Recently, an increase in the number of operations on the lacrimal ducts performed by otorhinolaryngologists has been noted throughout the world. During these operations, intra- and postoperative complications may develop, which require the intervention of an ophthalmologist. A rare complication of endoscopic dacryocystorhinostomy is the development of retrobulbar hemorrhage. This article presents a clinical case of the development of retrobulbar hemorrhage in endoscopic dacryocystorhinostomy. The patient with dacryocystitis underwent endoscopic dacryocystorhinostomy. Preoperative computed tomo

Retrobulbar injection has been widely practiced as a technique of ocular anesthesia for many decades. Nevertheless, the technique is not free from complications. Vascular occlusion secondary to retrobulbar injection is rare but can be vision threatening. We report a case series of two such patients who presented with poor vision following retrobulbar injection. Fundus showed pale retina with cherry red spot suggestive of central retinal artery occlusion in case 1 and pale disc with sclerosed vessels and multiple superficial hemorrhages suggestive of a combined occlusion of retinal artery and v

The surgical treatment of orbital floor fracture, a common facial injury, is not a risk-free procedure. Complications after orbital surgery can include infection, implant migration, mydriasis, epiphora, persistent diplopia, enophthalmos, infraorbital numbness, retrobulbar hemorrhage, and blindness. Blindness has been ascribed to retrobulbar hematoma in almost 50% of cases. In our experience, blood collection above the polydioxanone sheet after the treatment of orbital floor fracture can be caused by the tight adhesion of the sheet to the bony edges of the fracture. Here, we present a simple pr

To evaluate and compare intraoperative pain, perioperative complications and hemodynamic changes during phacoemulsification under topical and retrobulbar anesthesia. A total of 564 patients were randomly allocated into topical and retrobulbar groups. All patients received 2 mcg kg(-1) fentanyl 5 min before the start of their procedures. Patients in the topical group were treated with tetracaine 0.5% eye drops and given preservative free lidocaine 2% intracamerally during surgery. Patients in the retrobulbar group received a 4 mL lidocaine 2% into the retrobulbar space. The number of complicati

A 6-month-old intact female mixed-breed kitten presented with severe exophthalmos of the left eye. Periocular lesions, including subconjunctival haemorrhage, third eyelid protrusion, and left eyelid oedema, were detected in the absence of globe retropulsion. The left intraocular pressure was increased, and ocular ultrasonography revealed ipsilateral retrobulbar fluid. Coagulation panels were markedly prolonged and severe anaemia was detected. Ultrasound-guided retrobulbar centesis performed to decrease intraocular pressure yielded blood. Based on the history and clinical findings, anticoagulan

We commend Narjus-Sterba et al. for their significant contribution in reporting the prevalence and consequences of retrobulbar hematoma (RBH) in a large cohort of patients with orbital fractures. Their work draws attention to a vision-risking but under-emphasized complication and underscores the utility of CT scan and pupillary reflex as early indicators of visual prognosis. Nevertheless, several aspects warrant further discussion. Of particular note is the failure to stratify outcomes by exact time-to-treatment intervals, lack of long-term visual follow-up in a considerable number of patients

Retrobulbar haematoma (RBH) is a rare complication that may affect vision after a trauma or a surgical procedure. The diagnosis must be made promptly, as only early surgical intervention can adequately prevent irreversible visual impairment. Because of the bony orbital walls, there is hardly any room for the increasing intraorbital volume due to the retrobulbar haemorrhage. This leads to an increase in intraorbital pressure and subsequently to compression of the optic nerve. Symptoms include disorders in ocular motility, ophthalmoplegia, diplopia, conjunctival chemosis, subconjunctival haemorr

Point-of-care ultrasound has become an integral part of the evaluation of monocular vision loss. Most commonly, it has been used to evaluate retinal detachment, vitreous hemorrhage, and posterior vitreous detachment. Point-of-care ultrasound can also be used to evaluate central retinal arterial occlusion, whereby a retrobulbar spot sign is present. We present a case series of 4 patients presenting with monocular vision loss who were found to have central retinal artery occlusion. We describe what a retrobulbar spot sign is and how its presence or absence can assist in the evaluation and treatm

Valsalva-associated orbital compartment syndrome in the setting of orbital cellulitis, mucocele, or subperiosteal abscess has not been previously reported. A previously healthy girl presented with orbital cellulitis complicated by a subperiosteal abscess and frontoethmoidal mucocele. On the day of her planned orbitotomy and endoscopic sinus surgery, she developed a Valsalva-associated retrobulbar hemorrhage and elevated intraocular pressure after crying during a blood glucose fingerstick. An urgent canthotomy and cantholysis in addition to the planned endoscopic sinus procedure was performed.

CASE DESCRIPTION: We present a case of a 65-year-old patient who underwent heart transplantation. After the surgery, left proptosis, conjunctival chemosis, and ipsilateral palpebral ecchymosis were found while he was still intubated. A retrobulbar hematoma was suspected, confirmed by a computed tomography scan. Initially, expectant management was considered, but with the appearance of an afferent pupillary defect, the patient underwent orbital decompression and posterior collection drainage, which prevented visual impairment.
CONCLUSION AND IMPORTANCE: Spontaneous retrobulbar hematoma after h

A case of large fronto-temporo-parietal epidural hematoma associated with ipsilateral retrobulbar hematoma is reported. A 24-year-old man soon after a head injury due to a traffic accident became comatose with anisocoria and hemiplegia and developed exophthalmos, conjuctival chemosis, downward and lateral displacement of the eyeball. CT scan of the brain and the orbit showed the large epidural clot communicating with an ipsilateral retrobulbar hematoma through a sphenoid bone fracture. The patient underwent emergency surgery. Postoperatively, he improved and was discharged in good conditions.

A 5-year-old Doberman Pinscher dog was presented with sudden onset left-sided periocular bleeding following third eyelid gland replacement surgery. Left-sided exophthalmos and 360-degrees subconjunctival hemorrhage were present. The ophthalmic examination revealed blindness with absent direct and consensual pupillary light reflexes. A superficial exposure ulcerative keratitis due to exophthalmos was also present. Computed tomography (CT) revealed a large left-sided retrobulbar mass lesion surrounding the optic nerve, compatible with a retrobulbar hematoma. Due to absence of orbital fractures a

BACKGROUND: Cataract extraction is among the most commonly performed surgical procedures and is widely recognized for its high safety profile. With advances in surgical methods, small incision cataract surgery (SICS) now allows the use of shorter-acting and minimally invasive anesthetic techniques. The present study aimed to evaluate and compare the efficacy of peribulbar and sub-Tenon's anesthesia with respect to analgesic effectiveness, ocular akinesia, and anesthesia-related complications such as subconjunctival hemorrhage, chemosis, retrobulbar hemorrhage, globe perforation, and brainstem
