Assessing preoperative bleeding risk using international normalized ratio and activated partial thromboplastin time: a systematic review.

BACKGROUND: The prothrombin time (PT) and activated partial thromboplastin time (APTT) are often considered routine tests in the assessment of perioperative bleeding risk. Physicians continue to order these tests, and major medical societies guidelines exhibit significant heterogeneity regarding the utility of coagulation tests as a general screening tool, despite many advocating for a risk-stratified approach over routine testing. OBJECTIVES: This study aimed to assess for association between preoperative coagulation tests and bleeding events around elective surgery. METHODS: We conducted a

APTTO: a clot waveform analysis-based algorithm for diagnostic triage of prolonged activated partial thromboplastin time.

BACKGROUND: Prolonged activated partial thromboplastin time (APTT) is a common laboratory finding that frequently triggers extensive diagnostic testing and may delay clinical decision-making, particularly in the preoperative setting. However, many patients have no clinically relevant bleeding disorder. Clot waveform analysis (CWA), generated during routine APTT testing, provides dynamic information on coagulation but remains underused in prolonged APTT. OBJECTIVES: To develop and temporally validate a CWA-based algorithm to assist in the evaluation of prolonged APTT. METHODS: In a multicente

Activated partial thromboplastin time levels and coronary artery lesions in Kawasaki disease: A retrospective cohort study.

OBJECTIVE: Kawasaki disease (KD) is an acute systemic inflammation, that affects medium-sized arteries. Coronary artery lesions (CALs) were the most serious complication or sequelae of KD. The intense inflammatory response leads to platelet activation, further exacerbating inflammation, which plays an important role in the pathogenesis of CALs in KD patients. Plus, coagulation factors are closely related to platelet activation. Therefore, we speculate that the activated partial thromboplastin time (APTT), an indicator of coagulation factor function, may be involved in the occurrence of CALs, b

Thromboembolic risk factors in patients undergoing kidney transplant: implication of abnormally short activated partial thromboplastin time.

This study was designed to examine the relationship of short activated partial thromboplastin time (aPTT) and prothrombin time (PT) to the incidence of thromboembolic events, hereditary and acquired coagulation defects associated with an increased risk of thrombosis, or cardiovascular diseases in patients undergoing renal transplantation. The prevalence of these conditions in our patients (n = 436) was 55%. Forty-two percent of the patients had short aPTT or PT. Multivariate analysis revealed that patients with short aPTT have an odds ratio (OR) = 2.15, 95% Confidence Interval (CI) (1.27-3.64)

Automated amidolytic method for evaluating the activated partial thromboplastin time compared with a conventional coagulation method.

We report studies of the validity and clinical application of a new amidolytic method for evaluating the activated partial thromboplastin time (APTT) compared with a conventional clotting method. The results with the two methods were well correlated for normal plasma and plasma from hemophilia patients. Congenital deficiencies of of the intrinsic coagulation pathway other than hypo- and dysfibrinogenemia detected by the amidolytic method agreed with those detected by the clotting APTT. The results with the two methods for plasma from patients under heparin treatment were statistically differen

Pre-analytical storage and reagent-dependent sensitivity as sources of variability in prothrombin time and activated partial thromboplastin time measurements.

BACKGROUND: Prothrombin time (PT) and activated partial thromboplastin time (aPTT) are widely used coagulation assays whose results can vary due to pre-analytical and analytical factors. Understanding these sources of variability is essential for reliable laboratory practice and external quality assessment (EQA). OBJECTIVES: This study evaluates the impact of pre-analytical storage and method-dependent sensitivity on PT and aPTT measurements and provides guidance for EQA programmes and clinical interpretation. METHODS: Plasma samples from 42 healthy volunteers were analysed within 4 h of col

Evaluation of an integrated activated partial thromboplastin time (Cephen LS/Cephen) for the detection of lupus anticoagulant.

INTRODUCTION: It is recommended to use two chronometric assays of different principles for the diagnosis of lupus anticoagulant (LA), consisting in diluted Russell Viper Venom Time (dRVVT) and activated Partial Thromboplastin Time (aPTT). Yet, there are only a few integrated aPTT assays; this study aims to evaluate one of them: Cephen LS/Cephen (Hyphen Biomed). METHOD: 249 samples of patients were included in this study. Normal reference ranges were determined with platelet-poor plasma (PPP) from healthy blood donors. Performances were then evaluated by comparing Cephen LS/Cephen test results

PARTIAL THROMBOPLASTIN TIME TEST WITH KAOLIN. NORMAL RANGE AND MODIFICATIONS FOR THE DIAGNOSIS OF HAEMOPHILIA AND CHRISTMAS DISEASE.

The partial thromboplastin time test provides a convenient and sensitive screening procedure for deficiencies of thromboplastic factors, especially factors VIII and IX. The test is carried out after preincubating the plasma for 10 minutes with kaolin, and Inosithin is used as a platelet substitute. The ;normal range' of the test has been estimated in terms of the differences encountered between random normal plasmas tested in pairs, because individual patients are usually tested against single control subjects. A patient's partial thromboplastin time should be regarded as abnormal if it is mor

Reagent-Dependent Perioperative Activated Partial Thromboplastin Time Prolongation Leading to Diagnosis of Antiphospholipid Syndrome: A Case Report.

Activated partial thromboplastin time (APTT) prolongation in antiphospholipid syndrome (APS) varies according to reagent sensitivity to lupus anticoagulant (LA). Additionally, direct oral anticoagulant (DOAC) therapy can interfere with LA testing, requiring careful consideration of timing. We report a surgical case in which preoperative APTT, despite DOAC cessation, became markedly prolonged after the APTT reagent was replaced with one with higher LA sensitivity, prompting suspicion of APS. Postoperatively, LA positivity was confirmed during interruption of DOAC therapy. This case highlights r

Acquired haemophilia B in Kyasanur Forest disease: a case series of isolated prolonged activated partial thromboplastin time.

BACKGROUND: Kyasanur Forest disease (KFD) is a tick-borne viral haemorrhagic fever in which thrombocytopaenia and transaminitis are well recognized, but the basis of coagulation abnormalities remains incompletely understood. Isolated prolongation of the activated partial thromboplastin time (APTT) with a normal prothrombin time (PT) has been frequently observed, although the mechanism remains unclear. CASE SERIES: We describe nine patients with confirmed KFD who had isolated prolonged APTT with normal PT during the acute phase of illness. The patients ranged in age from 29 to 78 years, and se

A Patient with Severe Fever with Thrombocytopenia Syndrome, Activated Partial Thromboplastin Time Prolongation, and Transient Antiphospholipid Antibodies.

A prolonged activated partial thromboplastin time (APTT) is observed in patients with severe fever with thrombocytopenia syndrome (SFTS) and is one of the risk factors for severe disease. The mechanism underlying a prolonged APTT is largely unknown. The presence of antiphospholipid (aPL) antibodies in various viral infections has been documented but never reported in a patient with SFTS. We herein report the first SFTS patient with APTT prolongation and concurrent transiently positive aPL antibodies (lupus anticoagulants and anticardiolipin antibodies) with no coagulation factor deficiency.

Prolonged Activated Partial Thromboplastin Time after Successful Resuscitation from Cardiac Arrest is Associated with Unfavorable Neurologic Outcome.

Coagulation abnormalities after successful resuscitation from cardiac arrest may be associated with unfavorable neurologic outcome. We investigated a potential association of activated partial thromboplastin time (aPTT) with neurologic outcome in adult cardiac arrest survivors. Therefore, we included all adults ≥18 years of age who suffered a nontraumatic cardiac arrest and had achieved return of spontaneous circulation between January 2013 and December 2018. Patients receiving anticoagulants or thrombolytic therapy and those subjected to extracorporeal membrane oxygenation support were exclud

Correlation between partial thromboplastin time and thromboelastography in adult critically ill patients requiring bivalirudin for extracorporeal membrane oxygenation.

STUDY OBJECTIVE: Thromboelastography (TEG) offers a more dynamic assessment of hemostasis over activated partial thromboplastin time (aPTT). However, the clinical utility of TEG in monitoring bivalirudin during extracorporeal membrane oxygenation (ECMO) remains unknown. The purpose of this study was to evaluate the correlation between aPTT and TEG in adult ECMO patients anticoagulated with bivalirudin. DESIGN: Multicenter, retrospective, cohort study conducted over a 2-year period. SETTING: Two academic university medical centers (Banner University Medical Center) in Phoenix and Tucson, AZ.
