Pattern-Based P53 Immunohistochemistry Predicts Recurrence in Colorectal Liver Metastases Without Complete Pathologic Response After Neoadjuvant Chemotherapy.

BACKGROUND: Aberrant p53 expression may have prognostic significance in colorectal cancer; however, its role in patients with colorectal liver metastases (CRLM) who receive neoadjuvant chemotherapy but do not achieve pathological complete response (pCR) remains unclear. AIMS: We evaluated whether aberrant p53 expression assessed by immunohistochemistry (IHC) predicts recurrence-free survival (RFS) in patients with CRLM receiving neoadjuvant FOLFOXIRI followed by resection without achieving pCR. METHODS AND RESULTS: A prospective cohort study was conducted on 39 patients with CRLM who met the

Novel MRI-based Hyper-Fused Radiomics for Predicting Pathologic Complete Response to Neoadjuvant Therapy in Breast Cancer.

RATIONALE AND OBJECTIVES: To propose a novel MRI-based hyper-fused radiomic approach to predict pathologic complete response (pCR) to neoadjuvant therapy (NAT) in breast cancer (BC). MATERIALS AND METHODS: Pretreatment dynamic contrast-enhanced (DCE) MRI and ultra-multi-b-value (UMB) diffusion-weighted imaging (DWI) data were acquired in BC patients who received NAT followed by surgery at two centers. Hyper-fused radiomic features (RFs) and conventional RFs were extracted from DCE-MRI or UMB-DWI. After feature selection, the following models were built using logistic regression and the retain

Nomograms Integrating Body Composition Metrics Predict Total Pathologic Complete Response after Neoadjuvant Systemic Therapy for Breast Cancer.

PURPOSE: Neoadjuvant systemic therapy (NST) is a systemic treatment for locally advanced or initially unresectable breast cancer before surgery. Patients who achieved total pathologic complete response (tpCR) after NST exhibited significantly better overall prognosis than patients with non-pathological complete response. MATERIALS AND METHODS: This study collected baseline indicators, body composition indicators, and tpCR results of breast cancer patients at the First Affiliated Hospital of Nanjing Medical University. Patients were divided into training set and validation set in a ratio of 7:

Retrospective analysis of multiparametric MRI in predicting complete pathologic response of neo-adjuvant chemotherapy in bladder cancer.

BACKGROUND: Muscle invasive bladder cancer (MIBC) treatment combines systemic therapy and radical cystectomy (RC) or local (chemo-)radiotherapy. Response to systemic therapy is an important outcome predictor but is difficult to assess pre-operatively. METHODS: We analyzed multiparametric MRI (mpMRI) in consecutive MIBC patients receiving cisplatin-based neo-adjuvant chemotherapy at our institution. Two readers, blinded for pathological outcome, independently scored mpMRI before and after 2 and 4 cycles using both a qualitative 3-step method and nacVI-RADS. We analyzed accuracy of mpMRI scores

Efficacy of Mammographic Artificial Intelligence-Based Computer-Aided Detection in Predicting Pathologic Complete Response to Neoadjuvant Chemotherapy.

This study evaluates the potential of an AI-based computer-aided detection (AI-CAD) system in digital mammography for predicting pathologic complete response (pCR) in breast cancer patients after neoadjuvant chemotherapy (NAC). A retrospective analysis of 132 patients who underwent NAC and surgery between January 2020 and December 2022 was performed. Pre- and post-NAC mammograms were analyzed using conventional CAD and AI-CAD systems, with negative exams defined by the absence of marked abnormalities. Two radiologists reviewed mammography, ultrasound, MRI, and diffusion-weighted imaging (DWI).

Toripalimab combined with targeted therapy and chemotherapy achieves pathologic complete response in gastric carcinoma: A case report.

BACKGROUND: Neoadjuvant or perioperative chemotherapy combined with surgery can reduce postoperative recurrence and improve the long-term survival rate of patients with locally advanced resectable gastric carcinoma. Nivolumab combined with chemotherapy has been recommended by the National Comprehensive Cancer Network guidelines as a first-line therapy for advanced gastric carcinoma/ adenocarcinoma of the gastroesophageal junction and serves as the basis for immunotherapy combined with chemotherapy to become a neoadjuvant therapy. Herein, we report a case in which pathologic complete response w

Complete response on MR imaging after neoadjuvant chemotherapy in breast cancer patients: Factors of radiologic-pathologic discordance.

PURPOSE: To evaluate the radiologic and clinicopathologic factors in radiologic-pathologic discordance (false-negative results) in breast cancer patients who demonstrate radiologic complete response (rCR) in MR imaging after neoadjuvant chemotherapy (NAC). METHOD: Our institutional review board approved this retrospective study. We included 209 consecutive patients who showed rCR in MR imaging after NAC. rCR was diagnosed when the original lesion site showed no enhancement. Pathologic CR (pCR) was defined as the complete absence of both invasive cancer and ductal carcinoma in situ in the brea

How Often Is Treatment Effect Identified in Axillary Nodes with a Pathologic Complete Response After Neoadjuvant Chemotherapy?

BACKGROUND: False-negative rates (FNR) of sentinel node biopsy (SNB) after neoadjuvant chemotherapy (NAC) in node-positive (N+) breast cancer patients are <10 % when ≥3 negative SNs are obtained. Marking positive nodes has been suggested to reduce FNR. Identification of treatment effect in the nodes post-NAC is an alternative to decrease FNR. We evaluated the frequency of treatment effect in N+ patients after a pathologic complete response (pCR) with NAC. METHODS: Biopsy-proven N+ patients receiving NAC were identified. Patients with nodal pCR after axillary lymph node dissection (ALND) or SN

Complete pathologic response to neoadjuvant radiotherapy is predictive of oncological outcome in patients with soft tissue sarcoma.

UNLABELLED: We sought to determine if complete pathological necrosis (pathCR) predicts favorable oncological outcome in soft tissue sarcoma (STS) patients receiving pre-operative radiation monotherapy (RT). PATIENTS AND METHODS: We evaluated 30 patients with primary STS treated with neoadjuvant RT followed by definitive resection, from 2000 to 2010 at our institution. We defined ≥ 95% tumor necrosis as pathCR. RESULTS: There were 22 STS of the extremities (73%), 7 of the retroperitoneum (23%), and 1 (4%) of the trunk. The median pathological percentage of tumor necrosis was 35% (range 5-100%

Chemotherapy-induced normalization of FDG uptake by colorectal liver metastases does not usually indicate complete pathologic response.

Dramatic responses are being observed in colorectal cancer liver metastases treated with newer chemotherapeutic regimens. These have been associated with normalization of [(18)F]fluoro-2-deoxy-D-glucose (FDG) uptake (complete metabolic response) on follow-up Positron Emission Tomography with [(18)F]fluoro-2-deoxy-D-glucose (FDG-PET) scans in some patients. It is unclear how often complete metabolic response is indicative of complete tumor destruction. We analyzed a subset of patients who had neoadjuvant chemotherapy for hepatic metastases from colorectal adenocarcinoma. Inclusion criteria were

Rate of pathologic complete response with increased interval between preoperative combined modality therapy and rectal cancer resection.

INTRODUCTION: Recent data suggest a favorable prognosis for rectal cancer patients with a pathologic complete response to preoperative combined modality therapy. Prolongation of the interval between preoperative combined modality therapy and surgery (RT-surgery interval) as a means of increasing pathologic complete response rate has not been fully examined. METHODS: One hundred and fifty-five rectal cancer patients undergoing preoperative pelvic external beam radiation therapy and 5-fluorouracil-based chemotherapy followed by rectal resection were identified. All patients had endorectal ultra

A supplement, not a substitute: Accuracy and completeness of ChatGPT responses for common elbow pathology.

HYPOTHESIS: Large language models (LLMs) like ChatGPT have increasingly been used as online resources for patients with orthopedic conditions. Yet there is a paucity of information assessing the ability of LLMs to accurately and completely answer patient questions. The present study comparatively assessed both ChatGPT 3.5 and GPT-4 responses to frequently asked questions on common elbow pathologies, scoring for accuracy and completeness. It was hypothesized that ChatGPT 3.5 and GPT-4 would demonstrate high levels of accuracy for the specific query asked, but some responses would lack completen

Pathological complete response after chemotherapy in initially unresectable distal cholangiocarcinoma.

Surgical resection is the only curative treatment for cholangiocarcinoma, but it is often diagnosed at advanced stages, making surgical resection infeasible. Recently, the concept of conversion surgery has expanded the indications for surgical treatment, thanks to advancements in both perioperative management and chemotherapy. However, it remains unclear which patients benefit most from this treatment strategy. We present a case of initially unresectable cholangiocarcinoma in which a pathologic complete response was achieved following chemotherapy. A man in his seventies presented with jaundic

UGT2B17 as a predictive biomarker of complete pathological response in HER2 + breast cancer.

BACKGROUND: Pathological complete response (pCR) after neoadjuvant therapy is a robust surrogate marker for long-term outcomes in breast cancer. Despite major advances with targeted therapies, a significant proportion of patients fail to achieve pCR, underscoring the urgent need for reliable biomarkers that can predict treatment response and guide patient stratification. METHODS: We conducted a two-phase study including 53 patients with HER2-positive, hormone receptor–negative breast cancer treated with neoadjuvant chemotherapy plus anti-HER2 agents. Transcriptomic profiling (discovery cohort,

Predicting breast cancer pathological complete response with clinical and imaging data.

INTRODUCTION: Pathological complete response (pCR) is a key prognostic indicator in breast cancer (BC) patients receiving neoadjuvant chemotherapy (NAC). Unimodal prediction models are limited, underscoring the need for multimodal machine learning approaches. METHODS: This retrospective study included 211 BC patients. A radiomics score (Radscore) was developed from multiparametric MRI, and integrated with clinical predictors using machine learning to build three models: clinical, radiomics, and a combined clinical-radiomics model. Their performance was systematically compared. RESULTS: The c

[Complete Pathological Response of Rectal Cancer with Preoperative Chemoradiotherapy-A Case Report].

A 79-year-old man diagnosed with rectal cancer and underwent preoperative chemoradiotherapy. After chemoradiotherapy, the patient underwent abdominoperineal resection. Postoperative pathological examination confirmed pathological complete response. Postoperatively, the patient developed a pelvic abscess due to wound infection from the anal fistula, which required drainage and antibiotic therapy. Treatment options that include a watch and wait approach are required in cases of resectable lower rectal cancer with a high risk of local recurrence that has shrunk after preoperative chemoradiation.

[Preoperative Chemoradiotherapy for Advanced Rectal Cancer Resulting in Pathological Complete Response].

A 73-year-old woman was referred to our institution due to the presence of narrow and bloody stools. On rectal examination, a rectal mass was observed. Colonoscopy revealed a type 2 tumor in the rectum(RbP)that extended to the dentate line. On biopsy, the tumor was diagnosed as tub1/tub2. No enlarged lymph nodes or metastases were noted on CT. On MRI, the tumor did not invade outside the rectum, and was noted to be proximal to the levator ani muscle. The patient was diagnosed with rectal cancer(cT4a, cN0, cM0, cStage Ⅱb). Preoperative chemoradiotherapy(CRT)was performed to preserve the patient

Predicting Factors of Complete Pathological Response in Locally Advanced Rectal Cancer.

Background: Current treatment of choice for locally advanced rectal cancer is neoadjuvant chemoradiotherapy (neo-CRT) followed by surgical resection and adjuvant chemotherapy. Some patients may experience complete pathological response (cPR) after the neoadjuvant treatment. However, the predicting factors are still debated. Methods: In this registry-based retrospective cohort study, 258 patients with locally advanced rectal cancer were included. Patients were categorized into two groups with or without cPR. Logistic regression analysis was recruited to investigate the odds ratio for all indepe
