Intraoperative hemodynamic patterns during hypotension prediction index-guided care: a high-resolution secondary analysis of the HYT trial.

BACKGROUND: Although the Hypotension Prediction Index (HPI) reduces intraoperative hypotension, improvements in patient-centered outcomes remain inconsistent. We performed a high-resolution signal analysis to describe the coexistence of arterial pressure, preload-related hemodynamic signals, and algorithm-derived hypotension risk during surgery. METHODS: We conducted a retrospective high-resolution signal analysis (20-second intervals) of 181 patients from the multicenter HYT trial. We quantified hypotension (mean arterial pressure [MAP] <65 mmHg), preload-dependent physiology (stroke volume

Postanesthesia care unit imaging is unnecessary when intraoperative imaging is used during anterior cervical decompression and fusion procedures.

STUDY DESIGN: Retrospective case series. OBJECTIVE: To characterize the clinical utility of imaging in the postanesthesia care unit (PACU) after anterior cervical decompression and fusion (ACDF) procedures. SUMMARY OF BACKGROUND DATA: Two sets of imaging are often taken at the end of ACDF procedures: one intraoperatively and the other in the PACU. The latter may have low clinical utility. MATERIALS AND METHODS: One hundred four patients who underwent ACDF procedures with anterior plate/screw constructs were identified. A panel assessed intraoperative and PACU series for adequacy of images t

Evaluating surface coatings to reduce bone cement adhesion to point of care 3D printed molds in the intraoperative setting.

BACKGROUND: Polymethyl methacrylate, or "bone cement," can be used intraoperatively to replace damaged or diseased bone and to deliver local antibiotics. 3D printed molds allow surgeons to form personalized and custom shapes with bone cement. One factor hindering the clinical utility of anatomically accurate 3D printed molds is that cured bone cement can be difficult to remove due to the strong adhesion between the mold and the bone cement. One way to reduce the adhesion between the 3D printed mold and the cured bone cement is with the use of a surface coating, such as a lubricant. This study

Intraoperative touch imprint cytology of sentinel lymph nodes in breast cancer: experience at a tertiary care center in Mexico.

BACKGROUND: Sentinel lymph node (SLN) biopsy in patients with breast cancer has emerged as a conservative and promising procedure. One of the most important issues is the intraoperative evaluation of the SLN with a high degree of accuracy by frozen section and/or imprint cytology. The objective of this study was to test the ability of intraoperative touch imprint cytology (ITIC) to predict metastasis on SLN. METHODS: SLNs were freshly examined, bisected in <0.5 cm or serially sectioned at 2 mm intervals on the long axis. Each surface of the section was touched on the glass slide, and stained.

Advanced Digital Workflow for Lateral Orbitotomy in Orbital Dermoid Cysts: Integration of Point-of-Care Manufacturing and Intraoperative Navigation.

Background: Orbital dermoid cysts are common benign lesions; however, deep-seated or recurrent lesions near the orbital apex pose major surgical challenges due to their proximity to critical neurovascular structures. Lateral orbitotomy remains the reference approach, but accurate osteotomies and stable reconstruction can be difficult to achieve using conventional techniques. This study reports our initial experience using a fully digital, hospital-based point-of-care (POC) workflow to enhance precision and safety in complex orbital dermoid cyst surgery. Methods: We present a case series of thr

Lack of Antenatal Care Resulting in Intraoperative Diagnostic Challenges of Dense Adhesions Imitating Placenta Accreta Spectrum: A Case Report.

BACKGROUND: Adhesions are a common long-term complication following surgery and can complicate surgical access. In some cases, intraoperative adhesion findings may resemble the placenta accreta spectrum (PAS), leading to diagnostic confusion and unnecessary referral to tertiary care centers. This case highlights a misdiagnosis of suspected PAS, which was ultimately identified as intraoperative adhesions. CASE ILLUSTRATION: A 30-year-old woman, G2P1 (2nd pregnancy), at full-term gestation, was referred from a district hospital with a prior history of cesarean section 12 years ago. The patient

Ultrasound Rounds: Intraoperative Point-of-Care Ultrasound Identifies Intracardiac Thrombus and Pulmonary Embolism Allowing for Alteration in Surgical Management.

Most of the cases demonstrating describing acute findings using point-of-care ultrasound (POCUS) have been described in emergency medicine and critical care medicine. While the use of POCUS has become more prevalent in anesthesia practice, documentation of acute findings resulting in alteration in management based on real-time ultrasound findings during pediatric anesthesia remains limited. This case highlights the use of POCUS during cardiopulmonary collapse occurring during correction of neuromuscular scoliosis. POCUS excluded the presumed diagnosis of venous air embolism and identified an i

[Intraoperative guidance of anesthesia: Analgesie Nociception Index (ANI) vs. standard care for hysterectomy under anesthesia with sevoflurane : A randomized controlled simple blinded study on intraoperative opioid consumption, postoperative pain and patient satisfaction].

BACKGROUND: General anesthesia leads to different effects depending on the drugs used (hypnosis, analgesia, muscle relaxation). While there are validated methods for clinical monitoring and control of hypnosis and muscle relaxation in routine anesthesia, the assessment of analgesia is still mainly based on the interpretation of clinical vital parameters, such as heart rate, blood pressure, perspiration or intraoperative movements of the patient. The present clinical study investigated whether the use of a so-called nociception monitor to record the intraoperative need for analgesics is superio

Guidelines for Enhanced Recovery After Trauma and Intensive Care (ERATIC): Enhanced Recovery After Surgery (ERAS) Society and International Association of Trauma Surgery and Intensive Care (IATSIC) Recommendations: Paper 1: Initial Care-Pre and Intraoperative Care Until ICU, Including Non-Operative Management.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols reduce length of stay, complications, and costs for elective surgical procedures. It remains challenging to implement ERAS concepts in the acute trauma patient due to deranged physiological reserve from the penetrating or blunt trauma producing altered physiology. However, systems of care improve access to early intervention and potentially reduce mortality. These consensus guidelines examine optimal pre-hospital, resuscitation-room, intra-, and post-operative treatment, systems of ethical management, and overall care for trauma pati

Incidence and risk factors for nursing-related intraoperative incidents: A 5-year retrospective study at a Japanese tertiary care hospital.

AIM: Nursing-related intraoperative incidents, such as specimen loss and retained surgical items, pose serious risks to patient safety. These events may be influenced by surgical complexity, team structure, and nursing experience. However, data on the specific impact of these factors, particularly in Japan, remain limited. This study aimed to determine the incidence of nursing-related intraoperative incidents and identify clinical and procedural factors associated with their occurrence. METHODS: We conducted a retrospective observational study of all surgeries performed at a tertiary care hos

Effect of Intraoperative Handovers of Anesthesia Care on Mortality, Readmission, or Postoperative Complications Among Adults: The HandiCAP Randomized Clinical Trial.

IMPORTANCE: Intraoperative handovers of anesthesia care are common. Handovers might improve care by reducing physician fatigue, but there is also an inherent risk of losing critical information. Large observational analyses report associations between handover of anesthesia care and adverse events, including higher mortality. OBJECTIVE: To determine the effect of handovers of anesthesia care on postoperative morbidity and mortality. DESIGN, SETTING, AND PARTICIPANTS: This was a parallel-group, randomized clinical trial conducted in 12 German centers with patients enrolled between June 2019 a

[Anaesthesia and intensive care during intraoperative intraperitoneal hyperthermic chemotherapy in patients with gastric cancer (literature review and own clinical experience)].

UNLABELLED: Despite a steady decline in the incidence in recent decades, gastric cancer remains one of the most frequent causes of death from cancer in the world. By the time of diagnosis the tumor process is locally advanced or generalized in nearly 70% of patients that reduces the possibility of radical surgical treatment. Peritoneal carcinomatosis is a one of the main causes of treatment failure. There patients need complex treatment with cytoreductive surgery and intraoperative hyperthermic intraperitoneal chemotherapy (IHIC) to improve the quality and duration of life. Today in Russia, su

Intraoperative Diagnosis of Situs Inversus Totalis in Laparoscopic Cholecystectomy: Taking Into Consideration Patient-Centered Care in a Limited-Resource Case.

Situs inversus (SI) is a rare congenital disorder in which the abdominal and thoracic organs create a mirror image orientation of their typical anatomical positions. In this article, we present the case of a 49-year-old female individual who underwent a scheduled laparoscopic cholecystectomy (LC) due to postprandial colic pain in the epigastrium. A prior ultrasound examination, conducted a year earlier, revealed the presence of small gallstones within the gallbladder, and a follow-up preoperative ultrasound suggested the possibility of SI, though the diagnosis was ruled out by a third ultrasou

Utility of Fluorescent Microscopy for Rapid Intraoperative Diagnosis of Rhinocerebral Mucormycosis: Experience from a Tertiary Care Center in Central India.

Rhino-orbital-cerebral mucormycosis is an uncommon life-threatening infection caused by the angioinvasive fungus Mucorales and is associated with high morbidity and mortality. In India, the pandemic of COVID-19 was associated with another deadly disease, rhinocerebral mucormycosis, which further complicated the course of the disease, necessitating an accurate and rapid diagnosis. Conventional methods of diagnosis, like fungal culture, histopathology, Gomori methenamine silver (GMS), and periodic acid-Schiff (PAS) stain, are not feasible for intraoperative diagnosis. We studied the use of fluor

Preoperative oral carbohydrate loading: Effects on intraoperative blood glucose levels, post-operative nausea and vomiting, and intensive care unit stay.

BACKGROUND AND AIMS: Preoperative fasting imparts safety to patients from aspiration of gastric contents, but undue fasting may predispose to hypoglycemia and dehydration. Preoperative carbohydrate drink reduces postoperative nausea and vomiting (PONV). Primary objective of the present study was to assess effect of carbohydrate drink given 2h before surgery on intraoperative blood glucose levels as compared to those who did not receive it. Secondary objectives included assessment of incidence and severity of PONV and duration of Intensive Care Unit (ICU) stay. MATERIAL AND METHODS: It was a p

Counting the seconds: a quality improvement initiative to accelerate intraoperative results for arthrocentesis cell counts in a paediatric tertiary care hospital.

INTRODUCTION: In paediatric patients with concern for septic arthritis, arthrocentesis may be performed under anaesthesia with intraoperative cell count determining need for surgical intervention. Shorter turnaround time (TAT) between collection and result minimises anaesthesia exposure for the patient and surgical time for the treatment team. This study evaluated a quality improvement (QI) initiative to decrease TAT to <1 hour by improving interdisciplinary communication and placing visual reminders ('stat card') to indicate priority handling. METHODS: 206 consecutive paediatric patients who

Controlling intraoperative hemorrhage during burn surgery: A prospective, randomized trial comparing NuStat® hemostatic dressing to the historic standard of care.

INTRODUCTION: One of the primary intraoperative challenges during burn surgery is to adequately excise the burn while avoiding massive hemorrhage. This has become increasingly important, as we see more burn patients that are older and with more medical comorbidities. While adequate excision down to healthy tissues for deep burns is essential for skin graft to take, it also leads to active bleeding that can be a challenge to control. Good hemostasis is imperative as a hematoma is the most common cause of graft loss. Several new products have become available to help control intraoperative hemor
