Hypothalamic sarcoidosis: a new cause of morbid obesity.

I have described a 21-year-old man who had biopsy-proven hypothalamic sarcoidosis and polyphagia with a weight increase to 393 lb (178.6 kg) on a 5 ft 4 inch frame during a seven-year follow-up. This morbid obesity appears to be due to sarcoid invasion of the satiety center in the ventral medial nucleus of the hypothalamus. In addition, sarcoid invasion of the supraoptic nucleus produced partial diabetes insipidus. Anterior pituitary deficiency, with complete gonadotropin and growth hormone deficiency, as well as partial ACTH and TSH deficiency, were also present.

Micronutrient deficiencies after gastric bypass for morbid obesity.

Gastric stapling for morbid obesity has been popularized, in part, because of an apparent lack of metabolic sequelae. Of our series of prospectively studied gastric bypass patients, 74 patients have been followed for more than 1 year. Anemia developed in more than one-third of the patients. Nearly two-thirds of the patients developed decreased levels of vitamin B12, and other abnormalities were commonly seen (folate, 38%; iron, 49%; potassium, 56%). While prompt recognition and treatment has prevented development of a clinical deficiency syndrome in most patients, 12 per cent became anemic mos

Radiologic evaluation before gastric bypass for morbid obesity.

During a 4 year period, 69 patients received gastric bypass with stapling and Roux-Y gastrojejunostomy for morbid obesity. The results of 67 preoperative radiologic evaluations of the gastrointestinal tract were analyzed. Of these patients, 17.9 percent (12 of 67) had previous cholecystectomy for cholelithiasis; 14.6 percent (10 of 67) had cholelithiasis found on preoperative evaluation. This gave an overall incidence of gallbladder disease of 32.8 percent. The upper gastrointestinal examination revealed four patients with hiatal hernia, three with reflux, and one with evidence of reflux esoph

Wernicke's disease complicating surgical therapy for morbid obesity.

A case of Wernicke's disease complicating surgical therapy for morbid obesity is presented. Features emphasized include the occurrence of the condition in a nonalcoholic patient and the persistence of neurological signs and symptoms despite prompt therapy. Prophylactic treatment in situations of potential thiamine depletion is essential if iatrogenic Wernicke's disease is to be avoided.

Jejunoileal bypass for morbid obesity. A critical appraisal.

One hundred one carefully screened morbidity obese patients underwent jejunoileal bypass and were followed closely over a mean follow-up period of 32 months. Although there were no operative deaths, three per cent of patients died postoperatively of liver failure or its complications. A fourth patient died of a pulmonary embolus after reoperation, and the fifth patient died cachectic with severe diarrhea after excessive weight loss. Nineteen per cent of the patients required restoration of intestinal continuity (reversal), most for either liver failure or late fluid and electrolyte derangement

Morbid obesity: a comparison between a general population and obesity surgery patients.

Characteristics of morbidity obese adults in the general population and morbidly obese adults presenting for obesity surgery were compared. Black morbidly obese women in the general population were less obese than black women obesity surgery patients, showed less emotional distress, and had fewer abnormal eating behaviours; age and socio-economic status were similar. A greater proportion of morbidly obese surgery patients were white women than would be expected either on the basis of the prevalence of morbid obesity in the general population or on census data. It was hypothesized that black wo

Adverse life experiences among patients with morbid obesity.

BACKGROUND: The association between highly stressful life experiences and morbid obesity is well documented internationally, but this knowledge is not incorporated to any great extent in Norwegian clinical practices. We have studied the reports of previous life stresses from a sample of Norwegian patients under assessment for morbid obesity at a centre where the topic of life experiences was included during the recording of patient histories. MATERIAL AND METHOD: In the summer of 2018, an invitation to participate in the study was distributed to the last 200 patients to have been examined at

Oral copper absorption in men with morbid obesity.

Copper deficiency has been reported in patients with severe obesity, both before and after bariatric surgery, the latter at increased rates. However, little is known about the absorption of copper. Because of differences in copper dynamics between men and women in the limited available data, we evaluated copper absorption in men who were candidates for bariatric surgery. All patients gave informed consent, approved by the University of Pennsylvania Institutional Review Board, to participate in a pharmacokinetics study. After a 12-h fast, body composition was measured using air displacement ple

[Wernicke's encephalopathy following sleeve gastrectomy for morbid obesity].

INTRODUCTION: Bariatric restrictive interventions, as sleeve gastrectomy or gastric banding can cause metabolic complications, especially when vomiting is present, such as thiamine deficiency that can lead to Wernicke's encephalopathy. CASE REPORT: A 31-year-old man with a 47kg/m(2) body mass index presented with Wernicke's encephalopathy, with ophtalmoplegia, nystagmus, ataxia and confusion, followed by a Korsakoff syndrome, occurring two months after a sleeve gastrectomy. MRI showed hyperintense signals on T2 and FLAIR image in both thalamus, periaqueducal area and mamillary bodies. CONCLU

Gastric volvulus after sleeve gastrectomy for morbid obesity.

BACKGROUND: Laparoscopic sleeve gastrectomy in morbid obesity has proved to be a safe and reproducible technique. Sleeve gastrectomy, however, is not free of complications. On the other hand, gastric volvulus is reported in those subjects where, either because of laxity of the gastric anatomical fixations or incorrect position of the stomach, rotation or turning is facilitated. CASE: We report the case of a patient with morbid obesity (Bone mass index / BMI 63 Kg/m2), who in the post-operative period immediately following a sleeve gastrectomy, presented early symptoms of upper gastrointestina

[Complications after gastric-bypass surgery for morbid obesity].

The use of conservative methods of treatment of obesity shows that decrease in body weight is accompanied by improvement of general condition, and a significant positive effect in essential types of metabolism, in particular, helps to reduce the frequency of diabetes type II and other components of the metabolic syndrome. However, to achieve sustained weight loss in patients with morbid obesity, using only conservative treatment is almost impossible. In connection with what clinicians have actively solve this issue, including the surgical treatment. Surgical treatment of obesity are shown in p

Peripheral polyneuropathy after bariatric surgery for morbid obesity.

A patient with peripheral polyneuropathy after bariatric surgery for morbid obesity is reported. She suffered from frequent episodes of vomiting and abdominal pain after surgery. Muscle weakness in her lower limbs developed 5 months later and she experienced difficulty in walking and standing. Wrist drop, foot drop, and marked distal limb muscle atrophy were found bilaterally. Electromyography showed the presence of sensorimotor axonal polyneuropathy. Nutritional deficiencies may play an important role in pathogenesis. This uncommon neurological complication might be due to rapid weight loss a

[Wernicke encephalopathy after subtotal gastrectomy for morbid obesity].

Wernicke's encephalopathy (WE) is one of the potential complications of obesity surgery. It is an acute neuropsychiatric syndrome resulting from thiamine deficiency often associated with repeated vomiting. The classic triad is frequently reported in these patients (optic neuropathy, ataxia and confusion), associated with uncommon features. Cerebral impairment affects the dorsal medial nucleus of the thalamus and the periaqueductal grey area, appearing on MRI, as hyperintense signals on T2, Flair and Diffusion weighted imaging. Early diagnosis and parenteral thiamine are required to decrease mo

Treatment of male infertility secondary to morbid obesity.

BACKGROUND: A 29-year-old man presented to a clinic with infertility and hypogonadism in the setting of morbid obesity. On presentation, he had notable gynecomastia and a low testicular volume. The patient's weight was 154 kg and his height was 168 cm (BMI 54.5 kg/m(2)). Before referral to the clinic, the patient had been treated with testosterone therapy for 4 months for hypogonadism. This treatment had caused his initially low sperm concentration to fall to undetectable levels. INVESTIGATIONS: Measurement of reproductive hormone levels, pituitary MRI, and semen analysis. DIAGNOSIS: Inferti

Laparoscopic total gastric vertical plication in morbid obesity.

BACKGROUND: The aim of this study was to introduce a new technique, total gastric vertical plication (TGVP), as a restrictive operation. It has the same result of weight loss as others with minimal risk of complication and very low cost, especially in developing countries. METHODS: This technique was used by one surgeon in private hospitals during 3 years in Tehran, Iran. Patients were placed in the supine position with a 30-degree reverse Trendelenburg position. Trocars were inserted based on an ergonomic assessment (three 5 mm and one 10 mm). After the release of the greater curvature, cont

[Morbid obesity: laparoscopic gastroplasty prior to plastic surgery].

OBJECTIVES: Reduction of the weight in obese patients is a pre-condition for the aesthetic improvement of obesity or its after effects by plastic surgery. The aim of this study was to evaluate the effectiveness and advantages of gastric banding, using a ring positioned under laparoscopic guidance. PATIENTS AND METHODS: Thirty-nine patients with morbid obesity were operated on. For the first nine patients, gastric restriction was obtained by Mason's traditional technique. In the remaining thirty, a Kuzmak ring was positioned a few centimeters under the cardia, under laparoscopic guidance. The

Liver transplantation after jejunoileal bypass for morbid obesity.

BACKGROUND: Jejunoileal (JI) bypass was developed as a therapy for morbid obesity in the late 1960s but has since been abandoned because of a high rate of complications, including cirrhosis. The need for liver transplantation after JI bypass has been infrequent, with only four previous patients reported in the literature; however, because the time to develop symptomatic end-stage liver disease after JI bypass may be quite long (25 years or more), the incidence of patients who will require liver transplantation may only now be increasing. STUDY DESIGN: We reviewed our experience with JI bypass
