[Prediction of idiopathic respiratory insufficiency using the gastric aspirate shake test].

To verify the usefulness of the gastric aspirate shake test as a predictor of idiopathic respiratory distress syndrome, we studied 110 premature newborns with respiratory failure to define the surfactant deficiency (negative test) or surfactant sufficiency (positive test). The results were correlated with definitive diagnosis at discharge. Our findings indicate that the gastric aspirate shake test has 97.5% sensitivity, 77.1% specificity 70.9% predictive value for surfactant deficiency, and 98.2% predictive value for surfactant sufficiency. Therefore it was considered useful to predict the idi

Predicting respiratory distress by thin-layer chromatography of the newborn gastric aspirate.

The analysis of gastric fluid from newborns for lecithin/sphingomyelin (L/S) ratio evaluation is useful in determining whether respiratory distress will follow an early delivery. Without exception, the L/S ratios obtained on 14 gastric aspirates paralleled and were slightly higher than intrapartum values obtained from corresponding amniotic fluids (5.6 vs 3.7). Of 46 normal infants, only 1 had a gastric aspirate L/S ratio less than 2:1. No false positive results were found. The obstetrician can easily obtain gastric fluid at delivery, and by ordering an L/S ratio determination, the neonatologi

An association of gastroschisis and fatal respiratory distress: does prenatal bile aspiration cause early-onset respiratory failure in neonates?

We present a neonate with gastroschisis and evidence of bile aspiration in utero, who developed severe respiratory distress that did not respond to postnatal intensive respiratory care. Although rare, a newborn with gastroschisis may develop severe respiratory distress due to bile aspiration in utero. Given the poor outcome in this case, we suggest a possible role for prenatal diagnosis and therapy.

Aspiration, respiratory complications, and associated healthcare resource utilization among individuals with Rett syndrome.

BACKGROUND: Individuals with Rett syndrome (RTT) are at high risk for aspiration and also experience high rates of lower respiratory tract infections (LRTI) and respiratory failure (RF). METHODS: A retrospective comparative cohort analysis was performed among 89 individuals with RTT with and without evidence of aspiration, using EHR structured and abstracted clinical notes data. Individuals with known or suspected aspiration (per clinical documentation) (cases) were compared to controls on aspiration risk factors, RF, LRTI, and hospitalization. RESULTS: Of eligible individuals, 25 (28.1%) we

[A clinical study of respiratory infection isolating non-pathogenic Neisseria by transtracheal aspiration].

Neisseria species other than N. meningitidis and N. gonorrhoeae are generally regarded as commensal bacterial flora of the oropharynx, and little is known regarding cases of these non-pathogenic Neisseria species in the lower respiratory tract. We clinically examined respiratory tract infections from which non-pathogenic Neisseria species were isolated by transtracheal aspiration (TTA). The incidence of non-pathogenic Neisseria isolated was 54 (15.7%) out of 344 episodes of respiratory tract infections with isolated microorganisms from TTA, and was 17.6%, 15.8%, 14.3% for pneumonia, acute bron

[Animal model of acute respiratory insufficiency by HCl aspiration in Lewe mini-pigs].

Acute respiratory insufficiency or adult respiratory distress syndrome is a common medical emergency in intensive medicine complicated by high mortality. To study the acute respiratory insufficiency under standardized conditions an animal model in Lewe-mini-pigs has been developed. This model is based on aspiration pneumonitis produced by intrabrochial atomisation of 0.2 N hydrochloric acid. General anesthesia was performed by neuroleptanalgesia. In all animals the profound changes in gas exchange, pulmonary mechanics and hemodynamics as well as quasi-static volume-pressure relationships after

Exploring the role of respiratory virus infections in aspiration pneumonia: a comprehensive analysis of cases with lower respiratory tract infections.

BACKGROUND: While respiratory virus infection has been implicated in the onset of bacterial pneumonia, no research has investigated the association of respiratory viruses with the onset of aspiration pneumonia (AP). This study aimed to investigate the role of respiratory virus infections in AP. METHODS: Patients presenting with acute respiratory symptoms and undergoing influenza antigen testing at the emergency department of Okinawa Chubu Hospital from February 2020 to January 2021, and diagnosed with lower respiratory tract infections, were included. Cases were categorized into AP, pneumonia

Gastric Aspiration and Ventilator-Induced Model of Acute Respiratory Distress Syndrome in Swine.

INTRODUCTION: Mainstays of current treatment for acute respiratory distress syndrome (ARDS) focus on supportive care and rely on intrinsic organ recovery. Animal models of ARDS are often limited by systemic injury. We hypothesize that superimposing gastric aspiration and ventilator-induced injury will induce a lung-specific injury model of severe ARDS. MATERIALS AND METHODS: Adult swine (n = 8) were subject to a 12 h injury development period followed by 24 h of post-injury monitoring. Lung injury was induced with gastric secretions (3 cc/kg body weight/lung, pH 1-2) instilled to bilateral ma

Nasopharyngeal Swabs vs. Nasal Aspirates for Respiratory Virus Detection: A Systematic Review.

Nasal pathogen detection sensitivities can be as low as 70% despite advances in molecular diagnostics. This may be linked to the choice of sampling method. A diagnostic test accuracy review for sensitivity was undertaken to compare sensitivity of swabbing to the nasopharynx and extracting nasal aspirates, using the PRISMA protocol, Cochrane rapid review methodology, and QUADAS-2 risk of bias tools, with meta-analysis of included studies. Sensitivities were calculated by a consensus standard of positivity by either method as the 'gold standard.' Insufficient sampling methodology, cross sectiona

Acute Respiratory Distress Syndrome immediately following the removal of an aspirated foreign body.

Acute Respiratory Distress Syndrome (ARDS) is a form of diffuse lung injury with many potential etiologies, pneumonia being the most common cause developing outside of the hospital. Foreign body (FB) aspiration is a risk factor for pneumonia, and therefore, ARDS. Although these associations exist, the development of ARDS immediately following the removal of an aspirated FB appears quite rare. We present the case of an 11 year old male who was found to have a right-sided, post-obstructive pneumonia secondary to an aspirated FB obstructing the bronchus intermedius. Relief of the obstruction allo

Comparison of nasopharyngeal aspirate with flocked swab for PCR-detection of respiratory viruses in children.

Fast- and high-throughput molecular workflows require sample matrices to be suitable for automation. Respiratory swabs are better suited for this purpose compared to the more viscous nasopharyngeal aspirates. Samples collected by nasopharyngeal aspiration and nasopharyngeal flocked swab from 81 children were compared for detection and recovery of respiratory viruses. Using real-time RT-PCR, no statistically significant differences in virus detection between the two sample types were found, supporting the use of flocked swabs in children aged one month to two years.

Clinical and radiological characteristics of unrecognised foreign body aspiration into respiratory tract in children.

INTRODUCTION: Diagnosing of foreign body aspiration in children is often postponed. The aim of this study was to evaluate complications, clinical signs and radiological presentation of respiratory illness arising from unrecognized foreign body aspiration in pediatric population. MATERIAL AND METHODS: The study sample consisted of 33 patients aged from one to 18 years who had undergone bronchoscopy for foreign body removal over the 10 years' period (from 2000 to 2010). Neither their parents nor the pediatricians who had treated these patients before admission to hospital recognized the acute a

[A successfully treated case of aspiration pneumonia with adult respiratory distress syndrome and shock].

Right hip replacement was scheduled for a 74-year-old man who was treated with morphine for cancer pain. As the patient developed dyspnea and hypoxia after anesthesia, he was intubated and kept under mechanical ventilation. A diagnosis of aspiration pneumonia with adult respiratory distress syndrome was made based on the detection of gall obtained from the endotracheal tube. A chest X-ray showed pulmonary edema. He was treated with positive pressure ventilation and inotropic support. As he developed severe shock 10 hours after the intubation, he was treated with 20 mg.kg-1 of methylprednisolon
