The Banff pancreas working schema for diagnosis and grading of rejection is widely used for treatment guidance and risk stratification in centers that perform pancreas allograft biopsies. Since the last update, various studies have provided additional insight regarding the application of the schema and enhanced our understanding of additional clinicopathologic entities. This update aims to clarify terminology and lesion description for T cell-mediated and antibody-mediated allograft rejections, in both active and chronic forms. In addition, morphologic and immunohistochemical tools are describ

PURPOSE OF REVIEW: This review describes the utility and limitations of measure for assessing the presence, relative strength, and clinical impact of human leukocyte antigen (HLA) alloantibodies, as well as the other qualitative features of antibodies that are important considerations in assessing patient risk.
RECENT FINDINGS: Using MFI as a measure of antibody amount is limited for a variety of reasons. Standardized serum manipulations such as ethylene-diamine-tetra-acetic acid treatment or serum dilution results in better definition of relationships between MFI and antibody titer or comple

BACKGROUND: Alloantibodies to the low-frequency antigen Scianna-2 (Sc2) have been implicated in cases of hemolytic disease of the fetus and newborn but never in hemolytic transfusion reactions (HTRs); thus, the clinical significance of anti-Sc2 has yet to be fully addressed.
STUDY DESIGN AND METHODS: A 26-year-old woman with thalassemia presented rigors, fever, nausea, abdominal pain, and hemolytic biochemistry after exposure to 75 mL of plasma-reduced red blood cells (RBCs). The RBC unit was issued by electronic crossmatch but was 3+ incompatible on recrossmatch by gel indirect antiglobulin

Despite the success of desensitization protocols, antibody-mediated rejection (AMR) remains a significant contributor to renal allograft failure in patients with donor-specific antibodies. Plasmapheresis and high-dose intravenous immunoglobulin have proved to be effective treatments to prevent and treat AMR, but irreversible injury in the form of transplant glomerulopathy can commonly manifest months to years later. There is an unmet need to improve the outcomes for patients at risk for AMR. Updated Banff criteria now take into account the increasing understanding of the complex and heterogene

BACKGROUND: Tests for platelet-specific antibodies are important in the diagnosis of immune platelet disorders. Monoclonal antibody glycoprotein capture assays have been the gold standards in platelet antibody detection for almost 30 years. However, such assays are complex and cumbersome to perform, which limits their routine use. We report the performance of a newly developed, easy to perform platelet antibody bead array (PABA) for the detection of platelet-specific antibodies.
STUDY DESIGN AND METHODS: PABA is the equivalent of the monoclonal antigen capture enzyme-linked immunosorbent assa

The detection and characterization of anti-HLA antibodies and the clinical impact of their appearance following renal transplantation are areas of immense interest. In particular, de novo development of donor-specific antibodies (DSA) has been associated with acute and chronic antibody-mediated graft rejection (AMR). Recently, methods for antibody detection have evolved remarkably from conventional cell-based assays to advanced solid phase systems. These systems have revolutionized the art of defining clinically relevant antibodies that are directed toward a renal graft. While anti-HLA DSAs ha

Modification of low density lipoprotein (LDL) particles due to oxidation, glycation and binding of advanced glycation end-products (AGEs) or malondialdehyde (MDA, a final product of lipid peroxidation) is considered most important in the process of atherogenesis. Oxidatively modified LDL are distinguished by another receptor type, which was discovered on the surface of macrophages and was called the scavenger receptor. Uncontrolled intake of LDL converts macrophages to foam cells; their accumulation under the vascular endothelium is considered as the first stage of atherosclerosis. Oxidation o

To develop a novel enhancement medium using polyethylene glycol (PEG)-conjugated gold nanoparticles (AuNPs) for improved detection of clinically significant alloantibodies in antibody screening tests (AST), addressing the limitations of low ionic strength solution (LISS). PEG AuNPs were synthesized through a one-step method and characterized using UV-Visible spectroscopy, EFTEM, FESEM, particle size analysis, zeta potential measurement, and FT-IR spectroscopy. The PEG AuNPs' performance was evaluated against LISS in 120 plasma samples (both antibody-positive and negative) by scoring RBC agglut

INTRODUCTION: Antibodies of the Kidd blood group system are associated with hemolytic transfusion reactions and, less commonly, hemolytic disease of the fetus and newborn. Naturally occurring antibodies against Kidd antigens are exceedingly rare, with a limited number of cases reported worldwide. We report a case of a naturally occurring anti-Jka antibody detected in a 6-month-old male infant.
METHODS: The patient was admitted with a high fever after routine age-appropriate vaccinations. He had no history of blood transfusion, underlying disease, or perinatal complications. Blood, urine, and

PURPOSE OF REVIEW: Antibody-mediated rejection (ABMR) is the leading cause of allograft failure in kidney transplant recipients. This article provides a summary of the clinical research relating to donor-specific antibodies (DSA) and ABMR in kidney transplant recipients at the University of Wisconsin-Madison Transplant Center.
RECENT FINDINGS: Over 40% of the kidney transplant candidates on the UNOS waitlist are sensitized, and both preformed and de novo DSA are associated with increased risk of rejection and graft loss. We have developed graded induction-desensitization treatment and monitor

OBJECTIVE: Antibody-mediated rejection (ABMR) is an important cause of kidney allograft injury. In the last two decades, detection of complement split product C4d along transplant capillaries, a footprint of antibody-mediated classical complement activation, has evolved as a useful diagnostic marker of ABMR. While it was recognized that ABMR may occur also in the absence of C4d, numerous studies have shown that C4d deposition may indicate a more severe rejection phenotype associated with poor graft survival. Such studies suggest a possible diagnostic benefit of ex vivo monitoring the complemen

BACKGROUND: Naturally occurring alloantibodies produced against A and B red cell antigens in cats can cause acute hemolytic transfusion reactions. Blood incompatibilities, unrelated to the AB blood group system, have also been suspected after blood transfusions through routine crossmatch testing or as a result of hemolytic transfusion reactions.
HYPOTHESIS: Incompatible crossmatch results among AB compatible cats signify the presence of a naturally occurring alloantibody against a newly identified blood antigen in a group of previously never transfused blood donor cats. The associated alloant

Haemophilia patients with inhibitors are treated for acute bleeding with prothrombin complex concentrates (PCCs) or activated prothrombin complex concentrates (aPCCs). Despite this therapy, patients with high-level inhibitors are at increased risk of developing devastating joint disease. This paper examines available information that supports the study of PCCs and/or aPCCs as prophylactic therapy for haemophilia patients with inhibitors. This strategy would require that PCCs or aPCCs be administered repetitively in a dose that is sufficient to prevent haemarthrosis without causing thrombogenic

Using a commercial microwave oven (750 W, 2,450 MHz), we compared the use of microwave energy to the commonly used method of acid-glycine elution (Elu-Kit II, Gamma Biologicals, Houston, Tex., USA) to elute antibodies from direct antiglobulin test positive red blood cells (RBC). Using a 33% suspension of RBCs in cold saline (1-6 degrees C) in a polypropylene test tube, the microwave technique was comparable for eluting Rh (except anti-c and anti-e) and Kell and superior for eluting Duffy, Kidd antibodies while acid-glycine was superior in eluting S and s antibodies. Consistent results were obt

Two infants of one family and one infant of another with neonatal alloimmune granulocytopenia are described. All of the infants had mild infections, two of them mild pyogenic skin infections. Total leukocyte counts were within the normal range in all cases. The absolute neutrophil count was 0-0.2 x 10(9)/l and remained subnormal up to nine weeks. Both mothers had antibodies against their husbands' granulocytes. Detection of the antibody made it possible to postpone more invasive examinations. Parents should be examined if neutropenia in a newborn infant cannot be attributed to a normal cause s
