Brain potentials associated with expected and unexpected good and bad outcomes.

The error-related negativity (ERN) is an event-related brain potential observed when subjects receive feedback indicating errors or monetary losses. Evidence suggests that the ERN is larger for unexpected negative feedback. The P300 has also been shown to be enhanced for unexpected feedback, but does not appear to be sensitive to feedback valence. The present study evaluated the role of expectations on the ERN and P300 in two experiments that manipulated the probability of negative feedback (25%, 50%, or 75%) on a trial-by-trial basis in experiment 1, and by varying the frequency of positive a

Exploring the expectation-actuality discrepancy: a systematic review of the impact of preoperative expectations on satisfaction and patient reported outcomes in spinal surgery.

Quality in healthcare is increasingly graded through a patient-centric lens, using reports of satisfaction and self-perceived outcome. Preestablished expectations have been recognized to influence these measures. With this review, we aim to examine the impact of expectations on satisfaction and patient-reported outcomes (PRO) for individuals undergoing elective spine surgery. We systematically searched MEDLINE, EMBASE, CINAHL, and Cochrane Library electronic databases from inception to July 2015 for studies examining the relationship between expectations and satisfaction/PROs in the context of

Outcomes of spontaneous pneumothorax in neonates: treatments vs. expectant management.

BACKGROUND: Neonatal pneumothorax is dependent on severity of symptoms and leak size. Treatment of Pneumothorax with Nitrogen washout is controversial. OBJECTIVE: To compare outcomes of neonates treated for spontaneous pneumothorax (SP) compared with those managed expectantly. METHODS: A retrospective review of medical records of neonates (≥34weeks) diagnosed with SP born between 2011 and 2021. Neonatal characteristics and outcomes were compared between neonates treated for SP with either targeted oxygen therapy (to stabilize saturation ≥93%) or nitrogen washout and those under expectant man

Normal Speech Should be the Expected Outcome in the Adopted Cleft Child.

UNLABELLED: Significantly worse speech outcomes and higher complication rates are reported among internationally adopted cleft patients. We evaluated our cohort to provide more accurate counseling to adoptive parents. METHODS: We reviewed internationally adopted children with unrepaired cleft palate who had 2-flap palatoplasty with radical intravelarveloplasty from 2003 to 2015 in a single-surgeon, consecutive series. RESULTS: Seventy-two children adopted with unrepaired cleft palate were identified, 2 with syndromic association. The average age at palatoplasty was 28.1 months. Meaningful sp

Expectations for Tinnitus Treatment and Outcomes: A Survey Study of Audiologists and Patients.

BACKGROUND: Roughly 10-15% of the general population is affected by tinnitus and this percentage is estimated to rise in future. Because there is currently no cure for tinnitus, treatment is limited and is primarily achieved through management of symptoms and counseling. PURPOSE: This study compared audiologists' and patients' responses to related survey questions about their expectations regarding tinnitus treatment. Two separate surveys were created, one for patients with tinnitus, and one for practicing audiologists who may treat such patients. The surveys included several related question

Distal femur nonunion patients can expect good outcomes.

The purpose of this study is to describe our experience with distal femur nonunions and to report on the functional recovery of patients treated for these injuries. Twenty-two patients with an established distal femur nonunion were identified and followed prospectively. Results were compared with a control group consisting of 18 similar patients who had sustained an acute distal femur fracture. Compared with acute fracture patients, patients with a nonunion were more likely to have had an open fracture at initial injury (p = 0.02) and required a longer time to heal after final surgery (p = 0.0

Social anxiety and cognitive expectancy of aversive outcome in avoidance conditioning.

Fear conditioning studies have shown that social anxiety is associated with enhanced expectancy of aversive outcome. However, the relation between cognitive expectancy and social anxiety has never been tested in avoidance conditioning paradigms. We compared 48 low (LSA) and high socially anxious individuals (HSA) on subjective expectancy of aversive outcome during an avoidance conditioning task. Displays of neutral faces were coupled with an aversive outcome (US): a shout and a shock. Participants could avoid the US by pressing a correct button from a button box. First, HSA showed higher US ex

Fertility outcomes following expectant management of tubal ectopic pregnancy.

OBJECTIVES: To evaluate fertility outcome after the expectant management of tubal ectopic pregnancy. METHODS: Our dedicated early pregnancy database was searched for all women diagnosed with a tubal ectopic pregnancy between January 1999 and June 2003 who were either managed expectantly or underwent a salpingectomy. They were contacted to enquire about their ability to conceive following the ectopic pregnancy and about the outcomes of any subsequent pregnancies. RESULTS: Four hundred and forty-four women had a diagnosis of tubal ectopic pregnancy, and 173 (39%) were successfully contacted. A

Cervicofacial necrotizing fasciitis: can we expect a favourable outcome?

Necrotizing fasciitis of the head and neck is an uncommon, progressive, destructive soft tissue infection of mixed aerobic and anaerobic organisms, having high mortality if left untreated (22 to 100 per cent). This study makes an attempt to analyse various factors and management methods determining the overall prognosis. A retrospective analysis of all cases of necrotizing fasciitis involving the head and neck, with exclusion of those involving the eyelid and the scalp, was undertaken. Various parameters such as demography, aetiology, complications, management and outcome were studied. Males o

Intrahepatic cholestasis of pregnancy: perinatal outcome associated with expectant management.

OBJECTIVE: Our goal was to compare the pregnancy outcomes of patients with intrahepatic cholestasis of pregnancy managed expectantly with antepartum testing with those of other patients who were followed up with a similar testing scheme. STUDY DESIGN: Cases of intrahepatic cholestasis of pregnancy monitored with antepartum testing at our institution over a 7-year period were reviewed. Their pregnancy outcomes were compared with those of control patients followed up with the same testing scheme for a history of stillbirth. Both groups had at least weekly nonstress tests and amniotic fluid asse

Pygmalion in the nursing home. The effects of caregiver expectations on patient outcomes.

Several characteristics of nursing home care can diminish rather than enhance the clinical status of older residents. In view of evidence from other settings that "interpersonal expectancy effects" can influence outcomes in a variety of relations, we conducted a randomized controlled trial to test the effects of caregiver expectations on the clinical status of nursing home residents. Within 2 weeks of admission, 63 older residents at six nursing homes were given a comprehensive assessment of cognitive, functional, and emotional status. Residents were then randomly assigned to a "high-expectanc

Changes in self-efficacy and outcome expectancy as predictors of anxiety outcomes from the CALM study.

BACKGROUND: Although self-efficacy (SE) and outcome expectancy (OE) have been well researched as predictors of outcome, few studies have investigated changes in these variables across treatments. We evaluated changes in OE and SE throughout treatment as predictors of outcomes in a large sample with panic disorder, generalized anxiety disorder, social anxiety disorder, or posttraumatic stress disorder. We hypothesized that increases in SE and OE would predict reductions in anxiety and depression as well as improvement in functioning. METHODS: Participants (mean age = 43.3 years, SD = 13.2, 71.

Dyadic, longitudinal associations among outcome expectation and alliance, and their indirect effects on patient outcome.

Research indicates that patient outcome expectation (OE) correlates with improvement, and that this association may be mediated by better patient-therapist alliances. However, despite OE and alliance being dyadic and dynamic constructs, most research on these direct and indirect associations has assessed these variables from only one dyad member's perspective and at single time points. Addressing these gaps, we used a longitudinal actor-partner interdependence model to first examine OE-alliance associations. Namely, we assessed "actor" effects (relation between each member's OE at 1 session an

How the discrepancy between expectation and outcome influences expectation change in relation to depressive symptoms - Results from four experimental studies.

BACKGROUND: Traditional associative learning models and their clinical applications suggest that the greater the discrepancy between expectations and outcome, the greater the adjustment of expectations. Other frameworks, however, propose a "tipping point", above which the discrepancy is so large that the source of information is considered unreliable, resulting in reduced expectation change. METHODS: To test these competing predictions, we performed four experiments in which participants were provided with five levels of positivity/negativity in two different contexts: social interactions and

Expectations of improvement and attitudes to treatment processes in relation to outcome with four treatment methods for phobic disorders.

One hundred and three agora- or sociophobic women who were not suitable for insight-oriented psychotherapy filled in an inventory with questions pertaining to expectations as to improvement, and attitudes towards goal of treatment and function of the therapist. The patients received one of four randomly assigned types of outpatient treatment for 3 months. All patients received basal therapy in the form of medication, standardized information and self-exposure instructions. One group received this treatment only once a month (B). The others received in addition either therapist-directed prolong

Assault-related mild traumatic brain injury, expectations of injury outcome, and the effect of different perpetrators: A vignette study.

Few studies have examined the effect of varying the assault perpetrator on expectations of outcome from mild TBI. Using a cross sectional between groups design, individuals with no history of mild TBI were randomly allocated to one of two vignette conditions. The vignette depicted a mild TBI with fixed injury parameters and a different assault perpetrator (partner, n = 27; or stranger, n = 27). The participants were instructed to imagine that they had been injured as per the depiction, and then to anticipate the injury consequences 6-months later. An online questionnaire was used including: th
