Towards an integrated model for breast cancer etiology: the lifelong interplay of genes, lifestyle, and hormones.

While the association of a number of risk factors, such as family history and reproductive patterns, with breast cancer has been well established for many years, work in the past 10-15 years also has added substantially to our understanding of disease etiology. Contributions of particular note include the delineation of the role of endogenous and exogenous estrogens to breast cancer risk, and the discovery and quantification of risk associated with several gene mutations (e.g. BRCA1). Although it is difficult to integrate all epidemiologic data into a single biologic model, it is clear that se

Epistemic Risk Reduction in Psychedelic-Assisted Therapy.

Belief change is crucial to therapeutic benefit in psychedelic-assisted therapy as well as in more traditional forms of therapy. However, the use of psychedelics comes with a few unique challenges that require extra caution. First, drastic belief changes may occur faster than in regular therapy. Facing radical and transformative insights all at once rather than through a gradual process of discovery and integration can lead patients to a volatile, confusing or disorienting epistemic state. Additionally, we know psychedelic substances generate hallucinatory experiences that come with a high deg

Change in obesity-related metabolic abnormalities associated with body mass index improvement through life-style intervention: A meta-regression.

The reduction in body mass index standard deviation score (BMI-SDS) associated with improvement in biomarkers relating to metabolic health in obese children is unknown. We aimed to establish the change in BMI-SDS associated with improved inflammation, liver function, and insulin resistance to inform clinical guidelines for pediatric weight management interventions and to assess the efficacy of future trials. A large-scale systematic review was conducted to identify relevant studies. Studies of children with a diagnosis of obesity according to defined BMI thresholds, participating in lifestyle

Population-level interventions for dementia prevention: a systematic review.

Nearly half of dementia cases are potentially preventable through modifiable risk factors, yet few population-level interventions have been evaluated. This systematic review identified 12 studies from eight countries (Australia, Belgium, Chile, China, Denmark, the Netherlands, Puerto Rico, and the USA; sample size: 51-8360) evaluating population-level interventions focused on communicating dementia-specific risk reduction and prevention messaging. Intervention effectiveness varied. Mass media campaigns achieved broad reach but only marginal knowledge gains. Educational and interactive interven

[The principles of nutrition therapy of gout and hyperuricemia].

In 2012 the updated guidelines for the treatment of gout were published by the American College of Rheumatology (ACR), which underline the necessity of comprehensive treatment in this condition. According to the European League Against Rheumatism (EULAR), treatment of comorbidities, lifestyle modifications and proper diet may be of significant importance. In the primary care settings in Poland there is virtually no access to qualified dieticians. The nutritional counseling is sometimes implemented--usually to a very limited extent--by primary care physicians. It is believed that proper diet ma

Dissonance-based interventions for health behaviour change: a systematic review.

PURPOSE: Increasing evidence suggests that various health behaviours are amenable to change following the induction of cognitive dissonance. This systematic review sought to evaluate the effectiveness and methodological quality of dissonance-based health behaviour interventions and to explore identified sources of heterogeneity in intervention effects. METHODS: Bibliographic databases were searched for relevant articles from inception to March 2012. Only studies targeting non-clinical health behaviour in non-clinical populations were included in the review. One author extracted data and asses

The principles of Population-Level Approaches to Dementia Risk Reduction (PLADRR).

Dementia is a leading health policy challenge, with cases expected to triple by 2050, particularly in low- and middle-income countries. Epidemiological evidence demonstrates falling age-specific incidence rates in high-income countries, suggesting risk can be lowered at the population level. The Population-Level Approaches to Dementia Risk Reduction (PLADRR) Research Group is a diverse, international network of researchers committed to investigating how structural, social, and environmental conditions can promote life course brain health and reduce dementia risk across the population. This Polic

Obesity management--an opportunity for cancer prevention.

There is increasing evidence of an association between obesity and the development, morbidity and mortality of cancers of the colorectum, (post menopausal) breast, endometrium, kidney, pancreas and oesophagus. In addition to obesity per se, waist circumference is now emerging as a clear indicator of disease risk. Weight gain during adult life also appears to increase risk for breast and colon cancers. Major causative factors which are influenced by excess energy storage include hormones involved in metabolic control (insulin and leptin), cell growth (IGF-I and IGF-binding proteins) and reprodu

Pulses and Cancer Outcomes: A Scoping Review of Human Studies on Risk Reduction.

Background/Objectives: Pulses are nutrient-dense, low-glycaemic legumes rich in fibre and bioactive compounds that may modulate carcinogenesis through effects on diet quality, metabolism, and the gut microbiome. This scoping review mapped human evidence on pulses in relation to cancer risk reduction and related mechanistic and survivorship-relevant outcomes. Methods: Following the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) and Joanna Briggs Institute (JBI) Population, Concept and Context (PCC) guidance, we searched CENTRAL, Sco

Value-Added Electrodiagnostics: Targeting Interventions for Fall Risk Reduction.

Walking confers numerous health benefits, particularly for middle-aged and older patients with diabetes and metabolic syndrome. Nevertheless, it brings a risk of injurious falls, especially among populations with diabetes and metabolic syndrome-related distal neuromuscular decline and frank neuropathy. Those who stand to benefit most from walking are at greatest risk. Development of practical clinical tools to more precisely quantify neuromuscular function and link it to mobility outcomes will help clinicians target interventions toward those at risk for falls. Electrodiagnosis, with inclusion

A Systematic Review and Meta-analysis of Psychosocial Interventions to Reduce Drug and Sexual Blood Borne Virus Risk Behaviours Among People Who Inject Drugs.

Opiate substitution treatment and needle exchanges have reduced blood borne virus (BBV) transmission among people who inject drugs (PWID). Psychosocial interventions could further prevent BBV. A systematic review and meta-analysis examined whether psychosocial interventions (e.g. CBT, skills training) compared to control interventions reduced BBV risk behaviours among PWID. 32 and 24 randomized control trials (2000-May 2015 in MEDLINE, PsycINFO, CINAHL, Cochrane Collaboration and Clinical trials, with an update in MEDLINE to December 2016) were included in the review and meta-analysis respecti

Harm Reduction and electronic gambling machines: does this pair make a happy couple or is divorce foreseen?

Recent empirical studies have evaluated if modifying electronic gambling machine (EGM) structural features could encourage safer gambling behaviors and decrease gambling-related problems. Several of these studies refer to Harm Reduction (HR), suggesting that the HR paradigm is useful to design, implement and test the efficacy of various prevention and treatment programs applied to EGM users. After reviewing the origins of HR and specifying its operational definition, this paper discusses the relevance of the HR framework for the study of measures related to EGM use and gambling in general. [Psychological treatment for obesity].

Obesity is a chronic disease. Current management is based in the modification of the lifestyle, mainly regarding to eating habits and physical activity. Eating habits are acquired during the childhood and kept through the entire life. Modification of any habit requires the use of specific psychological methods such as cognitive-behavioural therapy, which is based on first notions of learning theories. Very often, obese patients and therapist think that the obesity is a problem not related to other aspects of their lives. Thus the objective of the treatment is only weight loss, instead of looki

Heart Health and Behavior Change in HIV-Infected Individuals.

Management of cardiovascular disease risk requires many lifestyle changes involving diet, smoking, and exercise. Individuals with arterial plaque are encouraged to adopt these changes to promote longevity through a variety of interventions. This study examined behavioral changes in response to the standard of care after detection of arterial plaque, specifically among HIV-infected cocaine users. 127 individuals (HIV - COC - n = 43, HIV + COC - n = 19, HIV + COC + n = 35, HIV - COC + n = 30) were followed after a standard of care intervention and assessed 1 and 2 years later on a variety of lif

Controversies in the prevention of spontaneous preterm birth in asymptomatic women: an evidence summary and expert opinion.

Preterm birth prevention is multifaceted and produces many nuanced questions. This review addresses six important clinical questions about preterm birth prevention as voted for by members of the UK Preterm Clinical Network. The questions cover the following areas: preterm birth prevention in 'low-risk' populations; screening for asymptomatic genital tract infection in women at high risk of preterm birth; cervical length screening with cerclage or vaginal pessary in situ; cervical shortening whilst using progesterone; use of vaginal progesterone in combination with cervical cerclage; and optima

Reducing risk of head injury in youth soccer: An extension of behavioral skills training for heading.

Recently, concerns regarding sport-related concussions have increased within the research literature, the media, and popular culture. One potential source of soccer-related concussions involves the purposeful striking of the ball with one's head (i.e., heading). There is currently limited research on an effective teaching method to improve safe heading technique. In the current study, Behavior Skills Training (BST) was evaluated as a method to teach correct heading techniques to youth soccer players. BST increased the percentage of correct steps for each player based on a task analysis of head
