The management of spine pathology in Papua New Guinea.

This paper outlines the principles of the management of different spinal diseases. In Port Moresby General Hospital between 2004 and 2006 there were 41 spinal injuries, 36 cases of spinal tuberculosis (8 of whom were operated on), 3 non-tuberculous infections and 11 degenerative conditions. The incidence of spinal dysraphism is low in Papua New Guinea with only 5 cases recorded in Port Moresby over the 3-year period. Identification and assessment of spinal pathology were sometimes limited by a lack of radiological investigation and often the resources were not available for optimal treatment.

Galbulimima bark and ethnomedicine in Papua New Guinea.

This article reviews the literature and current information on the use of Galbulimima bark in traditional medicine in Papua New Guinea. Galbulimima bark is used in Papua New Guinea as an analgesic, to treat fever or to get rid of head lice. It is used in divination to produce trance-like states for counteracting malevolent power that is thought to be the cause of a variety of illnesses. Galbulimima bark has also been chewed, or drunk as a decoction, to induce visions and a dream-like state. It is also chewed and then rubbed on the legs before fighting.

AIDS in Papua New Guinea: situation in the Pacific.

There has long been reason to anticipate a major heterosexual epidemic of acquired immunodeficiency syndrome (AIDS) in Papua New Guinea (PNG) and probably in the rest of Melanesia. From the social and behavioural perspectives, Melanesia is strikingly similar to other areas of the world with serious epidemics of AIDS. High levels of other sexually transmitted infections indicate behaviour patterns that would also facilitate transmission of human immunodeficiency virus (HIV) and presence of cofactors for HIV infection. Low levels of male circumcision parallel the situation in other epidemic area

Predisposition to hookworm infection in Papua New Guinea.

Reinfection with hookworm (Necator americanus) following chemotherapy was studied over 8 years in a rural village in Madang Province, Papua New Guinea. Faecal egg counts were performed on up to 202 individuals in July 1988, August 1990 and November 1996; the study population was treated after sampling in 1988 and 1990. Reinfection burdens in 1996 did not differ significantly from pretreatment burdens (in 1988), and were significantly higher than burdens in 1990. However, the prevalence of hookworm infection was significantly lower in 1996 than in either 1988 or 1990. There was significant pred

The repair of frontal encephaloceles in Papua New Guinea.

Frontal encephaloceles are common in Papua New Guinea (PNG). Seventeen cases collected over 7 years are reported. Eleven frontal (sincipital) encephaloceles were repaired successfully via an extracranial approach. One of these patients with a frontonasal encephalocele developed a recurrence following the extracranial approach, which was subsequently repaired intracranially. Three patients with small naso-ethmoidal encephaloceles were repaired intracranially via an extradural approach. The other three cases have not yet had surgical correction. Complex craniofacial surgery which corrects hypert

Injuries caused by pigs in Papua New Guinea.

Pigs are intelligent animals that can be formidable adversaries to humans because of their sharp tusks and their ability to attack swiftly. Domestic and feral pigs have an important role in the ecology of village life in Melanesia. A six-year review of all injuries that were caused by pigs that were referred from the villages in Milne Bay Province, Papua New Guinea, to the Provincial Hospital was completed. Some of the injuries that were seen among the 20 patients who were studied included: three penetrating abdominal injuries with prolapse and strangulation of the intestine; a "sucking" chest

Status of granuloma inguinale in Papua New Guinea.

Granuloma inguinale has been endemic in Papua New Guinea for at least eighty years and recently appears to be increasing, particularly in the urban centres. Most patients were found to be recent migrants to the Port Moresby area with low socioeconomic status and poor personal hygiene. The disease was found more frequently in males. A possible association between carcinoma of the penis and granuloma inguinale was observed. Lesions were chronic, progressive and located mainly in the genital region. Attempts to culture the causative agent were unsuccessful.

Pneumonia research in Papua New Guinea: 1967-1986.

Between 1967 and 1985 research on pneumonia in Papua New Guinea (PNG) was fundamental not only to standard treatments of disease in PNG, but also to the establishment of the World Health Organization's global Program for Control of Acute Respiratory Infections. Pneumonia was the leading cause of death in both population-based and hospital studies. Research that began in 1967 revealed a pattern of disease in adults reminiscent of that seen in industrialized countries in the early 20th century. Streptococcus pneumoniae (pneumococcus) was the predominant causative organism. Pneumococci were comme

Antibiotic-resistant bacterial sepsis in Papua New Guinea.

Infections due to antibiotic-resistant bacteria, especially gram-negative bacteria, are a common cause of child mortality in Papua New Guinea. Antibiotic-resistant bacteria include the enteric gram-negative bacilli, especially Escherichia coli, Klebsiella and Enterobacter, and Haemophilus influenzae type b, a major respiratory tract pathogen and cause of meningitis. Among these bacteria there is now high-level resistance to standard antibiotics, including chloramphenicol, amoxycillin and cotrimoxazole. Reasons behind the increase in antibiotic-resistant bacterial infections are the widespread

Postoperative epidural morphine analgesia in Papua New Guinea.

In order to introduce the technique of epidural narcotics for pain relief in Papua New Guinea, fifty patients were given low doses of epidural morphine and thereupon the quality of analgesia and morbidity evaluated. The lowest effective epidural morphine dose was determined by considering the patient's characteristics and height of surgical incision. Pain relief was provided for three postoperative days in the surgical or gynaecological wards. The quality of analgesia thus provided was excellent in all the patients with no incidence of clinically significant respiratory depression or hypotensi

Puerperal sepsis in Port Moresby, Papua New Guinea.

Because puerperal sepsis is the second most common cause of maternal death in Papua New Guinea some aspects of its aetiology and prevention have been investigated. 91% of 125 women with puerperal fever were found to have post-partum uterine infection. The anaerobes Bacteroides and Peptostreptococcus singly or together, were the commonest causes, while Streptococcus pyogenes was the predominant aerobic species. Antibiotic sensitivity patterns were established, and chloramphenicol was found to be the most active agent against anaerobic and most aerobic strains. Endocervical cultures obtained fro

Occasional parasitic infections of man in Papua New Guinea and Irian Jaya (New Guinea).

Most parasites of man in New Guinea are cosmopolitan species, widely distributed and highly prevalent in the island. Reports of other parasite infections are reviewed. This impoverished fauna includes anthroponotic species normally occurring at low prevalences, e.g. Isospora belli, Dientamoeba fragilis, Trichomonas hominis and a few zoonotic forms with pigs as reservoir hosts, notably Balantidium coli and Entamoeba polecki. Other zoonoses are very few and extremely rare. This may be due to the reduced placental mammal fauna. The potential for the spread of Taenia solium infection is highlighte
