Adverse events during continuous renal replacement therapy in critically ill patients: A retrospective cohort study.

OBJECTIVE: The objective of this study was to identify risk factors for adverse events (AEs) occurring during continuous renal replacement therapy (CRRT) in critically ill patients. METHOD: A retrospective cohort study was conducted in an intensive care unit of a private hospital in Sao Paulo, Brazil, with patients over 18 years of age, admitted between 2018 and 2023. Demographic, clinical, and AE data were analysed. Negative binomial regression was used to identify risk factors. A p value ≤0.05 was considered significant. RESULTS: A total of 1.361 patients underwent CRRT, and, from these, 1

[The course and outcome of acute kidney failure in patients treated by hemodialysis].

In 1989-1995 229 patients with acute renal failure were treated by hemodialysis. A total of 1470 procedures have been performed, 8.8 +/- 1.5, on the average. 61 patients (26.6%) died. There is no significant relation between duration of oliguria, maximum BUN, creatinine level and lethality. The latter is associated with the patient's age, acute respiratory failure, sepsis, coma, hyperbilirubinemia and hypoproteinemia. Lethality was higher at failure of two and more organs and poor prognosis defined by a simplified acute physiology score (SAPS). 10 patients (5.9%) were discharged with increased

A prospective observational study to correlate lung ultrasound with clinical severity and prognosis score in patients with primary pulmonary pathology on invasive ventilatory support.

BACKGROUND: Lung ultrasound (LUS) is a known imaging modality employed for monitoring patients in an intensive care unit. This study evaluates, LUS in assessing disease severity and prognosis, by correlating its score with the three commonly used clinical severity scoring systems (CSSS), namely, sequential organ failure assessment (SOFA) score, acute physiology and chronic health evaluation (APACHE) II score, and simplified acute physiology score (SAPS) II. METHODS: This single-center prospective observational study included 54 adult patients of primary lung disease-induced acute respiratory

Factors associated with death and readmission into the Intensive Care Unit.

This study identifies the factors associated with death and readmission into the Intensive Care Unit. This is a longitudinal prospective study of 600 adult patients admitted in general Intensive Care Units of four hospitals in São Paulo, Brazil. The multiple regression analysis showed that patients with a longer length of stay and higher Nursing Activities Score, Simplified Acute Physiology Score II and Logistic Organ Dysfunction Score on the last day in the unit had a higher chance of death. Nevertheless, higher Nursing Activities Score and Logistic Organ Dysfunction Score on the first day in

Altered lower brainstem neurophysiological response is associated with mortality in deeply sedated critically ill patients.

BACKGROUND AND OBJECTIVES: Absent cough reflex is associated with mortality in intensive care unit (ICU) patients requiring deep sedation, suggesting that lower brainstem dysfunction contributes to adverse outcomes. We conducted a multicenter observational cohort study to confirm this hypothesis by assessing the peak latency (PL) of the lower brainstem-generated P14 evoked potential (EP), which is slightly increased by sedatives. We aimed to demonstrate that a P14-PL > 16 ms is independently associated with day-28 mortality. PATIENTS AND METHODS: Mechanically ventilated adult patients, comato

Copeptin levels and commonly used laboratory parameters in hospitalised patients with severe hypernatraemia - the "Co-MED study".

BACKGROUND: Hypernatraemia is common in inpatients and is associated with substantial morbidity. Its differential diagnosis is challenging, and delayed treatment may have devastating consequences. The most important hormone for the regulation of water homeostasis is arginine vasopressin, and copeptin, the C-terminal portion of the precursor peptide of arginine vasopressin, might be a reliable new parameter with which to assess the underlying cause of hypernatraemia. METHODS: In this prospective, multicentre, observational study conducted in two tertiary referral centres in Switzerland, 92 pat

Hyperoxemia as a risk factor for ventilator-associated pneumonia.

BACKGROUND: Consequences of hyperoxemia, such as acute lung injury, atelectasis, and reduced bacterial clearance, might promote ventilator-associated pneumonia (VAP). The aim of our study was to determine the relationship between hyperoxemia and VAP. METHODS: This retrospective observational study was performed in a 30-bed mixed ICU. All patients receiving invasive mechanical ventilation for more than 48 hours were eligible. VAP was defined using clinical, radiologic, and quantitative microbiological criteria. Hyperoxemia was defined as PaO2 > 120 mmHg. All data, except those related to hyper

Associations between YKL-40 and markers of disease severity and death in patients with necrotizing soft-tissue infection.

BACKGROUND: Necrotizing soft-tissue infection (NSTI) is a severe and fast-progressing bacterial infection. Prognostic biomarkers may provide valuable information in treatment guidance and decision-making, but none have provided sufficient robustness to have a clinical impact. YKL-40 may reflect the ongoing pathological inflammatory processes more accurately than traditional biomarkers as it is secreted by the activated immune cells, but its prognostic yields in NSTI remains unknown. For this purpose, we investigated the association between plasma YKL-40 and 30-day mortality in patients with NS

Determinants of the calibration of SAPS II and SAPS 3 mortality scores in intensive care: a European multicenter study.

BACKGROUND: The aim of the Simplified Acute Physiology Score (SAPS) II and SAPS 3 is to predict the mortality of patients admitted to intensive care units (ICUs). Previous studies have suggested that the calibration of these scores may vary across countries, centers, and/or characteristics of patients. In the present study, we aimed to assess determinants of the calibration of these scores. METHODS: We assessed the calibration of the SAPS II and SAPS 3 scores among 5266 patients admitted to ICUs during a 4-week period at 120 centers in 17 European countries. We obtained calibration curves, Br

Lung Injury Etiology and Other Factors Influencing the Relationship Between Dead-Space Fraction and Mortality in ARDS.

BACKGROUND: In ARDS, elevated pulmonary dead-space fraction (VD/VT) is a particularly strong indicator of mortality risk. Whether the magnitude of VD/VT is modified by the underlying etiology of ARDS and whether this influences the strength of its association with mortality remains unknown. We sought to elucidate the impact of ARDS etiology on VD/VT and also to determine whether ARDS severity, as classified by the Berlin definition, has correspondence with changes in VD/VT. METHODS: This single-center, retrospective, observational study (2010-2016) measured VD/VT in 685 subjects with ARDS as

Sepsis and organ failure are the major risks for allogeneic transplant patients in the post- or late- engraftment phase admitted to the ICU.

OBJECTIVES: Allogeneic hematopoietic stem cell transplantation (allo-HSCT) can lead to complications, depending on the period of the transplantation. Our study aimed to evaluate the outcome and prognostic parameters for the survival of patients admitted to the intensive care unit (ICU) in the post- or late-engraftment phase. METHODS: The survival of patients aged ≥ 18 years in the post- or predominantly late-engraftment phase was analyzed retrospectively by using baseline data, laboratory parameters, scoring systems and vital parameters. RESULTS: 46 cases were recorded. Sepsis-related respir

Scoring of severity in patients admitted to a respiratory intensive care unit.

A number of attempts have been made to develop measures of severity of illness for patients in the intensive care unit (ICU), but the impact of these indices in a Respiratory Intensive Care Unit (RICU) has not really been studied. At present, outcome can be accurately predicted in approximately 85% of ICU patients, while no data are available for the RICU. These indices will be reviewed in the article. The sophisticated Mortality Probability Model (MPM) II is used to predict the outcome of ICU patients. This technique is based on statistically-derived weights for its variables. The "most popul

Utility of SpO2/FiO2 ratio for acute hypoxemic respiratory failure with bilateral opacities in the ICU.

Acute hypoxemic respiratory failure (AHRF) with bilateral opacities causes fatalities in the intensive care unit (ICU). It is often difficult to identify the causes of AHRF at the time of admission. The SpO2 to FiO2 (S/F) ratio has been recently used as a non-invasive and alternative marker of the PaO2/FiO2 (P/F) ratio in acute respiratory failure. This retrospective cohort study was conducted from October 2010 to March 2019 at the Showa University Hospital, Tokyo, Japan. We enrolled 94 AHRF patients who had bilateral opacities and received mechanical ventilation in ICU to investigate their pr
