A 40-year old man, highly myopic in both eyes, had laser in situ keratomileusis (LASIK) in the left eye in November 1996. Corneal melting and ulceration and fine striae-like interface infiltrates were noticed 1 day postoperatively. There was no response to intensive topical antibiotics in the form of hourly ofloxacin 3% (Tarivid), and satellite lesions developed on day 4. PURPOSE: Ectasia after laser in situ keratomileusis (LASIK) is a rare but serious complication. Prevention includes proper patient selection with detection of those at particular risk. UNLABELLED: A 36-year-old woman with chronic psoriasis developed blurring of vision and intraepithelial infiltrates within the flap in the left eye concurrent with the appearance of acute psoriatic generalized skin lesions 14 days after bilateral laser in situ keratomileusis. PURPOSE: To analyze the safety of photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK) with the Nidek EC-5000 excimer laser for surgery performed from Jan 1999 to Dec 2001.
METHODS: Retrospective study of excimer laser in 683 eyes: PRK was performed in 103 eyes and LASIK was performed in 580 eyes using the Nidek EC-5000 excimer laser and Moria Carriazo Barraquer microkeratome. We classified complications as intraoperative, early postoperative (30 days) and late postoperative complications. All patients were followed for at least 6 months.

Three patients who had laser in situ keratomileusis (LASIK) experienced severe direct corneal trauma 3 to 38 months postoperatively. Flap dislocation resulted in all 3 patients. Presentation following trauma ranged from a few hours to 9 days. All patients were successfully managed by surgical flap repositioning, and all maintained a best corrected visual acuity of 20/20. A literature review of traumatic flap dislocation cases and the most recent methods of diagnosis and management are included.

A 37-year-old woman had bilateral hyperopic laser in situ keratomileusis (LASIK). Six months postoperatively, an epithelial pigmentation ring pattern was identified on both corneas. The appearance of the ring pattern was similar to the iron deposits of the Fleischer ring of keratoconus. While corneal iron deposits in various patterns have been reported after other forms of ocular therapeutic and refractive surgery, this is the first report of the association between hyperopic LASIK and a corneal iron line, which we have called the "pseudo-Fleischer ring."
