Digital transformation in community pharmacy management: Pharmacists' readiness for cloud computing systems, implementation concerns, and adoption support.

BACKGROUND: Cloud computing is increasingly positioned as a key enabler of digital transformation in pharmacy practice, offering potential benefits for information access, workflow flexibility, data storage, backup, and service continuity. However, pharmacists' readiness to adopt cloud-enabled pharmacy systems remains insufficiently examined, particularly in community pharmacy settings. OBJECTIVE: This study assessed pharmacists' readiness for cloud computing adoption as part of digital transformation in community pharmacy management, focusing on perceived usefulness, digital-service orientat

Aboriginal and Torres Strait Islander peoples' perspectives on community pharmacists prescribing: a co-designed study protocol.

Background In Australia, medications can be prescribed by medical practitioners, dentists, nurses, and dispensed by pharmacists. Until recently, pharmacists have been limited to prescribing Schedule 2 and 3 medications, and optometrists, podiatrists, and nurse practitioners can prescribe medications under their scope of practice in some areas of Australia. Recently, the New South Wales (NSW) Government initiated a trial where approved pharmacists in NSW and Australian Capital Territory have an expanded scope of practice to prescribe further medications for urinary tract infections, dermatology

Influence of Change of Full-Time Equivalents on Post-prescription Review with Feedback Interventions in an Antimicrobial Stewardship.

Few studies have investigated the influence of more full-time equivalents (FTEs) of infectious disease (ID) pharmacists on the likelihood of a post-prescription review with feedback (PPRF) intervention. This study focused on this in community hospitals before and after the Japanese medical reimbursement system was revised to introduce antimicrobial stewardship (AS) fees. We collected data for two periods: before (April 2017 to March 2018) and after (April 2018 to March 2019) AS fee implementation. The efficacy of the PPRF by the ID pharmacist was assessed based on the usage of broad-spectrum a

Home visit survey on outpatient antibiotic use: Assessing leftover medications and factors influencing appropriate use by community Pharmacists.

This cross-sectional study investigated the prevalence and factors associated with leftover antibiotics in Japanese households through pharmacist home visits. The research, conducted in collaboration with pharmaceutical associations in Kyoto and Hirakata, included 37 cases analyzed from October 2023 to March 2024. Participants had a median age of 81 years and received a median of two antibiotic prescriptions in the past year. Leftover antibiotics were found in 18.9 % of cases, with 5 out of 7 cases resulting from prophylactic prescriptions. Levofloxacin was the most common leftover antibiotic.

Comparison of Type 2 Diabetes Mellitus Management by an Ambulatory Care Pharmacist vs Usual Care in a Medically Underserved Population.

Pharmacists have been shown to improve HbA1c reduction among medically underserved Type 2 diabetes mellitus (T2DM) patients when compared to standard of care. However, there is limited literature exploring the difference in prescribing patterns between these 2 cohorts. This study evaluated the impact of pharmacy-driven T2DM management utilizing a collaborative practice agreement (CPA) compared to usual care at a family medicine clinic by comparing HbA1c reduction and prescribing patterns. This single-center, retrospective chart review identified patients with an HbA1c >9% within a 1-year perio

Impact of Clinical Pharmacist Practitioner-Driven High Opioid Dose Reevaluation in Veterans with Chronic Non-Cancer Pain.

Veterans Health Administration implemented the Opioid Safety Initiative (OSI) in 2013 to promote safe/rational opioid prescribing. West Palm Beach VA Healthcare System (WPBVAHCS) has been an outlier for the percentage of Veterans with chronic non-cancer pain receiving ≥90 mg Morphine Equivalent Daily Dosing (MEDD) in Veterans Integrated Service Networks (VISN) 8 since the 2016 fiscal year. Screening for sleep disorders in community pharmacies--evaluation of a campaign in Switzerland.

BACKGROUND: In 2003 the Swiss federation of pharmacists organized a campaign "sleep disturbances--daytime sleepiness". The goal was to assist pharmacy clients in detecting likely causes of any sleep disturbance or daytime sleepiness through a free of charge screening, and to deliver targeted counselling. For pharmacy practice there are no screening or triage guidelines to assess the severity of sleep and wakefulness disturbances and potential causes for those disturbances. In this paper the outcome of the campaign in terms of feasibility, participation, observed response patterns, sale of over

Antiplatelet therapy in patients with unstable angina and non-ST-segment-elevation myocardial infarction: findings from the CRUSADE national quality improvement initiative.

Evidence-based clinical practice guidelines encapsulate current knowledge to guide health care professionals in the treatment of patients with unstable angina or non-ST-segment-elevation myocardial infarction (NSTEMI), yet adherence to guideline recommendations is suboptimal. Guideline adherence may be improved by quality improvement programs such as the CRUSADE (Can Rapid Risk Stratification of Unstable Angina Patients Suppress Adverse Outcomes with Early Implementation?) National Quality Improvement Initiative of the American College of Cardiology-American Heart Association Guidelines. The C

Clinician attitudes toward referring patients to pharmacists for tobacco cessation services.

BACKGROUND: The role of pharmacists in tobacco cessation has grown substantially in recent years, now including the ability to prescribe medications in many states. Although pharmacists can fill a gap in care by helping patients quit, other clinicians' perceptions regarding referring patients to pharmacists for these services have not been described. OBJECTIVE: To characterize clinicians' current referral patterns to pharmacists for tobacco cessation services, intention to refer in the future, and perceived barriers to and facilitators of referrals. METHODS: A cross-sectional survey was admi

Managing opioids and mitigating opioid risks in patients with cancer: An environmental scan of the attitudes, confidence, and practices of ambulatory, community and hospital pharmacists practicing in Canada.

INTRODUCTION: Canada is in the midst of an overdose crisis. The use of prescription opioids in Canada has increased steadily over the past two decades, with stark increases in opioid-induced respiratory depression and related deaths. Opioids are the mainstay of treatment for cancer-related pain. Patients with cancer are not immune to the risks associated with opioid use but are underrepresented in available literature outlining risk mitigation strategies. Pharmacists are ideally placed to employ opioid risk mitigation practices to support safe and effective opioid use for patients with cancer-

Development and validation of a machine learning model to improve precision prediction for irrational prescriptions in orthopedic perioperative patients.

OBJECTIVE: Our objective was to develop a machine learning model capable of predicting irrational medical prescriptions precisely within orthopedic perioperative patients. METHODS: A dataset comprising 3047 instances of suspected irrational medication prescriptions was collected from a sample of 1318 orthopedic perioperative patients from April 2019 to March 2022. Four machine learning models were employed to forecast irrational prescriptions, following which, the performance of each model was meticulously assessed. Subsequently, a thorough variable importance analysis was conducted on the mo

Pharmacists' Attitudes to and Perceptions of Pseudoscience: How Pseudoscience Operates in Health and Social Communication.

Given the expansion of pseudoscience, there is a need to understand its mechanisms of diffusion. Our aim was to evaluate how pseudoscience operates among pharmacists. We performed 29 semi-structured interviews to assess the stance of pharmacists regarding pseudoscience. Interview data were analyzed qualitatively to seek common themes. Our results indicate that although pharmacists were broadly opposed to more extreme pseudoscientific practices, some attitudes were detected that may contribute to pseudoscience acceptance. We identified some of the processes by means of which pseudoscience bound
