Emergency rigid bronchoscopy in two lower social class patients with mirror like complete pulmonary malignant atelectasis.

Several factors as cultural factors and social class other than biological and genetic factor can affect symptom perception in patients with malignant airway obstruction. Poor perception of dyspnoea can result in the delayed seeking of medical care so increase access to intensive care due to impeding respiratory failure. In patients issued from malignant airway obstruction, therapeutic bronchoscopy procedure can not affect the endotracheal extubation although immediate airway patency can be obtained. We reported the outcome of two patients from lower social classes admitted in intensive care a

POC-LUS guiding pleural puncture drainage to treat neonatal pulmonary atelectasis caused by congenital massive effusion.

Point of care lung ultrasound (POC-LUS) has played important roles in diagnosing neonatal lung diseases and assisting in their treatment. A newborn infant with severe respiratory distress diagnosed as pulmonary atelectasis caused by congenital massive pleural effusion, whose consolidated lung recruitment after pleural puncture drainage under POC-LUS guidance. Lung ultrasound can be performed easily and timely at bed-side with free of radiation exposure, thus it should be used extensively in the neonatal department.

Migrating atelectasis in Werdnig-Hoffmann disease: pulmonary manifestations in two cases of spinal muscular atrophy type 1.

Spinal muscular atrophy (SMA) or Werdnig-Hoffmann disease is the second most common neuromuscular disease, with 25% of cases presenting in infancy. Deletions in the survival motor neuron gene are believed responsible for autosomal-recessive SMA. SMA affects about 1 in 10,000 births. Symptomatic newborns have severe hypotonia, may have respiratory distress, may be unable to feed, and rapidly progress to death early in infancy. This paper describes another early pulmonary manifestation of SMA, i.e., migrating or rotating atelectasis, in 2 patients with infantile SMA. Migrating or rotating atelec

Lung ultrasound: A useful additional tool in clinician's hands to identify pulmonary atelectasis in children with neuromuscular disease.

INTRODUCTION AND OBJECTIVES: Patients with neuromuscular disease (NMD) are often exposed to ionizing radiations which could be reduced if a noninvasive and reliable diagnostic method is identified. The major aim of this study was to compare the use of chest X-ray (CXR) with lung ultrasound (LUS) in pediatric patients with NMD, to identify pulmonary atelectasis (PA). MATERIALS AND METHODS: A prospective study was conducted on children affected by NMD. In all patients who underwent CXR, a LUS was also performed and results compared for the assessment of PA. RESULTS: Forty children affected by

Hypoxemia with atelectasis during clazosentan therapy after subarachnoid hemorrhage: a potential effect of suppressed hypoxic pulmonary vasoconstriction.

Clazosentan, a selective endothelin-A (ET-A) receptor antagonist, is used to prevent cerebral vasospasm after aneurysmal subarachnoid hemorrhage (SAH). Although pulmonary edema and fluid retention are recognized adverse effects, hypoxemia associated with atelectasis has not been described. Endothelin-1 contributes to hypoxic pulmonary vasoconstriction (HPV), which preserves ventilation-perfusion (V/Q) matching by diverting blood flow from poorly ventilated regions; pharmacologic ET-A blockade may attenuate HPV, increasing intrapulmonary shunt and impairing oxygenation. We report a man in his 6

High prevalence of upper lung field pulmonary fibrosis radiologically consistent with pleuroparenchymal fibroelastosis in patients with round atelectasis.

BACKGROUND: Upper-lung field pulmonary fibrosis (upper-PF), radiologically consistent with pleuroparenchymal fibroelastosis (PPFE), was reported to develop in patients with a history of asbestos exposure and tuberculous pleurisy, indicating that chronic pleuritis is correlated with upper-PF development. Round atelectasis reportedly emerges after chronic pleuritis. This study aimed to clarify the association between round atelectasis and upper-PF. METHODS: We examined the radiological reports of all consecutive patients with round atelectasis between 2006 and 2018 and investigated the incidenc

Unilateral Pulmonary Contusion after Cardiopulmonary Resuscitation Complicated by Delayed Obstructive Atelectasis: A Case Report and Review of the Literature.

We present a previously healthy 57-year-old man who had a sudden cardiac arrest intraoperatively and developed unilateral pulmonary contusion shortly after cardiopulmonary resuscitation. He was gradually improved by supportive management. But, several days later he had worsening of respiratory symptoms and obstructive atelectasis was proven by bronchoscope finding of thick mucus plugging left main bronchus. These are rarely recognized but important post-resuscitation complications. Early detection is crucial to improve outcome. How to cite this article: Klumusuk P, Pirompanich P. Unilateral Pu

The Usefulness of Continuous Respiratory Sound Monitoring for the Detection of Pulmonary Atelectasis in a Ventilated Extremely Low Birth Weight Infant.

The assessment of auscultation using a stethoscope is unsuitable for continuous monitoring. Therefore, we developed a novel acoustic monitoring system that continuously, objectively, and visually evaluates respiratory sounds. In this report, we assess the usefulness of our revised system in a ventilated extremely low birth weight infant (ELBWI) for the diagnosis of pulmonary atelectasis and evaluation of treatment by lung lavage. A female infant was born at 24 weeks of age with a birth weight of 636 g after emergency cesarean section. The patient received invasive mechanical ventilation immedi

Efficacy analysis of the lung recruitment maneuver in correcting pulmonary atelectasis in neurological intensive care unit-a retrospective study.

BACKGROUND: Atelectasis after supratentorial craniotomy is common. It can lead to the decrease of arterial partial pressure of oxygen (PaO2) in patients with neurosurgical intensive care units (NICU), and the recovery of neurological function is more and more difficult. However, due to the particularity of maintaining the stability of intracranial pressure (ICP), there are few reports on effective ways to alleviate atelectasis and improve oxygenation in patients with NICU effectively. METHODS: A retrospective analysis was conducted to analyze the clinical data of patients with atelectasis who

Total unilateral pulmonary collapse secondary to allergic bronchopulmonary aspergillosis: a case series of an unusual cause of complete atelectasis.

BACKGROUND: Allergic bronchopulmonary aspergillosis (ABPA) is a bronchopulmonary disease caused by a complex hypersensitivity to Aspergillus and is usually associated with underlying respiratory diseases such as asthma or cystic fibrosis. Mucus plugging can lead to segmental or lobar atelectasis, but complete lung atelectasis has been exceptionally reported in the literature, making it difficult to diagnose. The diagnosis of ABPA may however be suggested in patients without known predisposing respiratory disorder, even in the absence of other relevant radiographic findings. CASE PRESENTATION:

[Animal experiments on particulars of the time factor in the development and solving of pulmonary atelectasis caused by foreign body (author's transl)].

In literature are only older publications about the temporal beginning of the acute atelectasis after obstructive foreign body occlusion of the bronchial system which are operated at the open thorax. For this reason experiments on animals were made to find out the earliest beginning of an atelectasis by a simple bronchial occlusion as an imitation of a natural process of a foreign body aspiration. The alterations in each single phase of development showed regularly based on serial radiographies and by the autopsy macroscopic and microscopic that about 2 or 3 h after aspiration of an obstructiv

Intrapulmonary percussive ventilation for obstructive atelectasis caused by bronchial cast-like clots after pulmonary hemorrhage in a child with congenital heart disease.

Pulmonary hemorrhage may cause persistent airway obstruction due to retained intrapulmonary blood and clot material, resulting in obstructive atelectasis that may be difficult to resolve with conventional airway-clearance strategies. We report the case of a 3-year-old boy with congenital heart disease who developed extensive obstructive atelectasis associated with bronchial cast-like clots after right pulmonary hemorrhage during angioplasty. Chest physiotherapy, postural drainage, prone positioning, and repeated bronchoscopic airway clearance did not improve aeration. Intrapulmonary percussive

Effect of one-lung ventilation in children undergoing lateral thoracotomy cardiac surgery with cardiopulmonary bypass on postoperative atelectasis and postoperative pulmonary complications.

BACKGROUND: Right lateral thoracotomy is increasingly used because of its cosmetic benefits, shorter hospital stays, rapid return to full activity, and ease of reoperation in pediatric patients with uncomplicated congenital heart disease. Currently, one-lung ventilation (OLV) is used in these children to facilitate surgical exposure. We aimed to assess the effect of OLV on postoperative outcomes. METHODS: Children aged 6 months to 6 years undergoing right lateral thoracotomy cardiac surgery with cardiopulmonary bypass (CPB) were randomized into an OLV group or a control group. Bedside critical ultrasound as a key to the diagnosis of obstructive atelectasis complicated with acute cor pulmonale and differentiation from pulmonary embolism: A case report.

Acute attack of dyspnea may be combined with acute cor pulmonale (ACP). Rapid and accurate identification of the etiology of ACP is the key to its diagnosis and treatment. Echocardiography is a better imaging tool in the assessment of right ventricular function. Under the guidance of the theory of cardiopulmonary interaction, ultrasonography can detect lung lesions, which causes ACP. We report the case of a 67-year-old man who received mechanical ventilation for acute respiratory failure. Right ventricular dysfunction was detected by echocardiography. Lung ultrasound showed a high risk of pulm

[Pulmonary atelectasis in patients with neurological or muscular disease; gravity-related lung compression by the heart and intra-abdominal organs on persistent supine position].

We report 10 cases of pulmonary atelectasis diagnosed by chest computed tomography in patients with neurological or muscular disease. Atelectasis was frequently seen in hypotonic patients who could not roll over on their own. The atelectases located mostly in the dorsal bronchopulmonary segments, adjacent to the heart or diaphragm. Atelectasis diminished in two patients after they became able to roll themselves over. Gravity-related lung compression by the heart and intra-abdominal organs on persistent supine position can cause pulmonary atelectasis in patients with neurological or muscular di
