Antimicrobial Resistance Surveillance and New Drug Development.

Surveillance represents an important informational tool for planning actions to monitor emerging antimicrobial resistance. Antimicrobial resistance surveillance (ARS) programs may have many different designs and can be grouped in 2 major categories based on their main objectives: (1) public health ARS programs and (2) industry-sponsored/product-oriented ARS programs. In general, public health ARS programs predominantly focus on health care and infection control, whereas industry ARS programs focus on an investigational or recently approved molecule(s). We reviewed the main characteristics of i

From crisis to care: exploring the resilience of pediatric urologists in tackling complex urological challenges in a resource-limited country during volunteer campaigns: a qualitative study.

Surgeons operating in resource-limited settings encounter unique challenges due to the scarcity of materials and resources. Complex urological disorders (CUDs) such as bladder exstrophy, cloacal exstrophy, and posterior urethral valves, prevalent in these settings, often lead to varying surgical outcomes. This study aims to understand the experiences of surgeons treating pediatric patients with varying CUDs in a setting where resources are scarce through a qualitative phenomenological approach. We conducted six in-depth interviews with six pediatric urologists and surgeons who participated in

Lessons learned from a multi-data source research collaboration: The mirabegron post-authorization safety study program.

BACKGROUND: Many factors contribute to developing and conducting a successful multi-data source, non-interventional, post-authorization safety study (NI-PASS) for submission to multiple health authorities. Such studies are often large undertakings; evaluating and sharing lessons learned can provide useful insights to others considering similar studies. OBJECTIVES: We discuss challenges and key methodological and organizational factors that led to the delivery of a successful post-marketing requirement (PMR)/PASS program investigating the risk of cardiovascular and cancer events among users of

APOL1 Long-term Kidney Transplantation Outcomes Network (APOLLO): Design and Rationale.

INTRODUCTION: Much of the higher risk for end-stage kidney disease (ESKD) in African American individuals relates to ancestry-specific variation in the apolipoprotein L1 gene (APOL1). Relative to kidneys from European American deceased-donors, kidneys from African American deceased-donors have shorter allograft survival and African American living-kidney donors more often develop ESKD. The National Institutes of Health (NIH)-sponsored APOL1 Long-term Kidney Transplantation Outcomes Network (APOLLO) is prospectively assessing kidney allograft survival from donors with recent African ancestry ba

Tracking the pharmaceutical pipeline: clinical trials and global disease burden.

Aggregate data about pharmaceutical research and development (R&D) tend to examine Phase III trials. Hence, there are few published data about investigational drugs in earlier phases of clinical development that might fail. It is also unclear how well R&D corresponds to disease burden. We track the pharmaceutical pipeline using data from industry publications that provide otherwise unreported information about industry-sponsored clinical trials. The sample includes 2,477 unique drug entities in 4,182 clinical trials. The majority of drugs targeted neoplasms (26.20%), neurological diseases/dise

Venous thromboembolism (VTE) prevention in the trauma patient: A national survey of practice.

AIM: Due to complex injury patterns, the safe timing of venous thromboembolism (VTE) prophylaxis in trauma patients is challenging. The purpose of this study was to characterize current thromboprophylaxis practice in trauma patients amongst trauma providers in Ireland. METHODS: We conducted a national, cross-sectional survey of trauma providers. The survey was sponsored by the Irish Network for VTE Research (INViTE) and disseminated through the national trauma and VTE office. The survey was a 35-item questionnaire and collected information regarding non-pharmacologic prophylaxis practice, pre

Physician-reported reasons for limited follow-up of patients with a positive fecal occult blood test screening result.

OBJECTIVE: Fecal occult blood testing (FOBT) screening can reduce colorectal cancer (CRC) mortality when patients with an abnormal result [FOBT(+)] undergo a complete diagnostic evaluation (colonoscopy or double-contrast barium enema with or without flexible sigmoidoscopy). The aim of this study was to determine common reasons for nonperformance of a complete diagnostic evaluation. METHODS: We identified 544 FOBT(+) patients, aged 50 yr or older, who had participated in a managed care organization-sponsored CRC screening program. The performance of a complete diagnostic evaluation was determi

The new classifications of ovarian, fallopian tube, and primary peritoneal cancer and their clinical implications.

The roles of histologic characterization and staging are to provide reproducible metrics for cancer classification with which to direct the most appropriate clinical care and to yield the most stable reliable system to allow both prospective and retrospective data analysis. Both the histologic and staging classifications of malignant ovarian/tubal/peritoneal cancers have recently changed. The World Health Organization sponsored a review and reclassification of the pathology of cancers of the ovaries, fallopian tubes, and peritoneum, and published these updates in 2014. In so doing, they codifi

Strategies and endpoints of antifibrotic drug trials: Summary and recommendations from the AASLD Emerging Trends Conference, Chicago, June 2014.

There is an urgent need to develop antifibrotic therapies for chronic liver disease, and clarify which endpoints in antifibrotic trials will be acceptable to regulatory agencies. The American Association for the Study of Liver Diseases sponsored an endpoints conference to help accelerate the efficient testing of antifibrotic agents and develop recommendations on clinical trial design for liver fibrosis. In this review, we summarize the salient and novel elements of this conference and provide directions for future clinical trial design. The article follows the structure of the conference and i

The myelodysplastic/myeloproliferative neoplasms: myeloproliferative diseases with dysplastic features.

The 2001 World Health Organization (WHO)-sponsored classification of hematopoietic tumors has, for the first time, clearly defined a group of rare myeloid neoplasms termed myelodysplastic/myeloproliferative diseases (MDS/MPDs). This group includes three main entities, chronic myelomonocytic leukemia, atypical chronic myeloid leukemia and juvenile myelomonocytic leukemia, and also several less well defined, 'unclassifiable' disorders with MDS/MPN-like features. In the upcoming fourth edition of the WHO fascicle, due out later this year, the term 'MPD' is replaced by 'myeloproliferative neoplasm

Implementation of an emergency response protocol for overseas surgical outreach initiatives.

BACKGROUND: Many health organizations sponsor overseas surgical outreach initiatives, yet none has published a standardized protocol to prevent and manage unforeseen emergencies. Surgeons, anesthesiologists, nurses, and administrators-working together on a brief overseas humanitarian initiative-benefit from education and training to maximize their collective emergency responsiveness. This article outlines the emergency response protocol instituted by the Global Smile Foundation, a 501(c)(3) nonprofit global outreach organization providing comprehensive cleft care for the past 25 years. Mapping the evolving trends of robotic surgery in gastric and colorectal cancer: a clinical trial landscape analysis.

Robot-assisted surgery provides important support for the treatment of gastric cancer (GC) and colorectal cancer (CRC) with its minimally invasive advantages. Through a systematic analysis of 188 clinical trials registered on ClinicalTrials.gov, this study aims to synthesize the research landscape in the field, identify evidence gaps, and provide a basis for optimizing clinical practice and guiding the design of future clinical trials. The analysis of 71 interventional trials with robotic surgery as the core intervention shows that the research activity has increased significantly after 2020.

Obtaining and managing data sets for individual participant data meta-analysis: scoping review and practical guide.

BACKGROUND: Shifts in data sharing policy have increased researchers' access to individual participant data (IPD) from clinical studies. Simultaneously the number of IPD meta-analyses (IPDMAs) is increasing. However, rates of data retrieval have not improved. Our goal was to describe the challenges of retrieving IPD for an IPDMA and provide practical guidance on obtaining and managing datasets based on a review of the literature and practical examples and observations. METHODS: We systematically searched MEDLINE, Embase, and the Cochrane Library, until January 2019, to identify publications f
