[CPAP as a method of primary respiratory support in premature infants with acute respiratory distress syndrome].

Recent years neonatologists attempt to support the premature infants breathing without the intubation and mechanical ventilation use. Resent studies and new modern equipment for noninvasive ventilation resume the interest in CPAP The article presents the review of the recent studies which investigated CPAP effectiveness as a method of primary respiratory support in premature infants with acute respiratory distress syndrome. Big attention was paid to the surfactant use and InSurE approach in the modern treatment of respiratory disorders according the latest data of evidence based medicine.

A Pilot Randomized Control Trial of Holding During Hypothermia and Effects on Maternal and Infant Salivary Cortisol Levels.

BACKGROUND: The lack of physical contact during therapeutic hypothermia (TH) is challenging for parents of newborns with hypoxic ischemic encephalopathy. Holding is often avoided due to concerns for effects on infant temperature and for dislodging equipment. PURPOSE: We assessed the effect of holding during TH on maternal and infant salivary cortisol levels and on infant vital signs. METHODS: Prospective crossover study with infants randomized to a 30-minute session of holding on day-2 versus day-3 of TH. "No-holding" occurred on the alternate day at the same time. Pre- and post-holding sali

A Call to Action: Addressing Socioeconomic Disparities in Childhood Unintentional Injury Risk.

Maya arrives at the clinic after a long bus ride with her infant and toddler. When the pediatrician asks about childcare, Maya explains that her aunt babysits while Maya works. Maya does not own a car, but her aunt sometimes drives the children in her car, which is not equipped with car seats. Maya is unable to afford them and is reluctant to ask her aunt to purchase them, fearful of seeming unappreciative. The pediatrician offers Maya an infant car seat and a booster seat from the clinic’s supply, but Maya pictures herself, hands full with an inquisitive toddler and a fussy infant on the bus,

A multicenter evaluation of how many infants with permanent hearing loss pass a two-stage otoacoustic emissions/automated auditory brainstem response newborn hearing screening protocol.

OBJECTIVE: Ninety percent of all newborns in the United States are now screened for hearing loss before they leave the hospital. Many hospitals use a 2-stage protocol for newborn hearing screening in which all infants are screened first with otoacoustic emissions (OAE). No additional testing is done with infants who pass the OAE, but infants who fail the OAE next are screened with automated auditory brainstem response (A-ABR). Infants who fail the A-ABR screening are referred for diagnostic testing to determine whether they have permanent hearing loss (PHL). Those who pass the A-ABR are consid

[Prevention, care and screening of hips of newborn infants].

The frequency of C.D.H. and the indications for surgical treatment have been considerably reduced. Since a 3-part prevention protocol has been initiated: screening of newborns, prophylactic triple-diapering in slight flexion-abduction, aided by the availability of the equipment, and repeated examinations of infants.

Laryngoscopic endotracheal intubation of rats for inhalation anesthesia.

Endotracheal intubation of the rat under direct vision is described together with the details of procedures and apparatus for conducting inhalation anesthesia in this species. Our intubation method requires no special manufacture of equipment, because it employs the human laryngoscope equipped with an infant blade (size 0). Using inhalation anesthetics such as enflurane or halothane for induction, clear laryngoscopic visualization of the glottis is reliably obtained, allowing rapid and routine intubation of the rat in a highly predictable amount of time. In contrast, the injected anesthetics s

Tape: an alternative to stirrups or wedges in positioning for infant cystoscopy.

Performing a cystourethroscopy in children can range from reviewing a 100 kg teenage male with posterior urethritis, to assessing a 3 kg neonate with posterior urethral valves. Across this range the patient needs to be positioned in a way that allows passage of the rigid cystoscope and visualisation of the urethra and bladder. We describe a technique for positioning the legs of infants for cystoscopy using skin friendly tape, rather than wedges, stirrups or boots. The advantage of this being that it is cheap, adaptable and holds the infant in the perfect position, without needing complex equip

The breastfeeding dyad: early assessment, documentation, and intervention.

The early postpartum period is a critical time for the establishment of lactation. Although newborns are well equipped with reflexes that enhance breastfeeding, many aspects of the experience can be assisted by the knowledge and skills of clinicians. To prevent negative outcomes for mothers and infants, it is essential that the clinician make an accurate assessment of breastfeeding technique and milk intake by the infant. An understanding of the mechanics of milk transfer serves as a foundation for developing interventions to be applied to the common breastfeeding challenges (including difficu

Interference of Residual 99m Tc-MIBI Activity in Syringe Needle on 99m Tc-DMSA Scan Interpretation; Medical Error.

A 4-month-old infant with a small right kidney underwent a 99m Tc-DMSA scan showing unjustifiable findings including global cortical loss in the left kidney, increased liver and bowel uptake. Retrospective review revealed reuse of a sterile needle contaminated with residual 99m Tc-MIBI, compromising scan quality and falsely elevating right kidney function. A repeat scan showed normal left kidney uptake and significantly reduced right kidney function. This case highlights the importance of employing safe techniques with disposable equipment, thorough workflow review, and transparent error repor

Bacillus cereus Sepsis in Preterm Neonates Caused by Central Venous Catheter: A Case Report.

In recent years, Bacillus cereus infection has emerged as a main concern in the field of children's public health. This bacterium, known to be a pollutant, can be found in various settings such as hospital wards, equipment, breast milk, nutrient solution, and so on. With its high pathogenicity and toxicity, Bacillus cereus infection can lead to severe and life-threatening symptoms, particularly in premature infants. This case report documents the death of a preterm infant due to Bacillus cereus sepsis, septic shock, meningitis, and pneumonia, all of which were linked to the use of a central ve

Maternal holding during therapeutic hypothermia for infants with neonatal encephalopathy is feasible.

AIM: Concerns for infant destabilisation often prohibit parental holding of infants during therapeutic hypothermia (TH). We assessed the feasibility of maternal holding during TH, as the inability to hold can impede bonding. METHODS: Vital signs were assessed in stable infants before, at two-minute intervals during and 30 minutes after a single 30-minute holding session. The infant remained on the blanket throughout holding, and both infant and blanket were placed into the mother's arms on top of a thin foam insulating barrier. Mothers and nurses were surveyed about their experience. RESULTS

Assessment of infant cry: acoustic cry analysis and parental perception.

Infant crying signals distress to potential caretakers who can alleviate the aversive conditions that gave rise to the cry. The cry signal results from coordination among several brain regions that control respiration and vocal cord vibration from which the cry sounds are produced. Previous work has shown a relationship between acoustic characteristics of the cry and diagnoses related to neurological damage, SIDS, prematurity, medical conditions, and substance exposure during pregnancy. Thus, assessment of infant cry provides a window into the neurological and medical status of the infant. Ass

Enteral nutrition support of the preterm infant in the neonatal intensive care unit.

The delivery of a preterm baby is a nutrition emergency. Growth and the accumulation of nutrient reserves are higher during the third trimester of pregnancy than at any other time during the life cycle. Enteral nutrition is the preferred mode of support and human milk the preferred source of enteral nutrition. Human milk is highly digestible and contains many anti-infective components, which confer a lower risk of infection. The mother of a preterm infant requires education, equipment, and encouragement to successfully initiate and sustain lactation. Human milk requires nutrient fortification

Correction Factor Evaluation and Between-System Comparison of Behavioral Threshold Predictions From Auditory Brainstem Response Measures in Infants.

PURPOSE: Auditory brainstem response (ABR) thresholds are corrected to estimate behavioral thresholds in infants. Corrections were validated, and a comparison of behavioral threshold estimates between systems was conducted to inform equipment transition and protocols in Ontario, Canada. METHOD: In Study 1, a retrospective file review was conducted. ABR threshold estimates from 84 infants with hearing loss were compared to behavioral thresholds to validate the accuracy of the ABR corrections applied in the Ontario Infant Hearing Program since 2016. Study 2 examined the precision of two differe

Numerical investigation of the convective heat transfer coefficient for a sleeping infant in a ventilation room.

This study aimed to investigate the influence of wind speed and direction on the convective heat transfer from a sleeping infant in different postures. A computational fluid dynamics (CFD) model of a virtual infant manikin with realistic dimensions was developed to obtain the convective heat transfer coefficient (hc ) at the body surface and the airflow and temperature distributions. The numerical model was validated beforehand using experimental data collected from infant thermal manikin experiments. The simulation results revealed that the infant's whole-body hc increased from 4.00 to 15.73

Neonatal cross-infection due to Listeria monocytogenes.

Neonatal listeriosis is rare and detecting more than one case together would be unlikely without a causal link. Thirty-five instances of neonatal listeriosis where cross-infection occurred in the UK and Ireland were reviewed together with 29 other similar episodes reported elsewhere. All instances comprised an infant who was ill at or within one day of delivery and who had direct or indirect contact with a second infant, or in the minority, two or more infants, who then usually developed meningitis 6 to 12 days later. In most instances, the infants were nursed on the same day in obstetric unit

Early assessment of the implementation of a national programme for the prevention of mother-to-child transmission of HIV in Cameroon and the effects of staff training: a survey in 70 rural health care facilities.

OBJECTIVES: To assess the availability of equipment and the staff's knowledge to prevent Mother-To-Child Transmission (PMTCT) in rural healthcare facilities recently covered by the national PMTCT programme in Cameroon. METHODS: In eight districts inventories of antiviral drugs and HIV test kits were made on site, using a standardised check-list. Knowledge of HIV and PMTCT was evaluated with a multiple-choice (MC) questionnaire based on typical clinical PMTCT cases. Staff participated subsequently in a 2-day training on HIV/AIDS and the Cameroon PMTCT guidelines. Immediately after training and

Utilization of Volume-Targeted Ventilation in Neonatal Intensive Care Units in the United Arab Emirates: A Survey of Practicing Neonatologists.

BACKGROUND: Volume-targeted ventilation (VTV) is an evidence-based, lung-protective strategy demonstrated to reduce the incidence of bronchopulmonary dysplasia (BPD) and severe neurologic injury in premature infants. While variations in VTV adoption are well-documented in North America and Europe, data characterizing its utilization across the broader Middle East remains limited. OBJECTIVE: To evaluate the utilization rates of VTV, the selection of tidal volume (VT) targets across various clinical scenarios, and the perceived barriers to VTV implementation among practicing neonatologists in t

[Transportation of sick newborn infants].

In a six month period, 96 newborn infants were transferred from local hospitals to the Department of Neonatology, State University Hospital (Rigshospitalet), Copenhagen. Complications related to transport and potential risk factors were registered prospectively. Considering the condition of the infants, 31% of the transports were not carried out under optimal conditions. Serious complications occurred in 13% of the transports of infants at risk for cardio-pulmonary insufficiency. In 25% of the intubated infants, complications related to displacement of the tracheal tube were observed on arriva

Economic and health consequences of non-invasive respiratory support in newborn infants: a difference-in-difference analysis using data from the Norwegian patient registry.

BACKGROUND: Newborn infants with respiratory failure are often treated with intubation and mechanical ventilation for prolonged periods of time. Our objective was to evaluate whether increasing use of non-invasive respiratory support in newborn infants can improve patient health and reduce costs. METHODS: We utilized a natural experiment that took place in October 2008 when a large neonatal intensive care unit in Norway moved into a new hospital building with new medical equipment. A change in respiratory support towards increasing use of nasal biphasic positive airway pressure (n-BiPAP) inst
