Fiducial markers implantation for prostate image-guided radiotherapy: a report on the transperineal approach.

INTRODUCTION: In the external beam prostate cancer radiation therapy, daily gland displacement could lead to a target missing. The use of intra-prostatic gold fiducial markers for daily prostate position verification and correction before and during treatment delivery (image-guided radiotherapy, IGRT) is widely used in the radiation therapy centers to accurately target the prostate. Usually, the fiducial markers are implanted through the rectum, with complications such as infections and rectal bleeding. We report our experience in prostate fiducial markers implantation through a transperineal

Compared with intensity-modulated radiotherapy, image-guided radiotherapy reduces severity of acute radiation-induced skin toxicity during radiotherapy in patients with breast cancer.

Radiotherapy (RT) is an effective treatment for breast cancer. The side effects of breast irradiation, including skin toxicity in the irradiation field, cause considerable discomfort. This study compared the severity of skin toxicity caused by image-guided RT (IGRT) and intensity-modulated RT (IMRT) combined with an electronic portal imaging device (EPID) in breast cancer. This study retrospectively analyzed 458 patients with breast cancer who had received RT. The patients were divided into two groups: 302 and 156 patients in the IMRT and IGRT groups. In the IGRT group, 8 and 148 patients had

Daily image-guided carbon ion stereotactic radiotherapy of hepatocellular carcinoma: paving the way for MR-guided online adaptive treatment.

BACKGROUND: We aim to quantify the dosimetric benefits of daily online adaptive (DOA) carbon ion stereotactic radiotherapy (CISR) in hepatocellular carcinoma (HCC). PATIENTS AND METHODS: 13 patients received 4 × 10.5 Gy (RBE-weighted dose) CISR to 14 HCC lesions in challenging locations (N = 10: liver dome, N = 3: < 1 cm from intestines). All patients underwent daily in-room CT, with forward calculation of the baseline plan. Additionally, four patients were shuttled to a 1.5 Tesla MR scanner in treatment position for MRI simulation. Based on visual review of all data, the physician decided to

Assessing the quality of deformable image registration in magnetic resonance image guided carbon ion liver radiotherapy with an anthropomorphic phantom.

BACKGROUND AND PURPOSE: Carbon ion radiotherapy (CIRT) for liver patients is usually administrated in four high-dose fractions, requiring high geometrical precision. Work is underway to implement online magnetic resonance (MR)-guided adaptive workflows for this indication. For the generation of a daily computed tomography (dCT), deformable image registration (DIR) can be used. Here, a study on a deformable anthropomorphic abdomen phantom was conducted to assess the accuracy of MR-to-CT DIR. MATERIALS AND METHODS: We applied eight different static compressions on the phantom to induce deformat

Monte Carlo-based optimization of CTV-to-PTV margins for image-guided VMAT prostate radiotherapy.

BACKGROUND: The van Herk margin formula, derived for 3-D conformal radiotherapy with a uniform 3.2 mm dose penumbra and no intrinsic dose buffer, remains widely used to design CTV-to-PTV expansions in contemporary image-guided prostate radiotherapy. However, coplanar VMAT techniques often feature broader penumbrae and explicit PTV-isodose clearances, potentially violating the assumptions underlying the original formulation and leading to overly conservative margins. PURPOSE: We investigated whether the widely adopted van Herk margin formula overestimates clinical target volume (CTV)-to-planni

Deep learning-enhanced image registration for accelerating daily adaptive magnetic resonance imaging-guided prostate radiotherapy.

BACKGROUND AND PURPOSE: Daily auto-contouring remains a workflow bottleneck in magnetic resonance-guided adaptive prostate radiotherapy (MRgRT). This study proposes and clinically validates a novel deep learning-enhanced deformable image registration (DIR) solution to accelerate this critical step. MATERIALS AND METHODS: A hybrid framework combining a 3D nnU-Net segmenting bladder/rectum on planning/daily MRI with an in-house DIR algorithm was implemented for 5-fraction prostate MRgRT ( 5 × 7.25 Gy) on an MR-Linac. The DIR uses nnU-Net contours to propagate target and organs-of-interest stru

Image-guided targeting of mitochondrial metabolism sensitizes pediatric malignant rhabdoid tumors to low-dose radiotherapy.

Tumor hypoxia leads to radioresistance and markedly worse clinical outcomes for pediatric malignant rhabdoid tumors (MRTs). Our transcriptomics and bioenergetic profiling data reveal that mitochondrial oxidative phosphorylation is a metabolic vulnerability of MRT and can be exploited to overcome consumptive hypoxia by repurposing an FDA-approved antimalarial drug, atovaquone (AVO). We then establish the utility of oxygen-enhanced-multispectral optoacoustic tomography, a label-free, ionizing radiation-free imaging modality, to visualize and quantify spatiotemporal changes in tumor hypoxia in re

Experimental investigation of real-time 3D beam's eye view image-guided radiotherapy for prostate SBRT.

BACKGROUND: Real-time image-guided radiotherapy (IGRT) is critical for accurate dose delivery during stereotactic body radiotherapy (SBRT). Beam's eye view (BEV) imaging offers the unique advantage of reporting motion in the most dosimetrically relevant frame of reference without additional imaging dose. However, its clinical use is limited by poor contrast-to-noise ratio and marker occlusion by treatment beam apertures. Deep learning enables fast identification of indistinct marker features even in low contrast images, facilitating real-time BEV-IGRT. To support integration with standard-equi

Accuracy of automatic structure propagation for daily magnetic resonance image-guided head and neck radiotherapy.

BACKGROUND AND PURPOSE: Deformable image registration (DIR) and contour propagation are used in daily online adaptation for hybrid MRI linac (MRL) treatments. The accuracy of the propagated contours may vary depending on the chosen workflow (WF), affecting the amount of required manual corrections. This study investigated the impact of three different WFs of contour propagations produced by a clinical treatment planning system for a high-field MRL on head and neck cancer patients. METHODS: Seventeen patients referred for curative radiotherapy for oropharyngeal cancer underwent standard CT-bas

CBCT-based synthetic CT image generation using a diffusion model for CBCT-guided lung radiotherapy.

BACKGROUND: Although cone beam computed tomography (CBCT) has lower resolution compared to planning CTs (pCT), its lower dose, higher high-contrast resolution, and shorter scanning time support its widespread use in clinical applications, especially in ensuring accurate patient positioning during the image-guided radiation therapy (IGRT) process. PURPOSE: While CBCT is critical to IGRT, CBCT image quality can be compromised by severe stripe and scattering artifacts. Tumor movement secondary to respiratory motion also decreases CBCT resolution. In order to improve the image quality of CBCT, we

Sequential monoscopic image-guided motion compensation in tomotherapy stereotactic body radiotherapy (SBRT) for prostate cancer.

PURPOSE: To manage intra-fractional motions, recent developments in tomotherapy enable a unique capability of adjusting MLC/jaw to track the moving target based on the intra-fractional motions detected by sequential monoscopic imaging. In this study, we evaluated the effectiveness of motion compensation with a realistic imaging rate for prostate stereotactic body radiotherapy (SBRT). The obtained results will guide optimizing treatment parameters and image-guided radiation therapy (IGRT) in tomotherapy using this approach. METHODS: Ten retrospective prostate cases with actual prostate motion

Degradable multifunctional gold-liposomes as an all-in-one theranostic platform for image-guided radiotherapy.

To improve tumor destruction and minimize adverse effects to healthy tissues, image-guided radiation therapy (IGRT) has been developed to allow for the accurate delivery of radiation energy to tumor sites facilitated by real-time imaging. Nevertheless, the current IGRT platform still suffers from the limitation of poor tissue contrast, resulting in the incidental irradiation of healthy tissue. Gold nanoparticles (GNPs) have been identified as promising candidates to simultaneously improve both radiotherapy and imaging, thereby improving both the accuracy and safety of IGRT. However, despite mu
