Molecular testing in lung cancer in the era of precision medicine.

The clinical expectations how pathologists should submit lung cancer diagnosis have changed dramatically. Until mid 90-ties a clear separation between small cell lung carcinoma (SCLC) and non-small cell lung carcinoma (NSCLC) was mostly sufficient. With the invention of antiangiogenic treatment a differentiation between squamous and non-squamous NSCLC was requested. When epidermal growth factor receptor (EGFR) mutation was detected in patients with pulmonary adenocarcinomas and subsequent specific treatment with tyrosine kinase inhibitors (TKIs) was invented, sub-classification of NSCLC and mo

Interventions for muscular dystrophy: molecular medicines entering the clinic.

Muscular dystrophies are individually rare genetic disorders that cause much chronic disability, affecting young children and adults. In the past 20 years, more than 30 genetic types of muscular dystrophy have been defined. During this time, precise diagnosis, genetic counselling, and medical management have improved. These advances in medical practice have occurred while definitive therapies based on an improved knowledge of disease pathogenesis are awaited. A wide range of therapeutic options have been tested in animal models, and some are being tested in clinical trials. Various therapeutic

Melanoma: from diagnosis to immunotherapy - The journey through molecular medicine.

Malignant melanoma is one of the most aggressive skin neoplasms and a major example of progress in modern oncology. The objective of this article is to review the most recent therapeutic data and the transition from diagnosis to modern immunotherapy based on a comprehensive review of the current literature and pivotal clinical trials. The results demonstrate that the profound understanding of the relationship between cancer and the immune system, along with the development of immunotherapies (CTLA-4 and PD-1 inhibitors) and targeted therapies (BRAF/MEK inhibitors), have radically changed the t

Molecular convergence enables precision medicine for pediatric low grade gliomas.

Pediatric low-grade gliomas are the most common CNS tumors in children, comprising heterogeneous WHO grade 1–2 neoplasms. Despite histologic diversity, these tumors show remarkable molecular convergence with near-universal RAS/MAPK pathway activation, typically driven by single oncogenic alterations. The most frequent molecular alterations include BRAF abnormalities (V600E mutations, KIAA1549-BRAF fusions) and FGFR alterations. Most tumors occur sporadically, though some arise in cancer predisposition syndromes, particularly neurofibromatosis type 1 and tuberous sclerosis complex. Clinical pre

New molecular tools for precision medicine in pituitary neuroendocrine tumors.

Precision, personalized, or individualized medicine in pituitary neuroendocrine tumors (PitNETs) has become a major topic in the last few years. It is based on the use of biomarkers that predictively segregate patients and give answers to clinically relevant questions that help us in the individualization of their management. It allows us to make early diagnosis, predict response to medical treatments, predict surgical outcomes and investigate new targets for therapeutic molecules. So far, substantial progress has been made in this field, although there are still not enough precise tools that

Molecular Tumour Board (MTB): From Standard Therapy to Precision Medicine.

Background: In the metastatic setting, cancer patients may not benefit from standard care regimes and their diseases undergo drug resistance due to tumour cell heterogeneity and genomic landscape complexity. In recent years, there have been several attempts to personalise the diagnostic-therapeutic path and to propose novel strategies based on not only histological test results but also on each patient's clinical history and molecular biology. Profiling molecular tests allows physicians to investigate the single tumour genomic landscape and to promote targeted approaches. The Molecular Tumour

Molecular signatures of ovarian diseases: Insights from network medicine perspective.

Dysfunctions and disorders in the ovary lead to a host of diseases including ovarian cancer, ovarian endometriosis, and polycystic ovarian syndrome (PCOS). Understanding the molecular mechanisms behind ovarian diseases is a great challenge. In the present study, we performed a meta-analysis of transcriptome data for ovarian cancer, ovarian endometriosis, and PCOS, and integrated the information gained from statistical analysis with genome-scale biological networks (protein-protein interaction, transcriptional regulatory, and metabolic). Comparative and integrative analyses yielded reporter bio

Considerations of red blood cell molecular testing in transfusion medicine.

The field of transfusion medicine is on the threshold of a paradigm shift, as the technology for genotyping of red blood cell antigens, including US FDA-approved arrays, is now moving into standard practice. Access to cost-efficient, high-resolution genotyping has the potential to increase the quality of care by decreasing the risk for alloimmunization and incompatible transfusions in individuals on long-term blood transfusion protocols, including patient groups with hemoglobinopathies and other chronic diseases. Current and future applications of molecular methods in transfusion medicine and

Precision medicine in myelofibrosis: from molecular profiling to personalized therapy.

Precision medicine in myelofibrosis (MF) has moved beyond confirmation of a driver mutation toward an integrated interpretation of marrow morphology, molecular profile, cytogenetics, clinical phenotype, symptom burden, and transplant fitness. Myelofibrosis is biologically heterogeneous across both primary MF and secondary MF arising after polycythemia vera or essential thrombocythemia, with clinically relevant differences in disease origin, driver distribution, prognostic model validation, phenotype, and outcome. Several prognostic tools and much of the strongest molecular-risk evidence remain

The skeletal connectome: From molecular circuitry to systemic network medicine.

Although classically viewed as a mechanical scaffold, the skeleton is increasingly recognized as a dynamic endocrine and sensory hub that integrates systemic biophysical and biochemical cues. In this review, we rethink skeletal physiology by proposing the 'skeletal connectome,' a network medicine framework that links circuit-level remodeling programs to multiorgan homeostasis. Structurally, we first distill the cellular logic of bone remodeling, emphasizing how osteoblasts, osteoclasts, and osteocytes act as plastic network nodes whose state transitions are constrained by topology, compartment

Evolution of Precision Oncology, Personalized Medicine, and Molecular Tumor Boards.

With multiple molecular targeted therapies available for patients with cancer that correspond to a specific genetic alteration, the selection of the best treatment is essential to ensure therapeutic efficacy. Molecular tumor boards (MTBs) play a key role in this process to deliver personalized medicine to patients with cancer in a multidisciplinary manner. Historically, personalized medicine has been offered to patients with advanced cancer, but the incorporation of molecular targeted therapies and immunotherapy into the perioperative setting requires clinicians to understand the role of the M

Role of Hypoxia Induced by Medicinal Plants; A Revolutionary Era of Cellular and Molecular Herbal Medicine in Neuroblastoma Treatment.

As one of the most common solid pediatric cancers, Neuroblastoma (NBL) accounts for 15% of all of the cancer-related mortalities in infants with increasing incidence all around the world. Despite current therapeutic approaches for NBL (radiotherapies, surgeries, and chemotherapies), these approaches could not be beneficial for all of patients with NBL due to their low effectiveness, and some severe side effects. These challenges lead basic medical scientists and clinical specialists toward an optimal medical interventions for clinical management of NBL. Regardingly, taking molecular and cellul

Salivary microRNAs as new molecular markers in cleft lip and palate: a new frontier in molecular medicine.

MicroRNAs (miRNAs) are endogenous non-coding RNAs of about twenty-two nucleotides that regulate gene expression through post-transcriptional control. The purpose of the present study was to identify and describe the salivary miRNAs in cleft lip and palate (CLP) patients comparing them with a control healthy group. Twelve patients (mean age 11.9 ± 2.42 years; 6M/6F) formed the study group. The control group was created selecting twelve healthy subjects matched for age and sex with study group. We recorded differences in miRNA expression profile between the saliva of CLP patients and the control

Osteoarthritis phenotypes: advancing precision medicine through clinical, structural, and molecular stratification.

PURPOSE: Osteoarthritis (OA) is now understood as a heterogeneous syndrome driven by diverse biological, biomechanical, metabolic, genetic, and molecular mechanisms. This variability explains differences in disease progression and treatment response, challenging the traditional "one-size-fits-all" approach. This review highlights OA phenotyping as a key step toward precision medicine, focusing on clinical, structural, and molecular classifications that inform individualized care. METHODS: A narrative review was conducted using a non-systematic search of major databases and Osteoarthritis Rese

Vitamin D in Cardiovascular Medicine: From Molecular Mechanisms to Clinical Translation.

Vitamin D, a fat-soluble secosteroid traditionally recognized for skeletal health, exerts pleiotropic effects on cardiovascular physiology and disease. Circulating 25-hydroxyvitamin D [25(OH)D], the principal biomarker of vitamin D status, is frequently suboptimal worldwide, particularly in older adults, individuals with darker skin pigmentation, and populations at higher latitudes. Observational studies consistently associate low 25(OH)D concentrations with increased risk of hypertension, atherosclerosis, myocardial infarction, heart failure, arrhythmias, stroke, and cardiovascular mortality.

Gene Therapy for β-Haemoglobinopathies: From Molecular Correction to Curative Medicine.

Background: β-haemoglobinopathies, including sickle cell disease and transfusion-dependent β-thalassaemia, are among the most common monogenic disorders worldwide and represent a major global health burden. Conventional treatments, such as blood transfusions, iron chelation, fetal haemoglobin induction, and allogeneic haematopoietic stem cell transplantation, have improved outcomes but remain limited by treatment-related toxicity, donor availability, and incomplete curative potential. Methods: A narrative literature review was conducted using PubMed up to 2025. Search terms included "sickle ce

Molecular and cellular heterogeneity in breast cancer: challenges for personalized medicine.

Breast cancer is noted for disparate clinical behaviors and patient outcomes, despite common histopathological features at diagnosis. Molecular pathogenesis studies suggest that breast cancer is a collection of diseases with variable molecular underpinnings that modulate therapeutic responses, disease-free intervals, and long-term survival. Traditional therapeutic strategies for individual patients are guided by the expression status of the estrogen and progesterone receptors (ER and PR) and human epidermal growth factor receptor 2 (HER2). Although such methods for clinical classification have

Toward fulfilling the promise of molecular medicine in fragile X syndrome.

Fragile X syndrome (FXS) is the most common inherited form of mental retardation and a leading known cause of autism. It is caused by loss of expression of the fragile X mental retardation protein (FMRP), an RNA-binding protein that negatively regulates protein synthesis. In neurons, multiple lines of evidence suggest that protein synthesis at synapses is triggered by activation of group 1 metabotropic glutamate receptors (Gp1 mGluRs) and that many functional consequences of activating these receptors are altered in the absence of FMRP. These observations have led to the theory that exaggerate
