PURPOSE: To investigate the effects of needle perforation of proximal Molteno tubes on intraluminal pressure in a constant flow system.
MATERIALS AND METHODS: Balanced salt solution was infused at a rate of 2 microL/min through nonexpansile silicon tubing attached to a manometric reservoir, pressure transducer, and an occluded proximal Molteno glaucoma drainage device tube. Avoidance of hypotony in the early post-operative phase following glaucoma drainage device surgery is paramount, if serious complications are to be avoided. Following Molteno device implantation, trabeculectomy, or needling revision of trabeculectomy, six eyes of six chronic glaucoma patients developed a flat anterior chamber without hypotony, ciliochoroidal detachment, pupillary block, or external wound leak. OBJECTIVE: Previous studies have suggested that there are racial differences in the outcome of conventional filtration surgery. This study aims to evaluate the outcome of glaucoma drainage implants in Asian eyes with complicated glaucoma and to determine whether there are racial differences in the results of such implant surgery compared to previous reports in non-Asian patients.
DESIGN: The study design was a retrospective review of all patients with more than 6-months' follow-up after glaucoma implant surgery at the Singapore National Eye Centre between January 1993 and August 1996.

PURPOSE: Endoscopes for use in ophthalmology can be manufactured in dimensions smaller than 0.89 mm.
MATERIALS: Gradient-index lenses that give a higher resolution than fiber bundles can be miniaturized down to 0.35 mm.
RESULTS: We used this new endoscope in 6 eyes of 6 patients during anterior segment surgery. Preliminary clinical experience has demonstrated the usefulness and practical application of this new instrument.
CONCLUSION: The clear views the instrument provides of the ciliary sulcus and ciliary body behind the iris make it useful in secondary posterior chamber intraocular lens

The authors report a case of Listeria monocytogenes endophthalmitis in a patient who had undergone cardiac revascularization surgery and was receiving systemic corticosteroids. The organism was isolated both in the aqueous humor and in blood culture. OBJECTIVE: To investigate the therapeutic effect of a new aqueous humor drainage (HAD) implant made in China for the treatment of refractory glaucoma.
METHODS: The implant was designed by ourselves and manufactured by Beijing Rubber Products Design and Research Institute. We examined a series of 53 consecutive eyes form which Dubroff-style anterior chamber lenses were explanted. The mean time from implantation to explantation was 46.21 months (range six to 99 months). Problems leading to explantation included bullous keratopathy (83%), glaucoma (51%), chronic cystoid macular edema (40%), and iritis (13%). Molteno valves were required in 9% of eyes. PURPOSE: To investigate the lowering-pressure outcome and postoperative complications of trabeculectomy with implantation of silicon rubber slice for intractable glaucoma.
METHODS: Sixty eyes with intractable glaucoma, including 12 eyes with neovascular glaucoma, 14 eyes with aphakic or pseudophakic glaucoma, 4 eyes with congenital glaucoma, 18 eyes with failed filtering surgery glaucoma and 12 eyes with glaucoma secondary to trauma, were performed trabeculectomy with implantation of silicon rubber slice. The postoperative lowering-pressure outcome and complications also were observed.

PURPOSE: To reduce hypotony and shallowing of the anterior chamber after seton implantation.
METHODS: The silicone tube is ligated with an 8-0 nylon suture and a clear corneal graft placed over the tube to facilitate postoperative laser suture lysis.
RESULTS: Laser suture lysis has been performed in 34 cases to date with a follow-up period of up to 26 months. No severe complications occurred after laser suture lysis.
CONCLUSIONS: This technique provides another option to increase drainage postoperatively without the risk of releasable sutures or instrumentation within the eye.

Hypotony or hypertension in the early postoperative period following implantation of a nonvalved seton such as the Molteno tube is a common problem.
