The genetics of keratoconus.

Keratoconus is a relatively common, bilateral, non-inflammatory corneal ectasia. The aetiology of this condition is probably multifactorial, or it represents the final common pathway for a variety of different pathological processes. Although a familial history is present only in a minority of cases, one of the major aetiological factors is certainly genetic. This is evidenced by: the condition's familial inheritance; its discordance between monozygotic and dizygotic twins; and its association with other known genetic disorders such as Down's and Marfan's syndromes. In the keratoconic cornea,

Iridoschisis and keratoconus.

We report herein a patient with iridoschisis and keratoconus. Since the posterior layers of the cornea and the iris stroma have a common embryological deviation, the combination of these two conditions suggests an inter-related pathogenesis. This case is similar to those of patients with progressive essential iris atrophy and keratoconus described by Cavanagh.

Epikeratoplasty for keratoconus.

The 6 eyes of 6 patients operated on using keratoconic epikeratoplasty are evaluated, and the mean 11-month postoperative results are reported. Uncorrected visual acuities improved in 4 patients and were maintained in 1 patient. One patient revealed a decrease of uncorrected visual acuity in the postoperative 6th month. Two patients showed an improvement of over 2 Snellen lines. Two patients showed the same corrected visual acuity, and that of 2 patients decreased within the postoperative 6-months follow-up. There was a significant decrease of myopia in terms of spherical equivalent. As a resu

Epikeratophakia for keratoconus.

Results of the author's first 15 cases of epikeratophakia for keratoconus are presented. All patients were intolerant of contact lens correction and could not achieve satisfactory acuity with spectacle correction due to corneal ectasia and irregular astigmatism. The average follow-up was 13 months. Seventy-three per cent of eyes treated had a pre-operative spectacle acuity of 6/60 or worse. One eye had a preexisting amblyopia that limited final acuity, but every other eye achieved a post-operative spectacle acuity of 6/12 or better and 50% achieved 6/6. One eye had 11 dioptres of post-operativ

Personality and keratoconus.

We studied personality traits of patients with keratoconus in a controlled prospective study using the Minnesota Multiphasic Personality Inventory (MMPI). Whereas four of 16 (25%) control patients with herpes simplex keratitis had scores indicative of psychological abnormality, 15 of 28 (54%) keratoconus patients had scores indicative of abnormality (P less than .05). Keratoconus patients who had undergone penetrating keratoplasty had a lower rate of abnormal scores (3 of 9; 33%) than unoperated keratoconus patients (12 of 19; 63%) (P less than .05). Patients who considered themselves moderate

[Intraindividual Keratoconus Progression].

Introduction Keratoconus is a typically bilateral, progressive-extending corneal disease. The aim of this work is to evaluate the intraindividual progression of keratoconus. Patients and Methods This single-center retrospective study of disease progression based on topographic data from non-operated keratoconus patients (n = 48) was examined over a period of two years. Seven topographical keratoconus indices derived from Scheimpflug tomography were used to compare both the initial findings, as well as the progression of the fellow eyes. Results The mean observation period was 3.3 ± 0.9 years.

Keratoconus: current perspectives.

Keratoconus is characterized by progressive corneal protrusion and thinning, leading to irregular astigmatism and impairment in visual function. The etiology and pathogenesis of the condition are not fully understood. However, significant strides have been made in early clinical detection of the disease, as well as towards providing optimal optical and surgical correction for improving the quality of vision in affected patients. The past two decades, in particular, have seen exciting new developments promising to alter the natural history of keratoconus in a favorable way for the first time. T

Circumscribed posterior keratoconus.

BACKGROUND: Posterior keratoconus is characterized by an internal protrusion of the posterior corneal surface and associated localized or diffuse stromal thinning. Two cases of circumscribed (localized) posterior keratoconus are presented that are representative of the spectrum of the anomaly. CONCLUSIONS: The clinical and histological appearance of posterior keratoconus and its differential diagnosis from other corneal ectasias and developmental anomalies are reviewed to enable the eye care practitioner to better diagnose and manage this rare anterior segment disorder.

Obesity and Keratoconus: A Mediation Study Based on the Chinese Keratoconus Cohort.

PURPOSE: Growing evidence indicates a strong association between obesity and keratoconus (KC), although the potential mediating factors remain largely unknown. This study aimed to explore the potential pathways through which corneal biomechanical and topographic parameters might link obesity and KC using structural equation modeling (SEM). METHODS: This case-control study included 759 participants (363 patients with KC and 396 controls) from the Chinese Keratoconus (CKC) Cohort Study. Associations among body mass index (BMI), corneal biomechanical parameters, topographic parameters, and KC we

Sensitivity and specificity of Sirius indices in diagnosis of keratoconus and suspect keratoconus.

AIM: To estimate the sensitivity and specificity of topographic and tomographic corneal parameters as determined by Sirius (CSO, Florence, Italy) in discriminating keratoconus (KC) and suspect keratoconus from normal cornea. METHOD: In this retrospective case-series study, keratoconus screening indices were measured using Sirius tomographer. Receiver operating characteristics (ROC) curves were used to determine the test's overall predictive accuracy (area under the curve) and to identify optimal cut-off points to maximize sensitivity and specificity in differentiating keratoconus and suspect

[Keratoconus progression assessment].

Keratectasias including keratoconus are pathological curvature anomalies of the human cornea, which can progress and lead to permanent visual impairment. Corneal crosslinking can be used to stabilize the disease. Criteria for performing crosslinking have been defined by legislation in Germany, including an increase of the maximum anterior surface curvature Kmax by ≥ 1 diopter within 12 months. Unfortunately, there is still a lack of generally applicable standards for assessment of keratectasia progression. This overview presents the possibilities of topographic, tomographic and biomechanical a
