De-implementing spinal injections in pursuit of value-based care: Rethinking pain relief.

Despite guideline recommendations to the contrary, patients experiencing low back pain are commonly exposed to low-value care. Spinal injections for chronic low back pain are a key example of this disconnect. Their persistence highlights the broader challenge of removing low-value practices in pain care. Low-value interventions rarely fade with the introduction of new treatments; rather, their continued use reflects entrenched reimbursement incentives, provider training norms, and patient expectations. This Focus Article applies implementation science concepts to examine spinal injections as a

Artificial Intelligence for Procedural Guidance of Spinal Epidural Steroid Injections: A Scoping Review.

Spinal epidural steroid injections (SESI) are commonly used for radiating low back and cervical pain but remain limited by target imprecision, operator variability, and inconsistent outcomes. Artificial intelligence (AI) has been increasingly investigated as a tool to augment image interpretation, trajectory planning, and decision support in interventional procedures, prompting interest in its potential role in procedural guidance for SESI. This scoping review aimed to summarize the current evidence on the use of AI to support procedural guidance for SESI. A search was conducted across three d

Spinal Injections, Epidural Neurolysis and Denervation for Specific Low Back Pain and Sciatica.

BACKGROUND: Many members of the adult population suffer acute low back pain at some stage in life. A specific cause is found in only a rather small number of these patients. Some of the patients develop chronic low back pain and this is a major source of disability. On the other hand, there has been a great increase in Germany in the number of spinal operations due to degenerative spine disease and, in turn, unnecessary surgery on the spine is under debate. METHODS: This is a narrative review of different minimally invasive spine procedures in the treatment of specific low back pain. The effe

Correlation of spinal canal dimensions to efficacy of epidural steroid injection in spinal stenosis.

PURPOSE: To determine a critical canal dimension in patients with spinal stenosis that predicts response to epidural steroid injections (ESI). METHODS: Lumbar spinal stenosis patients with a computed tomography scan before ESI were identified through ICD-9/CPT codes. Using a digital caliper, canal dimensions on axial cuts of each lumbar intervertebral level were recorded: the transverse canal diameter in line with the facets including the soft tissues, TC; the transverse osseous canal diameter, OS; and the mid-sagittal anteroposterior diameter, MS. Minimum and maximum measurements were determ

Pelvic-Spinal Analysis and the Impact of Onabotulinum toxin A Injections on Spinal Balance in one Child With Cerebral Palsy.

BACKGROUND: In children with cerebral palsy, primary (eg, abnormal muscle tone and weakness) and secondary impairments (eg, contractures) can modify pelvic-spinal alignment. The main aim of this article was to establish a new approach to pelvic-spinal analysis in children with cerebral palsy, taking into account the whole pelvis-spine complex, illustrated by a case study. METHODS: This is a case study of an ambulatory child with cerebral palsy (spastic diplegia) who underwent analysis of the pelvic-spine complex from X-ray images taken in standing position from C2 to the proximal femur. An MR-Conditional Needle Driver for Robot-Assisted Spinal Injections: Design Modifications and Evaluations.

This paper introduces design modifications to our MR-Conditional, 2-degree-of-freedom (DOF), remotely-actuated needle driver for MRI-guided spinal injections. The new needle driver should better meet cleaning and sterilization guidelines needed for regulatory approval, preserve the sterile field during intraoperative needle attachment, and offer better ergonomics and intuitiveness when handling the device. Dy-namic and static force and torque required to properly install the needle driver onto our 4-DOF robot base are analyzed, which provide insight into the risks of intraoperative tool attach

A Stereotactic Device for Intraparenchymal Spinal Cord Injections: Latest Developments for Practical Clinical Use.

This manuscript introduces the latest generation of a patient-mounted platform designed for segmental injections of therapeutics direct into the spinal cord parenchyma. It emphasizes its importance and it presents the rationale for developing this delivery methodology. It compares the newest with the previous generations, detailing how the modifications can streamline transportation, assembly, sterilization, and utilization of the platform by different surgeons. Finally, the illustrations depict the main alterations, as well as a cadaveric assessment of the device prototype in the cervical and

Predicting Response to Epidural Steroid Injections for Lumbar Spinal Stenosis with Biomarkers and Electromyography.

BACKGROUND: Epidural steroid injections (ESIs) may be beneficial for lumbar spinal stenosis (LSS) symptoms. Past studies found interferon-gamma, fibronectin-aggrecan complex, and electromyography (EMG) to predict patients' response to ESIs for other spinal pathologies, but no similar studies have been done for LSS. OBJECTIVE: To explore the ability of biomarkers and EMG to help predict outcome after ESI in LSS. DESIGN: Prospective observational cohort. SETTING: The physical medicine & rehabilitation spine clinic at a tertiary care center. PARTICIPANTS: Eleven patients with LSS were recruit

Interlaminar versus transforaminal epidural steroid injections for the treatment of symptomatic lumbar spinal stenosis.

BACKGROUND: Lumbar spinal stenosis is a common condition that causes axial low back pain, radicular pain, and neurogenic claudication. Epidural steroid injections are commonly used for the treatment of radicular symptoms and neurogenic claudication associated with symptomatic lumbar spinal stenosis. No prior study has evaluated whether transforaminal or interlaminar epidural steroid injections produce better clinical outcomes. DESIGN: Retrospective case control study. METHODS: For each technique, 19 patients were retrospectively identified who received their first fluoroscopically guided epi

Subcutaneous injections as a risk factor of myositis ossificans traumatica in spinal cord injury.

Myositis ossificans traumatica is a pathological condition characterized by extraskeletal bone formation, induced by major or repeated minor trauma to the muscles. Our objective is to report an unusual case of myositis ossificans traumatica in a paraplegic patient. Bilateral swelling and erythema on the thighs of a 1-month paraplegic inpatient was diagnosed as myositis ossificans traumatica in the quadriceps muscles due to low-molecular-weight heparin injections for deep vein thrombosis prophylaxis. As a result, repeated injections under the spinal cord injury level should be avoided when poss

Supraspinal and spinal cord opioid receptors are responsible for antinociception following intrathecal morphine injections.

BACKGROUND AND OBJECTIVE: The clinical practice of spinal morphine administration for pain relief is based on observations in animals that opioid receptors exist in the spinal cord and intrathecal injections of opioids in those species (mostly rats) lead to antinociceptive effects. Clinicians are well aware that administration of spinal opioids is associated with side-effects, such as nausea and respiratory depression, that indicate supraspinal spread of the drug administered. Those observations call into question how much of the observed pain relief is due to action of the drug in the brain.

Spinal subdural abscess following epidural steroid injection.

The authors report the case of a 58-year-old man who presented with a cervicothoracolumbosacral spinal subdural abscess about a month after receiving an epidural steroid injection for management of low-back pain due to L5-S1 disc herniation. Although he presented with symptoms concerning for a spinal etiology, the subdural empyema was not evident on the initial MRI study and was observed on imaging 5 days later. This patient was successfully managed with surgical intervention and antibiotic treatment, and he is doing well more than 21 months after the operation. It is possible that a prior his
