Performance characteristics of high-frequency percussive ventilation under hyperbaric conditions.

INTRODUCTION: Safe administration of critical care hyperbaric medicine requires specialized equipment and advanced training. Equipment must be tested in order to evaluate function in the hyperbaric environment. High-frequency percussive ventilation (HFPV) has been used in intensive care settings effectively, but it has never been tested in a hyperbaric chamber. METHODS: Following a modified U.S. Navy testing protocol used to evaluate hyperbaric ventilators, we evaluated an HFPV transport ventilator in a multiplace hyperbaric chamber at 1.0, 1.9, and 2.8 atmospheres absolute (ATA). We used a t

Management of localized pneumothoraces after pulmonary resection with intrapulmonary percussive ventilation.

BACKGROUND: Intrapulmonary percussive ventilation (IPV) aims at clearing retained secretions through oscillary vibrations generated by high frequency bursts of gas delivered into the airways at rates between 200 and 300 breaths per minute and at a delivery pressure of 10 to 20 cm water. In addition, IPV can improve recruitment of alveolar units and deliver aerosolized medications. The use of IPV to resolve challenging postlobectomy localized pneumothoraces is hereafter described. METHODS: Between January 2005 and March 2009, four patients with long-term complicated postresectional residual ai

Development of an air bubble curtain to reduce underwater noise of percussive piling.

Underwater bubbles can inhibit sound transmission through water due to density mismatch and concomitant reflection and absorption of sound waves. For the present study, a perforated rubber hose was used to produce a bubble curtain, or screen, around pile-driving activity in 6-8-m depth waters of western Hong Kong. The percussive hammer blow sounds of the pile driver were measured on 2 days at distances of 250, 500, and 1000 m; broadband pulse levels were reduced by 3-5 dB by the bubble curtain. Sound intensities were measured from 100 Hz to 25.6 kHz, and greatest sound reduction by the bubble

Persistent pulmonary consolidation treated with intrapulmonary percussive ventilation: a preliminary report.

Intrapulmonary percussive ventilation (IPV) is a novel form of chest physiotherapy delivered by a percussive pneumatic device (IPV, Percussionaire, Sand Point, ID). There are few published reports about the use of IPV for diseases other than cystic fibrosis. We report our experience with three pediatric patients and one adult patient with persistent pulmonary consolidation refractory to conventional therapies. Three of the four patients had neuromuscular disease; one patient had segmental atelectasis due to aspiration. Three of the four patients showed clinical and radiographic improvement wit

Intranasal ketamine attenuates neuronal damage and behavioral deficits following mild fluid percussion injury.

Mild traumatic brain injury (mTBI) induces neuronal swelling, dendritic distortion, and microvascular damage. Following the initial insult, neurons undergo depolarization, triggering cortical spreading depolarization (SD) that contributes to secondary damage. Evidence indicates that N-methyl-d-aspartate (NMDA) receptor antagonists may counteract calcium imbalance and attenuate SD, thereby offering neuroprotection. In this study, we hypothesized that intranasal ketamine (INK), an NMDA receptor antagonist, could provide therapeutic benefit in mTBI. C57BL/6 mice were subjected to mild lateral flu

A Lateral Fluid Percussion Injury Model for Studying Traumatic Brain Injury in Rats.

Traumatic brain injury (TBI) diagnoses have increased in frequency during the past decade, becoming a silent epidemic. The pathophysiology of TBI involves pathophysiological processes affecting the brain, induced by traumatic biomechanical forces resulting in temporary impairment of neurological function. Preclinical models have been generated to recapitulate the mechanical, neuroinflammatory, and behavioral outcomes observed in the clinical setting. The lateral fluid percussion (LFP) model is the most extensively used and well-characterized model of nonpenetrating and nonischemic TBI. The mod

Increased intestinal permeability in rats subjected to traumatic frontal lobe percussion brain injury.

BACKGROUND: Dysfunction of the gastrointestinal tract is a common occurrence after traumatic brain injury (TBI). We hypothesized that increased intestinal permeability may result from a precisely controlled percussion injury to the exposed brains of anesthetized rats and that such an effect could be assessed in vitro using excised intestinal mucosae mounted in Ussing chambers. METHODS: After craniotomy over the left medial prefrontal cortex on anesthetized rats, neurotrauma was produced using a pneumatically driven impactor on the exposed brain. Control rats were subjected to identical proced

Rapid clinical diagnosis of pulmonary abnormalities in HIV-seropositive patients by auscultatory percussion.

A prospective, blinded study of pulmonary findings in hospitalized patients with HIV infection compared auscultatory percussion (AusP) with conventional percussion (ConP) and conventional auscultation (ConA) using chest radiographs as the gold standard. Sixty-three patients had chest radiographs and were examined by one to three examiners. Seventy of the 126 lungs had radiographic abnormalities (55.6 percent). Auscultatory percussion proved to be the most sensitive of all techniques for each examiner (range, 51.0 to 69.6 percent) for detecting radiographic abnormalities and also had higher lik

Early assessment of neurologic deficits in the fluid percussion model of brain injury.

This study was designed to quantify the early neurologic effects of brain injury elicited by fluid percussion to the dura of cats. Propofol was used for surgical anesthesia because recovery in normal animals from an intravenous infusion was found to be nearly complete within 2 h of cessation and absolutely complete by 4 h. In addition, a cat coma scale (CCS) was developed that reflects normal (CCS, 14) to moribund (CCS, 3) behavior. The CCS values at 6 h were compared with the force of injury expressed in atmospheres (atm), maximum blood pressure change, and gross neuropathology to ascertain w

Benefits of intratracheal and extrathoracic high-frequency percussive ventilation in a model of capnoperitoneum.

Abdominal inflation with CO2 is used to facilitate laparoscopic surgeries, however, providing adequate mechanical ventilation in this scenario is of major importance during anesthesia management. We characterized high-frequency percussive ventilation (HFPV) in protecting from the gas exchange and respiratory mechanical impairments during capnoperitoneum. In addition, we aimed to assess the difference between conventional pressure-controlled mechanical ventilation (CMV) and HFPV modalities generating the high-frequency signal intratracheally (HFPVi) or extrathoracally (HFPVe). High frequency percussive ventilation in burn patients: hemodynamics and gas exchange.

High frequency percussive ventilation (HFPV) is a recent ventilatory mode, which combines conventional cycles with high frequency percussions. HFPV was initially instituted as salvage therapy after acute respiratory failure following smoke inhalation injury achieving in each case a dramatic improvement of blood oxygenation, PaCO(2) and ventilatory pressures. This study investigates the influence of HFPV on hesmodynamics, blood oxygenation and ventilatory parameters in eight stable ICU burn patients requiring artificial ventilatory support during a postoperative period following traumatic injur

Use of High-Frequency Ventilation in the Newborn: Jet, Oscillator, Flow Interruption or Percussion?

In the sentence beginning with "High-frequency ventilation (HFV) maintains functional residual capacity and minimizes ventilator-induced lung injury by delivering subdead-space tidal volumes at supraphysiologic frequencies. Its performance depends on airway size and lung mechanics, making it effective in preterm infants with low compliance. HFV modalities include high-frequency jet ventilation, delivering rapid gas pulses with passive exhalation to improve gas exchange and support lung stabilization, especially in air-leak syndromes; high-frequency oscillatory ventilation, providing bidirectio

Productivity Prediction Model for Horizontal Wells of Shale under Cyclic Conflagration Compression Fracturing Percussion.

A multiscale complex fracture and matrix damage coupled productivity prediction model under cyclic percussion of horizontal wells is established, according to the evolution of matrix permeability and the characteristics of complex fracture seepage after cyclic conflagration compression fracturing. The effects of the conflagration loading rate, cyclic percussion times, horizontal in situ stress difference, seepage interference, and wellbore pressure drop on horizontal well productivity are analyzed. The results show that the loading rate and percussion times are positively correlated with the p

Please unmute your microphone: Comparing the effectiveness of remote versus in-person percussion training.

Although remote music training has its limitations, the use of technology can lower barriers to its accessibility. This exploratory study compared the effects of remote and in-person percussion training on motor performance, performance quality, and students' enjoyment. The training involved the motor aspects of playing legato on percussion instruments. Twenty percussionists received the training either remotely from an instructor using videoconferencing technology or in person from the same instructor who was in the training room. Motor behavior, legato expressivity, performance quality, and

Percussion assisted radiation therapy in Hodgkin lymphoma allows a marked reduction in heart dose.

BACKGROUND AND PURPOSE: Early-stage Hodgkin lymphoma (HL) is a highly curable disease but the treatment can induce late complications many years later. Irradiation of the healthy heart is inevitable during radiation treatment of mediastinal sites. We developed a novel method to induce a prolonged apnea-like state that can help decrease the dose to organs at risk during radiation therapy. We present the results of the first 8 HL patients treated routinely with percussion assisted radiation therapy (PART) in our clinic. MATERIAL AND METHODS: We used a newly developed high-frequency non-invasive
