The permanent lesion of damaged recurrent laryngeal nerve (RLN) often manifests as an irreversible dysfunction of phonation and is the most common complication following thyroid surgery. The frequency of this complication ranges from 0.5 to 5% in different thyroid surgery centres and increases in case of both recurrent goiter and complete thyroidectomy due to thyroid cancer. The RLN lesion varies from irreversible, persistent and transient dysfunction with good prognosis of complete recovery. The time of RLN function recovery ranges from several weeks to two years (mainly 6 months). The aim of

Etiologies of adult vocal paralysis are varied by the site of the lesion as well as the extent and cause of the damage. Most large series point to surgery and neoplastic causes for recurrent nerve paralysis. A detailed history is important when working up a patient with this voice disorder. Knowledge of the anatomy of the head, neck, and chest as well as the mechanisms behind vocal fold paralysis is essential in the evaluation and treatment of recurrent nerve paralysis. Many of the surgical and traumatic causes of hoarseness are from compression type injuries. Recovery is dependent on the type

Abductor, adductor, and combined reinnervation procedures have been explored with variable success rates. We describe the experience of a tertiary care center with adductor reinnervation procedures, including preoperative and postoperative videostroboscopy and electromyography (EMG) findings. A retrospective chart review was performed from 1997 to 2001 that included 9 patients. Preoperative and postoperative voice comparison was performed by 3 blinded speech pathologists. Clinical comparisons of videostroboscopy findings for vocal fold bulk, tone, position, presence of gap, and movement are el

Intraoperative recurrent laryngeal nerve identification is sometimes difficult in reoperative cervical dissection or operation for inflammatory thyroid disorders. Three modalities have been described to intraoperatively assess nerve function: vocal cord visualization with fiberoptic bronchoscopy or direct laryngoscopy, electromyelographic surveillance of arytenoid muscle function, and cord function assessment with an electromyelogram-electrode endotracheal tube. Our study focused on patients requiring cervical dissection for thyroid or parathyroid disease in which intraoperative recurrent lary

OBJECTIVE: To study factors related to recurrent laryngeal nerve (RLN) paralysis, a major complication of thyroidectomy.
METHOD: Retrospective study of outcome of patient's medical records in Xijing Hospital and Tumor Hospital of Liaoning Province. Records of 1,563 patients who underwent thyroidectomy by departments of general surgery, ENT and head & neck surgery at these two hospitals were reviewed for RLN paralysis.
MAIN OUTCOME MEASURES: RLN injury was analyzed in relation to types of surgery, RLN identification, and histopathology.
RESULT: The incidence of RLN paralysis was 7.8%, and si

Cricotracheal resection has been advocated in the management of severe subglottic stenosis. One of the possible complications of this procedure is injury to the recurrent laryngeal nerve (RLN). We describe a new technique in which electrodes are placed directly through the thyroid cartilage to monitor the RLN intraoperatively. Nine cats' left vocal cords were monitored, and 3 cats had unilateral vocal cord injury postoperatively: 1 right cord and 2 left cords. Even though this technique was efficacious, our ability to monitor the RLNs was difficult secondary to difficulty interpreting the nerv

Recurrent laryngeal nerves (RLNs) from human autopsy material were collected and prepared for light microscopy. Within five of eight investigated RLNs, paraganglia-like organs were found, the mean cross-sectional area of which was estimated to be 0.03 mm2. These organs were composed of cells resembling the type I and type II cells of the carotid body. The possible function of laryngeal nerve paraganglia is discussed.

Spastic dysphonia is a severe vocal disability in which a person speaks with excessively adducted vocal cords. The resulting weak phonation sounds tight, as if he were being strangled, and has also been descrived as laryngeal stutter. It is often accompanied by face and neck grimaces. In the past it has been regarded as psychoneurotic in orgin and treated with speech therapy and psychotherapy with disappointing results. Because of laboratory and clinical observation that recurrent nerve paralysis retracts the involved vocal cord from the midline, it was proposed that deliberate section of the

Generally, less than half of neurogenic tumors are found in the head and neck region. Within that distinction, the majority of them originate from a branch of the superior laryngeal nerve in the neck. Schwannomas are benign tumors arising from Schwann cells, which form the myelin sheath of peripheral nerves. In the medical literature, evidence of recurrent laryngeal nerve schwannomas is sparse. This report describes the case of a 77-year-old woman with a history of multinodular goiter and primary hyperparathyroidism. During parathyroid exploration, an isolated schwannoma was incidentally found

BACKGROUND: In thyroidectomy, little has been reported on the differential recurrent laryngeal nerve (RLN) palsy rates between the left and right sides. Even less is known about the potential differences causing these differential rates. This study reports the left versus right RLN palsy rates of total thyroidectomy cases in a single institution, relating them to the comparative stiffness of the left and right porcine RLNs. Computed stress modelling was also used to estimate the differential levels of tension within each RLN.
METHODS: For the comparison of the left and right RLN palsy rates,

INTRODUCTION: There have been several reports on the feasibility and curability of thoracoscopic esophagectomy, which may reduce injury to the thoracic cage and decrease the invasiveness of surgery. Although the recurrent laryngeal nerve (RLN) is identified and kept intact during operations, RLN palsy sometimes occurs. Currently, surgical aides, including intraoperative neurological monitoring, are being utilized to avoid RLN injury during thyroid surgery. This system is utilized during thoracoscopic esophagectomy in the prone position.

UNLABELLED: Recurrent laryngeal nerve paralysis is a much-dreaded complication of thyroid surgery.
OBJECTIVE: To study the feasibility and the reliability of a recurrent laryngeal nerve monitoring technique.
MATERIALS AND METHODS: This was a prospective study including 36 patients proposed for thyroid surgery with recurrent laryngeal nerve monitoring. They underwent post-operative fibre-optic laryngeal examination and speech therapist consultation.
RESULTS: Our technique of nerve monitoring showed a 98 per cent sensitivity and 86 per cent specificity.
CONCLUSIONS: Recurrent laryngeal nerve

INTRODUCTION: Vocal cord paralysis is a important complication in thyroid gland surgery. A prospective study was conducted ascertain the frequency of laryngeal nerve palsy not caused by surgical trauma.
PATIENTS AND METHODS: Two hundred and ten patients were investigated laryngoscopically pre- and postoperatively after short-term intubation in the course of operations far removed from thorax or neck region. We noted the inner diameter of the tube, intubation problems, the qualification of the anaesthesiologist and the positioning of the patient.
RESULTS: Preoperatively we found previously un

A new "all in one" sensing device for tracing and continuous intraoperative monitoring of the recurrent laryngeal nerve during thyroid surgery is described. The system, based on a double ballooned endotracheal tube, is atraumatic, easy to use and sensitive even to imminent trauma to the nerves. The most striking feature of this instrument is that it operates outside the sterile operating field and truly monitors continuously throughout the operation.

Recurrent laryngeal nerve dysfunction is a significant complication of carotid endarterectomy and vocal cord paralysis is a major source of morbidity. This study prospectively assessed patients undergoing carotid endarterectomy to determine the nature and frequency of vocal cord damage and attempt to identify avoidable factors. Fifty consecutive patients undergoing carotid endarterectomy for symptomatic disease were studied. A standardized surgical technique was used emphasizing identification of the vagus nerve and minimal disturbance of the surrounding tissues. All patients underwent pre-ope
