahlin, uses restrain

[Pathogenesis of obsessional states against a psychopathic background].

The paper is concerned with a clinical study of 37 psychopaths with obsessional symptoms: 24 anancasts and 13 psychasthenics. In 14 patients the obsessional ideas were combined with obsessional actions, in 28 cases there was a defect of integrative automatized motions, in 6 cases--tics, in 2--writing spasms and in 1 case--a spastic torticollis without symptoms of an organic lesion of the nervous system. A hyperkinesis of the tic-type, professional dyskinesia and possibly obsessional actions may be explained by a functional dissociation of the neocrotex with subcortex and limbico-reticular comp
