The first successful living-donor lobar lung transplant (LDLLT) in Japan was performed at Okayama University in October 1998 after a long period of waiting for the legalization of thoracic organ transplantation. By May 2003, a further 41 lung transplants had been performed; 28 from living donors and 13 from cadaveric donors. The indications for a lung transplant are very specialized in Japan, the most common being primary pulmonary hypertension. Although the number of lung transplants is still small, survival in Japan, at 72% after 4 years, is better than the world average. Because the number

BACKGROUND: On the basis of a 5-year experience with heart transplantation and long-term animal experimentation, a lung transplantation program was instituted in 1987. After 10 years of experience, the entire patient population was reviewed.
METHODS: Hospital records were reviewed to evaluate the underlying diagnosis, year of transplantation, type of procedure, and long-term follow-up. The changing scope of indications, procedures performed, and donor criteria, as well as survival data for various subgroups of high-risk candidates, were also examined.
RESULTS: A total of 283 heart-lung (n =

Lung transplantation has become the therapeutic hope of terminal respiratory patients. Infections are among the main causes of morbidity and mortality in these patients. We therefore analyze infections suffered after lung transplants performed at Clínica Puerta de Hierro over a two-year period. The cases of 14 transplanted patients, 6 bilateral and 8 unilateral, were analyzed. Pre-transplant data available for all included analysis of serum antibodies to several viruses, as well as bacterial and fungal cultures, and Ziehl's sputum test. All received prophylactic antibiotic, antifungal and anti

Selected patients with terminal lung disease have been managed effectively by lung transplantation. Strict selection criteria for donors and recipients, attention to technical detail, and avoidance of perioperative corticosteroids increase the likelihood of success. The underlying pulmonary disease determines the appropriate procedure. Single-lung transplantation is most appropriate for patients with pulmonary fibrosis. Patients with emphysema or septic pulmonary disease who have adequate or recoverable cardiac function can be well served by double-lung transplantation. However, such patients

Between February 1988 and December 1989, 15 combined heart-lung, 2 double lung and 5 single lung transplants were performed at our institution for end stage lung disease. The indication for heart-lung transplantation was primary lung disease with associated secondary heart failure in 11 cases, diffuse pulmonary disease with extensive adenopathy of the hilum in 2 cases and profuse and antibiotic-resistant tracheobronchial infection due to Pseudomonas in 2 cases. A double lung transplant was performed in 2 patients with hypertensive emphysema. The indication for a single lung transplantation was

BACKGROUND: Renal dysfunction is common after lung and heart-lung transplantation (Tx), and it limits the recipient's survival and quality of life. This study analyzed the outcomes of simultaneous and late kidney Tx following lung and heart-lung Tx.
METHODS: From a single-center retrospective chart review of 1031 lung and heart-lung Tx recipients, we identified 13 simultaneous or late kidney Tx cases in 12 patients.
RESULTS: Three patients underwent simultaneous deceased donor lung and kidney Tx. Eight patients underwent lung and heart-lung Tx, followed by nine living donor kidney Tx (includ

Psychiatric problems in a consecutive series of lung transplant applicants (n = 116), candidates (n = 57) and recipients (n = 42) are reported. Fifty percent of all the applicants for lung transplantation reported a history of psychiatric disorder including organic brain syndrome (19%), major depressive episode (16%), panic or anxiety disorder not otherwise specified (11%) and alcohol or substance abuse (11%). In most cases, psychiatric problems had occurred in these patients as a complication of endstage lung disease. The most common new diagnoses in candidates awaiting surgery were adjustmen

The number of organ donations after brain death has significantly increased since the revised Japanese Organ Transplant Law took effect in July 2010. Sixty-one lung transplantations were conducted throughout Japan in 2013, including 20 living-donor lung transplantations and 41 brain-dead-donor lung transplantations (23 bilateral lungs, 17 single lungs, and 1 heart-lung transplantation). The number of lung transplant candidates newly registered at the Japan Organ Transplantation Network also increased to 126 in 2013, suggesting a severe donor shortage in Japan. More than 60 % of offered brain-d

OBJECTIVE: The purpose of this study was to compare outcomes after heart-lung or double-lung transplantation in patients undergoing transplantation because of end-stage suppurative lung disease.
METHODS: We reviewed our experience in patients with cystic fibrosis or bronchiectasis who had heart-lung or double-lung transplantation between January 1988 and September 1997. Twenty-three patients (14 male, 21 cystic fibrosis) had heart-lung transplantation and 24 patients (8 male, 19 cystic fibrosis) had double-lung transplantation. There were no statistically significant differences between the g

Lung transplantation is a valid treatment in cystic fibrosis patients at end stage of the disease. It is decided when the vital prognosis seems to be engaged within two years. Patients are put on the waiting list after they have been checked out according to their individual potential risks and difficulties. The waiting time on list may be prolonged and should be used to optimize nutritional status (gastrostomy) and to take into account the psychological aspect of the project. The choice between double lung transplantation and heart lung transplantation depends on teams experience. Survival ra

During the last decade, activity in lung transplantation increased rapidly to a plateau of approximately 900 transplants per year in the United States, and it is unlikely to increase further unless the supply of usable donor organs can be expanded. However, the demand for lung transplantation has continued to rise, and the median waiting time for lung transplantation in the United States is approaching 2 years. The main indications for lung transplantation have been chronic obstructive pulmonary disease, alpha1-antitrypsin deficiency emphysema, cystic fibrosis, idiopathic pulmonary fibrosis, p

Thanks to a simplification of surgical techniques, single or double lung transplants have expanded significantly in latter years. Infection remains an important cause for morbidity and mortality, more so in early rather than late stages. Bacterial infections cause approximately fifty per cent of all infections. They can be prevented in part by prophylaxis. Infections to CMV have become less frequent thanks to adequate prophylaxis with ganciclovir. Herpetic infections are prevented by acyclovir or ganciclovir. A better control of immunosuppression seems to be associated with fewer lymphoprolife

In recent years, heart transplantation has become a clinically routine means of treating patients with terminal cardiac disease. One-year survival rates of more than 80%, and five-year rates of about 70% vindicate the clinical acceptance of this form of treatment. Combined heart and lung transplantation has been practiced for the past ten years. Following the introduction of unilateral and bilateral lung transplantation in 1985, the combined heart/lung approach is now rarely indicated. In the treatment of interstitial, but also pulmonary vascular, diseases, unilateral and bilateral lung transp

End-stage lung disease in Cystic Fibrosis (CF) now is considered to be one of the indications for heart-lung or double lung transplantation. Results of this surgery for 50 or so CF patients in the US and Europe are about the same as for other diseases, although there are some postoperative problems specific for this diagnosis. These include: need for higher oral dosages of cyclosporine, likelihood of precipitation of diabetes mellitus with high dosage corticosteroid therapy for acute lung rejection, constant threat of pathogens remaining in the sinuses, increased likelihood of drug toxicity to

OBJECTIVE: To describe a series of patients with pathogenic variants in FLNA and progressive lung disease necessitating lung transplantation.
STUDY DESIGN: We conducted a retrospective chart review of 6 female infants with heterozygous presumed loss-of-function pathogenic variants in FLNA whose initial presentation was early and progressive respiratory failure.
RESULTS: Each patient received lung transplantation at an average age of 11 months (range, 5-15 months). All patients had pulmonary arterial hypertension and chronic respiratory failure requiring tracheostomy and escalating levels of

The association between interstitial lung disease and polymyositis/dermatomyositis is well known. It severely affects patients' quality of life, worsens prognosis and represents a major risk factor for premature death. Current treatment is unclear and therapeutic options are based on case series. We report the case of a 15-year old female diagnosed with end-stage lung disease associated to polymyositis who received a double lung transplant after 20 days of extracorporeal membrane oxygenation. She died 9 months later and microscopic post-mortem findings revealed recurrence of interstitial lung

As survival improves after lung and heart-lung transplants, the long term detrimental impact of current management on renal function becomes more apparent as the number of non-renal solid organ transplant recipients on renal transplant waiting lists increases. Progressive chronic kidney disease (CKD) is a significant cause of morbidity and mortality in the transplant population. In this review we discuss the specific problems prior to lung or heart-lung transplant that predispose to CKD, as well as potential renal complications encountered during the peri- and post-transplant period. BACKGROUND: The optimal therapy for end-stage Eisenmenger syndrome (ES) is unknown. We analyzed the United Network for Organ Sharing/International Society for Heart and Lung Transplantation Joint Thoracic Registry to determine predictors of survival.
METHODS: Univariate analysis was performed using Kaplan-Meier survival curves. Groups were compared using the log-rank test. Multivariate analysis was performed using a proportional hazards model.
RESULTS: There were 605 transplants performed between 1988 and 1998. The causes of ES included atrial septal defect (ASD) in 171, ventricular septal d

The main long-term complication after lung transplantation is chronic rejection in terms of obliterative bronchiolitis; when therapy does not arrest the progression of the airflow obstruction, lung retransplantation seems to be. at present, the only strategy in the management of chronic rejection. We report the 12 month follow-up of a single lung retransplantation in a 21 yr old female who had received a heart-lung transplantation 35 months previously for Eisenmenger syndrome. The patient had excellent first allograft function and quality of life for 26 months, then progressively deteriorated

Bronchial healing is good with heart lung transplantation but this technique is actually not often performed. Single lung transplantation and bilateral sequential lung transplantation (two single lung transplantations) have at present an incidence of anastomotic problems of between 4 and 30%. Efforts to improve surgical technique and medical perioperative treatment are described. Often the anomalies seen improve by themselves. On the other hand, endoscopic treatment is usually performed (dilation, yag laser, cryotherapy, stents) to maintain the bronchial airway opened. Precocious resutures, la
