OBJECTIVE: The study aimed to evaluate the changes in mandibular width after sagittal split ramus osteotomy (SSRO) in patients with mandibular asymmetric prognathism using cone-beam computed tomography (CBCT).
METHODS: Seventy patients who underwent SSRO for mandibular setback surgery were included in two groups, symmetric (n = 35) and asymmetric (n = 35), which were divided according to the differences in their right and left setback amounts. The mandibular width was evaluated three-dimensionally using CBCT images taken immediately before surgery (T1), 3 days after surgery (T2), and 6 months

BACKGROUND: Post-traumatic mandibular malocclusion is a complex condition that poses a significant challenge to reconstructive surgeons. The malocclusion that ensues from bilateral condylar and parasymphseal fractures presents a particular challenge as it leads to bilateral posterior shortening and lingual tilting of dental arch leading to a combination of open anterior bite, crossbite, overbite, underbite, and/or facial asymmetry. The complexity of such malocclusion requires intricate freedom of movement of the mandibular arch that can be achieved by performing a 5-segment mandibular osteotom

The aim of this study was to evaluate the results and complications following supra-foraminal high oblique osteotomy (SHOO) using patient-specific guides and implants (PSI). We selected consecutive patients who had undergone SHOO using PSI from May 2017 to December 2018 and had a minimum follow-up of 6 months. We collected the initial malocclusion and deformity, treatment planning, intraoperative and postoperative complications, stability, lower lip sensitivity, and temporo-mandibular joint disorders at 6 months. We also verified the amount of bone contact and mandibular sagittal repositioning

OBJECTIVES: The aim of this study was to evaluate the sensory function of the infraorbital nerve after orthognathic surgery (OS).
MATERIALS AND METHODS: Patients who underwent Le Fort I osteotomy with or without BSSO for dentofacial deformity treatment were studied. Two groups were created according to whether BSSO was performed. Class A tests were performed to determine the degree of peripheral nerve damage. The Class B test was performed if decreased sensation was detected in at least one of these tests. A Class C test was performed if abnormal sensation was detected.
RESULTS: Twenty-eight

The current standard approach to treat class II malocclusion is orthodontic preparation followed by a mandibular advancement through bilateral sagittal split osteotomy associated or not with a genioplasty. However, when a malocclusion with an important incisors' axis deviation is associated with a chin malposition, an alternative technique can be considered: simultaneous mandibular anterior segmental osteotomy and genioplasty. This technique is performed without a bony bridge between osteotomies. Care must be taken not to wound the lingual mucosa. If preserved, this technique is safe. The resu

BACKGROUND: Several methods are available for mandibular reduction, and each method is characterized by unique limitations and complications. However, only a few studies have systematically analyzed these complications. This study aimed to investigate the effects of the outcome of performing curved mandibular angle ostectomy and outer cortex grinding in 1-stage operation and to examine the causes of different types and characteristics of complications.
METHODS: The subjects consisted of 528 patients who were subjected to curved mandibular angle ostectomy and outer cortex grinding in 1-stage o

There is little objective data about whether surgical technique or mandibular anatomy are a risk for inferior alveolar nerve (IAN) injury during bilateral sagittal split osteotomy (BSSO). Orthodromic sensory nerve action potentials (SNAPs) of the IAN were continuously recorded on both sides in 20 patients with mandibular retrognathia during BSSO operation. Changes in latency, amplitude, and sensory nerve conduction velocity (SNCV) at baseline and at different stages of the operation were analyzed. The SNAP latencies prolonged, the amplitudes diminished, and the SNCVs slowed down during BSSO (P

Le Fort I osteotomy with anterior and superior repositioning of the maxilla was performed on adolescent Macaca fascicularis monkeys to study its effect on the subsequent growth and remodeling changes of the mandible. Six adolescent Macaca fascicularis monkeys were randomly divided into two surgical groups and eight others served as controls. Group I animals underwent a Le Fort I advancement and Group II animals experienced a Le Fort I advancement and impaction. All monkeys received tantalum implants in the anterior part of the cranial base, on opposite sides of craniofacial sutures and in mult

Two women, 43 and 46 years old, weighing respectively 51 kg and 49 kg, underwent mandibular osteotomy with retrusion, after which they began to snore heavily at night. The initial events were central; obstructive apneic and hypopneic events, documented by polysomnography, developed over time. When patient 2 was on her back, she frequently had central apneas (identified by esophageal pressure monitoring) preceded by increasing swings in pressure, indicative of increased airway resistance without oxygen desaturation.

This study systematically investigates the influence of different interface modelling assumptions on biomechanical predictions using patient-specific models and evaluates their agreement with postoperative configurations. Five mandibular reconstruction cases using vascularised osseous free flaps were analysed. For each case, preoperative VSP-based finite element models incorporating four interface conditions, rigid osseous contact, soft-tissue compliance, initial osteotomy gap, and heterogeneous bone properties, were compared against corresponding postoperative models reconstructed from CT dat

Surgical strategies for neck tumors extending to the skull base and high-lying carotid artery pathologies present significant challenges for surgeons, necessitating deviations from traditional neck surgery approaches. These tactics are adopted to expand the surgical approach. Although the mandibular sagittal split osteotomy (MSSO) has been routinely utilized in maxillofacial surgery, its integration into combined surgical approaches for head and neck pathologies has not been widely explored. We present our experience using MSSO in patients with neck pathology. A retrospective analysis was cond

Surgical treatment of facial asymmetry usually involves 2-jaw orthognathic surgery. But when the size of the mandible differs a great deal between the right and left sides, as in hemimandibular hyperplasia, additional contouring surgery is required. A 20-year-old woman presented with facial asymmetry, showing marked mandibular hyperplasia of the right side. She was treated with the use of 2-jaw surgery with mandibular body osteotomy in conjunction with orthodontic intrusion. Good esthetic outcome and functional occlusion were achieved.

Mandibular third molars are the most common impacted teeth. Mandibular first and second molars do not share the same frequency of occurrence. In rare cases the occlusal surfaces of impacted molars are united by the same follicular space and the roots pointing in opposite direction; these are called kissing molars. In some cases, a supernumerary fourth molar can be seen as unerupted and, in this case, such a supernumerary, deeply impacted fourth molar is seen neighboring kissing molars. The extraction of deeply impacted wisdom molars from the mandible may necessitate excessive bone removal and

To obtain sufficient correction of facial asymmetry by orthognathic surgery, precise and sufficient dental decompensation during preoperative orthodontic treatment is needed. Facial asymmetry often includes complicated 3-dimensional dental compensation in the anterior alveolar region, which can limit the treatment options and lengthen the treatment time. As an alternative, anterior decompensation using segmental osteotomy (ADSO) with the patient under local anesthesia could be a reasonable approach, as it represents an effective, selective, and relatively safe method for correcting lower anter

OBJECTIVES/HYPOTHESIS: Transoral Robotic Surgery (TORS) has revolutionized the surgical treatment of malignant lesions of the oropharyngeal region. Recent studies have shown that this approach is a very safe procedure and can provide favorable clinical and functional outcomes with respect to traditional approaches. However, a small minority of patients who present with lesions amenable to TORS resection may not be candidates due to anatomical access issues. Anatomic features such as a retrognathic mandible, macroglossia, trismus, dentition, and small oral aperture limit the ability to perform
