Pancreatic enzyme supplementation for patients receiving enteral feeds.

BACKGROUND: Pancreatic enzyme supplementation is an important part of management for a number of gastrointestinal conditions. For patients who are unable to swallow pancreatin capsules or granules, enteral feeding tubes can be used to administer the pancreatic enzyme. This presents challenges given the unique format of the pancreatic enzyme supplements, with common problems including tube blockage and loss of the enzyme's effect. METHODS AND RESULTS: A novel technique is described for administration of pancreatic enzyme via feeding tubes. For gastrically placed tubes, this involves opening th

Case report: colonic stricture and fibrosis associated with high-strength pancreatic enzymes in a child with cystic fibrosis.

High-strength pancreatic enzymes have recently been implicated as a cause of colonic strictures in children with cystic fibrosis (CF). We report a 5-year-old boy with CF who had a stricture of the hepatic flexure region with associated narrowing due to submucosal fibrosis of the transverse colon, secondary to high-lipase pancreatin therapy.

Effect of exogenous pancreatic enzymes on gastrointestinal and pancreatic hormone release and gastrointestinal motility.

In clinical practice, exogenous pancreatic enzymes are administered for the treatment of pancreatogenic steatorrhea or with the intention to relieve pain due to chronic pancreatitis. Moreover, a large number of patients take pancreatin (i.e., exogenous pancreatic enzymes) for functional dyspepsia. The effect of exogenous pancreatic enzymes on the enteropancreatic axis is a complex issue. Intraduodenal but not intrajejunal protease activity appears to exert a dose-dependent negative feedback on exocrine pancreatic secretion. Only enzymes with a proteolytic activity but not amylase and lipase ex

Investigation of Probiotic Properties of Lacticaseibacillus casei 4 N-6 Strain Isolated From Cow Milk.

The aim of the present study was to characterize the probiotic potential of Lacticaseibacillus casei 4 N-6 strain isolated from cow's milk. For this purpose, acid, bile salt, pancreatin, pepsin, phenol, and lysozyme tolerance, co-aggregation and auto-aggregation properties, phytase production, antibiotic resistance, and antibacterial properties were analyzed in vitro. The strain had relatively poor acid, bile salt, and pepsin tolerance. However, the strain showed a high pancreatin, lysozyme, and phenol tolerance. In addition, it exhibited moderate co-aggregation with E. coli and good autoaggre

Nutritional value, antibacterial activity, ACE and DPP IV inhibitory of red pomegranate seeds protein and peptides.

One of the challenges related to food and agriculture industries is the production of waste and by-products. In this study, red pomegranate seeds (PS) was selected as a by-product (from oil extraction) for the production of bioactive hydrolysate (with Alcalase, pancreatin, trypsin and pepsin). The composition of essential (~ 23.3%), hydrophobic (~ 32.9%), antioxidant (~ 13.9%) amino acids, and PER index (~ 2.1) especially in hydrolysates by alcalase (H-Al) indicated the nutritional value, antioxidant activity and high digestibility of hydrolysate. Secondary structures and amide regions (I, II

The Advancement of In Vitro Lipolysis: Two-Step Flow-Through Method for the Evaluation of Lipid-Based Drug Delivery Systems.

Objectives: A novel two-step flow-through in vitro lipolysis model was developed for the evaluation of drug release from a self-microemulsifying drug delivery system (SMEDDS). Methods: Firstly, the SMEDDS was dispersed in an acidic medium. Subsequently, the pH was increased, and a lipolytic reaction was immediately initiated, accompanied by medium flow onset. The latter enabled increase of the initial low pH of the medium, improving the physiological relevance of the method by simulating dosage form retainment in the stomach and transfer to the duodenum, which is very important for a weakly ba

Pancreatic Enzyme Supplementation Therapy.

Effective treatment of malabsorption due to severe pancreatic exocrine insufficiency requires delivery of sufficient enzymatic activity into the duodenal lumen simultaneously with meal nutrients. To achieve this, modern therapeutic concepts recommend administration of 25,000 to 40,000 units of lipase per meal using pH-sensitive pancreatin microspheres. In case of treatment failure, dosage should be increased two to three times. If this still is not successful, compliance may be checked by measurement of fecal chymotrypsin, although this is not a standardized procedure. In the compliant patient

An elemental diet fed, enteral or parenteral, does not support growth in young pigs with exocrine pancreatic insufficiency.

BACKGROUND & AIMS: Young individuals with exocrine pancreatic insufficiency (EPI) show growth reduction that can be reversed by dietary pancreatic enzyme supplementation. Here we investigated whether feeding an elemental diet could replace the growth-promoting effect of enzyme supplementation in EPI pigs. METHODS: Weaned pigs with intact pancreas (control) or pancreatic duct-ligated (EPI pigs) were given a commercial pig feed, a fat-enriched diet, or an elemental diet, intragastrically and intravenously, with or without porcine pancreatin (Creon) supplementation for 1 week. RESULTS: Control

A Mini-Review of Diagnostic and Therapeutic Nano-Tools for Pancreatitis.

Pancreatitis is an inflammatory reaction of pancreatic tissue digestion, edema, bleeding and even necrosis caused by activation of pancreatin due to various causes. In particular, patients with severe acute pancreatitis (SAP) often suffer from secondary infection, peritonitis and shock, and have a high mortality rate. Chronic pancreatitis (CP) can cause permanent damage to the pancreas. Due to the innate characteristics, structure and location of the pancreas, there is no effective treatment, only relief of symptoms. Especially, AP is an unpredictable and potentially fatal disease, and the tim

Growth is dependent on the exocrine pancreas function in young weaners but not in growing-finishing pigs.

A correlation between the exocrine pancreatic function and growth has been previously demonstrated in growing pigs but the data are inconsistent. This was investigated by studying the growth performance of pigs with exocrine pancreatic insufficiency (EPI) at different ages and maintained under similar conditions. Twelve 7 week old (10.5+/-1.3 kg) weaners, and twelve 16 week old (43+/-5 kg) growing-finishing pigs were used in the experiments, and 6 pigs from each group were operated and pancreatic duct-ligated. Starting at 3-5 weeks after the operation, when EPI had developed, weekly recordings

Colonic stricture in a boy with cystic fibrosis.

Many problems may arise within the gastrointestinal tract of patients with cystic fibrosis. We report a new cause of subacute intestinal obstruction due to a fibrotic stricture of the ascending colon in a child with cystic fibrosis. Treatment was with a right hemicolectomy. There has been no recurrence after nine months follow-up. Recent similar cases suggest that this new pathology is linked to the use of enteric-coated high-strength pancreatin microspheres.

[Optimization of hydrolysis of protein substrates by yeast proteases from Saccharomyces carlsbergenis].

Optimum conditions for enzymatic hydrolysis of bovine blood in the presence of brewer's yeast Saccharomyces carlsbergensis were studied. In addition to the amount of blood, yeast, sodium citrate, and ethanol in the mixture, the optimum process duration and temperature were determined. The yield of the major end product (blood hydrolysate) increased from 11 to 55%, and the amino acid composition of the end product was brought to a balanced level as a result of hydrolysis optimization and addition of pancreatin to the reaction mixture.

Nano-liposomal delivery of Gracilaria corticata bioactive peptides for improved stability and controlled gastrointestinal release.

This study aimed to develop and evaluate a liposomal nano-delivery system for bioactive peptide fractions (PFs) derived from Gracilaria corticata algae proteins. The peptides were obtained by enzymatic hydrolysis using alcalase, pancreatin, and trypsin. The resulting PF-loaded nanoliposomes were characterized in terms of degree of hydrolysis (DH), particle size, ζ-potential, encapsulation efficiency (EE), and antioxidant capacity (DPPH and ABTS assays). Among the tested enzymes, alcalase produced hydrolysates with the highest DH (33%) and EE (84%). The particle size of nanoliposomes ranged fro

Stability of human salivary extracellular vesicles containing dipeptidyl peptidase IV under simulated gastrointestinal tract conditions.

BACKGROUND: Extracellular vesicles (EVs) have been isolated from various sources, including primary and cultured cell lines and body fluids. Previous studies, including those conducted in our laboratory, have reported the stability of EVs under various storage conditions. METHODS: EVs from human whole saliva were separated via size-exclusion chromatography. To simulate the effects of gastric or intestinal fluids on the stability of EVs, pepsin or pancreatin was added to the samples. Additionally, to determine the effect of bile acids, sodium cholate was added. The samples were then subjected

A method for improving the nutritional value of food proteins: covalent attachment of amino acids.

Casein was modified by use of a series of active N-hydroxy-succinimide esters of amino acids in order to study the effects of new covalently linked hydrophobic or hydrophilic groups on its physical and nutritional properties. Tryptophan was used to determine the best conditions for the chemical reaction and to study the stability of the newly formed amide linkage (isopeptide bond). Casein was also modified with glycine, alanine, methionine, N-acetyl-methionine and aspartic acid. In vitro hydrolysis studies using bovine chymotrypsin, pancreatine and rat bile-pancreatic juice indicated that dige

Advances in the treatment of cystic fibrosis.

Cystic fibrosis is an autosomal recessive disease, characterised by pancreatic insufficiency, abnormal viscous mucus secretions and chronic respiratory tract infections. Treatment is with pancreatin preparations to improve intestinal digestion of food, but the use of high-potency products requires care. Postural drainage and other physiotherapeutic measures are essential to the relief of respiratory obstruction. A new approach to the respiratory problems of cystic fibrosis is dornase alpha, a mucolytic enzyme given by inhalation. Gene therapy may eventually provide the definitive answer to tre
